Assess / monitor the residents' nutritional status, provide recommendations to medical staff, and develop adequate care plans Evaluate special needs of residents regarding nutrition support, skin breakdown, and significant weight issues Coordinate procurement of nutrition support supplies and oral supplements with outside vendors as well as involved department heads Monitor resident meal service to ensure diet modifications are followed Educate residents, families and staff in concepts of nutrition & diet modification Routinely evaluate Dietary Care plans for effectiveness to ensure high quality of care Meet weekly with interdisciplinary team to review and adjust residents' plan of care Work closely with Speech-Language Pathologist to ensure appropriate mechanically altered diets are provided for residents with dysphagia Reviewing all menu changes to ensure they follow facility's policies/procedures & State/Federal guidelines Update diet order and menu changes in computer menu software on a daily basis Supervising & working closely with the dietary team to provide excellent resident care Conduct audits of areas relevant to providing quality nutrition care on a routine basis Ensure facility is in compliance with regulations and policies on weight monitoring Communicate with interdisciplinary team on a daily basis to provide quality care to residents REQUIREMENTS: Must be a Registered Dietitian Degree in Nutrition or Dietetics Exceptional interpersonal & leadership skills Knowledge of applicable state and federal guidelines Computer literacy and proficiency with EMR software and computer-based menu systems About us: Brooklyn Center for Rehabilitation and Healthcare is a state-of-the-art 281-bed rehabilitation and skilled nursing facility located in the Crown Heights section of Brooklyn. Includes documentation of nutrition related diagnosis when indicated. Demonstrate proficiency in accordance with Standards of Practice and Professional Performance for Registered Dietitian Nutritionist while providing age-appropriate plan of care for patients and families. Coordinates nutrition therapy with physicians and other health care professionals. Participates in multidisciplinary rounds and care planning sessions. Plans, implements, and adapts nutritional care plan to meet physician orders, age, and disease specific physiologic needs. Intervention plan includes appropriate feeding route, mode, and nutrition prescription. Teach cognitive behavioral therapy techniques to promote positive lifestyle changes. Serves as an active member on multidisciplinary teams. Monitors and evaluates patient response to medical nutrition therapy using specific goals and determines change if indicated. Performs patient, family, and care giver education related to nutritional therapy and incorporates best method of learning style and communication into sessions. Supports the development and revision of nutrition related department tools, training materials, policies, and procedures. Remains current on research developments and other methods of practice and incorporates this into current programs and services. Participates in the evaluation of clinical nutrition products and equipment. Conducts telemedicine consultation sessions with clients to assess and analyze diet and disease prevention strategies. Educates and trains interdisciplinary team members, students, and interns. Fosters teamwork and establishes effective strategies to implement innovative programs which leads to optimal clinical nutrition and financial outcomes. Supports quality improvement, research, and outcome studies. Identifies and communicates opportunities for waste reduction and service improvements. Documents accurately and completely, including appropriate utilization of order writing privileges in electronic medical record. Complies with all regulatory standards, including federal, state, and accrediting agencies and adhere to facility confidentiality, HIPAA regulations, and patient rights policies. Participates in and attend departmental meetings, staff development, and professional programs, as appropriate. Ensures national program standards are met and always maintained for certified and/or recognized programs, as applicable. Monitors nutritional care manages physician-ordered medical nutrition therapy assesses patient needs and provides nutrition education to individual patients with complex nutritional deficiencies. Formulates and implements pediatric specialty medical nutrition therapy plan of care. Documents in accordance with policy, procedure, and clinical practice guidelines. Understands and documents face to face and telehealth charging practices with subsequent input into the electronic charging system. Screens, triages, and prioritizes medical nutrition therapy consults for all age groups (neonates, infants, pediatrics, adolescent, and adult.) Completes nutrition assessments and identifies nutrition diagnosis/problem(s). Identifies and documents differing malnutrition indicators in the following groups: neonates, pediatrics, and adults. Monitors formula orders and calculates special recipes with or without additives for milk tech staff. Identifies and integrates trends in anthropometic indices and biochemical data, implications of diagnostic tests and therapeutic procedures. Performs pediatric nutrition focused physical findings to evaluate nutritional status throughout the lifecycle. Performs and ensures proper athropometric data and proper measuring techniques, including pediatric specific head circumference, segmental length, and recumbent length, and applies values to appropriate growth charts. Serves as resource for multidisciplinary team on proper anthropometric measuring techniques and equipment. Performs increased frequency of monitoring and adjusts pediatric plan of care, as pediatric patients can become malnourished quickly. Implements nutrition plan of care and works collaboratively with foodservice staff and milk techs to ensure conformance and continuation of said plan. Functions as an integral member of the clinical interdisciplinary team and the Nutrition Therapy team. Navigates wide network of pediatric formulas and additives and the corresponding companies to ensure tolerance and availability and patients fincial resources. Works with case management and social work to identify available resources for formula supplies between WIC, insurance, and community programs. Documents need for each resource, how much will come from each resource, and assists with document completion for insurance, DME and community resource formula coverages. Identifies goals and provides age-specific, culturally appropriate nutrition counseling and education to patients and their families/caregivers throughout the lifecycle. Uses evidence-based nutrition practice guidelines and appropriate instructional methods and tools based on appropriate age groups. Provides medical nutrition therapy using evidence-based practice to optimize patient care. Provides consultation to the healthcare team to ensure efficient and effective nutrition care to patients. Monitors and evaluates patient care and nutrition outcomes on an ongoing basis. Educates patients, healthcare members, dietetic interns, and members of the community. Participates in ongoing process improvement activities with the Nutrition Services Department. Maintains customer service, patient-centered approach at all times when fulfilling job duties. Meets the qualifications to provide care to patients in the assigned clinical areas. - Provides direct patient care using medical nutrition therapy for assigned patient population. - Develops and conducts education programs for patients, physicians, nurses, allied health professionals, students, peers, and the public. Preceptor for dietetic interns. - Completes nutrition assessments for all assigned patients which may include a Nutrition Focused Physical Assessment as indicated to determine appropriate nutrition diagnosis. - Provides appropriate intervention based on assessment and nutrition diagnosis. - Documents relevant, accurate, and timely information in the patient's electronic health record. - Develops and implements an individualized portfolio plan based on performance indicators that demonstrates professional growth and development, the basis for maintaining required competencies in practice and for maintaining status. Required Education: Graduate of an accredited program in Dietetics, Nutrition, or related fields (Bachelor's or Master's Degree).
