This position requires great attention to detail with strict adherence to deadlines and timelines; Ensures that the individuals' medical and psychiatric needs are met and accompanies or attends the individual/families to appointments as needed; Ensure implementation and evaluation of preventable treatments; Ensure timely medical examinations by maintaining a tracking record of dates due and completion; Participate in all hospital and rehabilitation admissions and discharges including planning for follow up treatments/appointments, medications and medical equipment; Obtain and/or provide training to individuals and families on diagnoses and treatments; Complete of monthly safety checks; Monitor documentation of fire drills and provide Fire Safety and Emergency Disaster Response training to every individual and family member; Ensure adequate supports in the homes for safety. This position ensures maintenance of the home the individuals live in and equipment within the home. This includes a review of the home and yard during monitoring visits for safety, cleanliness and regulatory compliance; Track completion of any issues and home repairs; Maintain a copy of building permits for home repairs, remodeling and renovations; Complete vehicle safety checks and complete forms for any needed repairs and upkeep of vehicles. Orients interested individuals, community members, birth families and other members of the Intellectual Disabilities system to the Lifesharing program; Completes the Initial Screening, Application Review/verification and Home Studies of interested community members including requesting Criminal History Checks, Driver's License checks, I-9 verification, Child Abuse checks, Medicaid Fraud checks and other applicable screenings; Develop/maintain relationships with contracted Lifesharing families through a minimum of weekly contact, continue visits with the family on a frequency of at least one per month; Arrange for transportation to and from individual's work locations and other locations as necessary; Provide training and monitor family compliance of training requirements and timelines; Convey changes in policies to all Lifesharing families in a timely manner; Schedule and conduct team meetings as needed; Provide direct support to individual and family as needed; Evaluate families in effectiveness of contract compliance and give support as needed; Coordinate proper disposition of employee, individual and family complaints and/or formal grievances. There is a need for detailed documentation with this position to include: Developing the annual Assessment, Lifetime Med History, Six-month Checklist and Individual Support Plan, etc.; Assure individuals, support staff and Lifesharing families participate in the Assessment, ISP, and any plans of support; Assure that all outcomes are addressed and documentation is completed; Complete monthly outcome reviews; Complete quarterly updates to the ISP; Write and monitor all plans of support when applicable; Performs the function of the Program Specialist as defined in the Chapter 6500 regulations; Ensure billing accuracy. Participates in the development, implementation and maintenance of the infection control program for monitoring communicable and/or infectious disease among staff and the people we support. Responds to incidents in accordance with regulations and established policy. Participates in Individual Service Plan (ISP) meetings as required. A Licensed Practical Nurse will follow and adhere to the provisions of the Skills Residential Programs Nursing Roles policy at all times. Participates in the annual Licensing Instrument Inspection LII process. Provides direct support in a backup role when necessary; required to provide direct support in Community Homes during staffing shortages. Act as the primary point of contact for medical care, the primary contact person for insurance companies, the primary contact for pharmacies, the medical contact person for families, guardians, or advocates. As primary contact, the nurse will ensure that all relevant and pertinent information is shared with medical providers prior to any procedure or during any visit. Verifies the accuracy of medication prescriptions and communicate changes with the pharmacies. Document medical appointments and follow up treatments clearly, including following up with applicable physicians to eliminate vague orders/directives found in discharge paperwork or in consult documentation and ensuring all test results are obtained in writing and information is clarified and shared with the team. Ensure use of Medical Consult Form for all external medical appointments, including all contact, including phone contact, with medical providers, and ensure documentation is transcribed into the electronic health record. Ensures that all medical treatment plans and health risk protocols are followed as written. Communicate medical events and changes, such as visits to the ER or urgent care facilities, to the person's primary care physician within 72 hours of the event. Participate in “on-call” as scheduled for response to standard nursing care, intervention, and direction to staff. WHEN ASSIGNED TO PHLEBOTOMY DUTIES (IF TRAINED) WILL PERFORM: Draws blood to complete the phlebotomy orders for routine and special tests/exams as ordered by the physician. Provides a safe and private environment for the person during the vein puncture procedure. Completes necessary paperwork to initiate and follow through with the phlebotomy order. Arranges for transportation of specimens as per OSHA and company policies. Maintains safety and confidentiality during transportation. Documents the vein puncture, the person's tolerance and its transport to lab in the progress notes and other necessary documentation as needed. Monitors and maintains a schedule of routine and required laboratory tests for each person. Monitors