Rutgers, The State University of New Jersey, is seeking a Senior Care Coordination Specialist for the department of Medicine at the New Jersey Medical School.
The Senior Care Coordination Specialist will work as the Licensed Social Work/ Licensed Associate Counselor Mental Health Clinician and contribute to the success of the program by managing the daily workflow to achieve high quality care management delivery and positive clinical outcomes, while meeting the administrative and contractual performance targets, documentation standards and billing requirements of the grant program(s). This position will focus on assessment, counseling, and coordinating care of high risk/high acuity patients with untreated mental, emotional or substance abuse or problems. The LSW/LAC MCM is responsible for meeting the HRSA standards for medical case management that include (1) initial assessment of service needs; (2) development of a comprehensive, individualized service plan; (3) coordination of services required to implement the plan; (4) client monitoring to assess the efficacy of the plan; and (5) periodic re-evaluation and adaptation of the plan as necessary over the life of the client, as well as patient specific health education, counseling, monitoring, assessment and deployment of techniques to assure the engagement and retention in care of the identified patients. LSW/LAC MCM activities include individual and group therapy, crisis intervention, conducting clinical assessments, medical case management, client advocacy, referral, prevention, and education.
Among the key duties of this position are the following:
* Assess and works collaboratively with the patients, medical provider, designated family/significant others and community support service providers to address the needs identified in the patient's health and psychosocial risk assessment and implement a plan of care based upon the needs identified.
* Facilitates the development and implementation of a comprehensive, multi-disciplinary plan of care for patients with complex mental health and/or substance abuse and socio-economic problems, in collaboration with the physician and other members of the healthcare team.
* Counsels and educates patients in individual and group sessions to assist them with managing experiences of trauma, illicit substance use, mental and physical illness, poverty, unemployment or abuse.
* Assists in the coordination and monitoring of the unit's buprenorphine program. Follows all guidelines and standards regarding record keeping and documentation.
* Provide co-occurring behavioral health psychotherapeutic interventions to provide psychosocial support with continued adherence to treatment and engagement in ongoing treatment and care or referral to higher level of community outpatient programs such as IOP or Partial Hospitalization.

Role Summary:

The Regional Operations Specialist I will ensure operations compliance through implementation and support of programs, initiatives, and audits. This position will ensure additional operations compliance through their support and involvement in incident investigations, operations training, and operating manuals development. This position will also ensure operations performance through monitoring, investigating, and establishing guidance for product inventory management, provide support for the Terminal Automation System, and have the ability to serve as a liaison between Buckeye and our customers.
Responsibilities & Essential Functions include:
Be safe, knowledgeable, and proficient at all Terminal Operator tasks below:
Support operations in product movement, maintenance, and all other aspects of continued, safe operation of facilities. Serve as the primary point of escalation for operations related issues that cannot be resolved at the local level. Provide feedback and actionable suggestions for individual and program improvements.
Initiatives Implementation. Act as primary point of contact for implementation of program initiatives at assigned locations. Provide support, audit, inspection, and status update of initiatives as needed.
Audit & Inspection. As assigned perform internal audits, assessments, and inspections to support corporate program goals. Perform work procedure reviews and other operational audits to ensure operations personnel are adhering to published procedures.
Operations Expertise. Provide expertise in problem solving and assisting operations in overcoming issues and inefficiencies. Awareness of maintenance priorities and preventative maintenance programs.
Inventory Control. Support operations in proper use of all computer inventory control systems and participating in any inventory issue investigations. Support operations teams in proper volumetric accounting.
Internal Compliance. Support operations in compliance with all safety directives, operating procedures, and applicable regulations.
Emergency Response. Responding to terminal, pipeline, rail and marine related emergencies and participating in annual emergency response training drills. Support ICS events within the planning or operations sections during emergency response events.
Annual Budgets. Assisting in compiling annual project lists and costs for annual budget purposes at each assigned facility.
Assist in completion of Incident Investigations.
