Thursday - Sunday Schedule, Home Health Visits
As a Weekend Home Health Registered Nurse, you will:
Provide admission, case management, and follow-up skilled nursing visits for home health patients on a Weekend Schedule (Thursday - Sunday fulltime).
Administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
Confer with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment. Provide hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision. Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation.
Coordinate appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
Report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis.
Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care. Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care.

Maintain a schedule for routine maintenance, service, trouble shooting and repair of equipment at solar photovoltaic facilities including but not limited to: o Single Axis trackers o Central or String Inverters o Transformers o AC and DC disconnects o Substation equipment
Maintaining company property such as vehicle, tools, meters and plant stock
Maintaining service reports, inspection documentation, safety paperwork, and spare parts inventory
Provide maintenance and troubleshooting training to technicians and others as needed.

Qualification Requirements
3+ years of experience PV maintenance, including PV DC operations, AC medium voltage systems, substation operations, network communications and monitoring or equivalent

Position Overview
The Division of Behavioral Health and Recovery is seeking to hire a Physician Specialist for the Chester Mental Health Center located in Chester, Illinois to serve as a non-administrative Illinois licensed Physician. Performs medical practice techniques in the specialty of internal medicine and provides delivery of medical services to patients and in consulting with other staff members concerning the medical status and treatment of patients and/or staff.
Essential Functions
* As a primary physician specializing in internal medicine, conducts thorough and comprehensive medical studies of recipients including mental and physical examinations.
* Coordinates and directs medical care through direct medical intervention by self or others, the medical treatment program for all patients of their assigned areas of responsibility.
* Conducts physical examination of employees with work related injuries or physical or mental complaints or inquiries of an emergency nature and provides specialized medical consultation, care treatment and appropriate referral as directed by facility and departmental policy.
* Provides emergency medical care when nursing staff identify an acute medical need and request a patient to be seen by the physician immediately.

Essential Functions of Housekeeping Assistant:
* Sweep and Mop floors
* Dust
* Wash windows
* Shampoo and vacuum carpets
* Arrange furniture
* Generally clean furniture, equipment, fixtures, and hardware
* Other duties as assigned
Qualifications of Housekeeping Assistant:
* High School Diploma or general equivalency is preferred
* 1 year of experience is preferred
* Candidates must maintain a working email address and phone number for employer communication.
* Ability to communicate in English in writing and verbally in conversation with different levels of staff, patient families and any outside customer

Homemakers must physically be able to assist the client with the following: dressing, feeding, hair care, mouth care, shaving (electric or safety razor only) nail care (excluding nail trimming), positioning, toileting, and transfers.
Able to push and pull a client at a minimum of 100 pounds.
Able to lift a client at a minimum of 50 pounds.
Ability to move in tight spaces, between objects and furniture.
Able to perform duties which require standing, walking, squatting, climbing stairs, bending, kneeling, twisting, sitting, reaching at, below, and over shoulder limits.
Able to continuously perform physical functions from up to an hour without rest.
