* Provides professional nursing services to patients, clients, and staff in chronic as well as acute care settings.
* Performs nursing assessment, diagnosis, treatment & evaluation of ill and disabled persons.

PHYSICAL WORKING CONDITIONS:

Performs wound assessment, reassessment, and management of wound patients per policy.
Utilizes principles of current wound management theory when providing care to patients with acute and chronic wounds.

Primary Responsibilities
Utilize broad-based clinical knowledge to assess, implement health promotion and prevention interventions, manage acute and chronic illness states, and assist patients/residents in attaining optimal levels of functioning and rehabilitation.
Evaluate patient/resident responses to actual or potential health problems and provide supportive measures for dying patients/residents and their significant others.

Provide patient/resident-centered care by assessing healthcare needs, developing plans of care, and performing/assigning delegated tasks.
Collect and document pertinent data into the electronic medical record, complete nursing assessments within required time frames, and act on significant changes in patient/resident health conditions.
Update patient/resident care plans, notify physicians of changes in condition, and report incidents of abuse immediately to the Nurse Manager or designated nursing leadership.
Duties:
Perform head-to-toe assessments and insert IV lines.
Manage peripheral and central lines, including PICC lines.

Perform wound care, including dressing changes and obtaining cultures.
Obtain and deliver specimens to the lab and perform tracheostomy care.
Provide non-acute and/or acute ventilator care, suctioning, and tube feedings.
Assist with urinary and bowel elimination, including catheter and colostomy care.
Educate patients/residents and families regarding health, diagnoses, and medication administration.
Respond and assist in emergency situations.
Provide clinical oversight to Licensed Practical Nurses, Nursing Assistants, and Health Technicians.
Minimum Qualifications
Current, unrestricted license from a State, Territory, or Commonwealth of the United States or the District of Columbia.
Graduate of a professional, accredited nursing school approved by the appropriate State or possession of current, unrestricted license for graduates of foreign nursing schools.
Basic Life Support (BLS) certification.
Advanced Cardiovascular Life Support (ACLS) certification is required for work in acute care, med-surg, or critical care areas.
Experience in ICU or critical care settings preferred.
Strong communication and interpersonal skills.
Ability to work effectively in a team environment and provide compassionate care to veterans.
