The responsibilities include but are not limited to: geriatric assessment, medical history, physical exam, diagnosis and treatment, development of the nursing plan of care, health education, physician referrals, case management referrals, follow-up and clear documentation according to ChenMed standards for quality, service, productivity and teamwork. It also includes the participation in clinical rounds and conferences plus in-depth documentation through written progress notes and summaries. Minimum Qualifications * Master's Degree from an accredited nursing program. * Must be a Registered Nurse with valid licensure and be board certified by either ANCC or AANP. * Must have prescription authorization. * Nursing experience working with adolescents is preferred. * Must be available for on-call phone consultations as needed. * Must be able to maintain compliance with attending agency required training programs. * Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
