Social Workers with OCL ensure a coordinated system to meet the needs of at-risk elderly and disabled individuals to support community living and prevent institutionalization. Responsibilities include eligibility determination, care planning, home visits, development and implementation of individualized care plans, home visits, and accurate documentation using web-based case management software.