results of lab work. Reports abnormalities to the appropriate team members. Files lab reports in the person's chart. Follows policy and procedure for laboratory studies. MEDICAL TRAININGS For consistency in care, Nurses facilitate and/or coordinate medical trainings for staff. It will be the responsibility for Nurses to: Obtain and maintain State Medication Administration Trainer Certification to provide Medication Administration Training to staff, Medication Administration Record (MAR) reviews and medication administration observations. Obtain and maintain cardiopulmonary resuscitation (CPR) and First Aid Trainer Certifications as needed to provide CPR/First Aid training to staff. Provide training on Bloodborne pathogens according to Occupational Safety and Health Administration (OSHA) regulations. Provide Medical Protocol Trainings to staff in accordance with Skills Program Policy #11.3 General Health. Provide training on the administration of non-oral medications and other non-standard treatment needs. Provide person-specific training to staff on how to observe and document vital signs, if required by a medical order for a person we support, person-specific health risk protocols, all new diagnoses, equipment, treatment, medications, medical orders, and any non-oral medication administration. Coordinate or arrange for staff to be trained by an authorized entity to provide diabetes care including injections as necessary according to 55 PA Code Chapter 6400 and 6500 regulations. Ensure that all training is documented, and documentation of all person-specific trainings will be kept with the person's medical record. DOCUMENTATION AND MAINTENANCE OF THE MEDICAL RECORD It is imperative for the proper health care of the person receiving services that clear and accurate documentation be established, gathered, maintained, and reviewed regularly. In order to provide quality support for the person receiving services, Nurses will be responsible for establishing, gathering, maintaining, and reviewing the following medical documentation: •Physical Examination •Monthly Medical Progress Report, as required by County Administrative Entity (AE) •Quarterly Medical Review •Electronic Medical Record (EMR) and/or other Medical Record, as available •Medical History •Medication Administration Records (MARS) •Treatment Administration Records (TARS) •CONSULT review •Accident, Illness and Injury Records (AIIs) Set up and maintain access to Patient Portals when available. Nurses will report on the quarterly progress of all health promotion as described in each person's ISP. This information will be reported to the person's program specialist at least five(5)calendar days prior to the end of each quarter. Following any medical event, the Nurse will input relevant medical information into the person's medical record within 5 days of the event. Nurses will be responsible to input medical documentation into the electronic health record, including the electronic medication administration record, within 5 days of receiving the documentation, including: •Medical Consults •Vitals tracking •Health promotion documentation •Changes to medication •Person-specific health risk protocols •Scanned documents from medical appointments, including discharge paperwork, lab results, etc. Nurses will be responsible for routine file monitoring to ensure accuracy of the record by correcting all identified discrepancies. PLAN AND REGULATORY COMPLIANCE A. The Nurse Supervisor will annually review the Individual Support Plan (ISP), Behavioral Support Plan (BSP), and any person-specific health risk protocols for people supported. B. The Nurse Supervisor will maintain a working knowledge of applicable regulations relating to individual health and medications pertinent to Residential Programs, specifically those regulations found in Title 55 of the Pennsylvania Code (55 Pa Code), Chapters 6400 and 6500. Nurses will also maintain a general knowledge of all other regulations pertinent to Residential Programs as described in 55 Pa Code Chapters 6400 and 6500. C. If a person receiving support refuses routine medical or dental treatment, the Nurse Supervisor will ensure the Nurse, with support of the Program Specialist, develops a support plan to train the person about the need for the care or treatment and ensure there is documentation of the training. The support plan should be unique to the needs of the person, include a timeline, and designate who is responsible to complete the training within the specified timeframe. Develops applicable ways to monitor target behaviors by use of data charts, spreadsheets, and graphs. Trains the individual, staff and families on the communication or behavioral guide. Provides additional training to staff, families, and people we support as appropriate and applicable. Monitors progress across all environments through data review, participation in team meetings and contact with the person receiving services. Providies direct and indirect services to the person supported and their team. Provides a written progress report monthly to the individual and to team members by the end of the following month. Completes debriefing of significant incidents with the team within 48-72 hours of the incident. Assists in implementing the plan via coaching, modeling and teaching the individual and team. Attends psychiatric or medical appointments as appropriate and assists in gathering and presenting information to the psychiatrist or medical provider to support the individual. Assists the team in finding information regarding medications and side effects as appropriate. Makes recommendations for additional services that are not within the scope of the Communication Specialist such as a Speech and Language Pathologist or Speech Therapist. Coordinates with other involved medical professionals to support the individual and the team. Helps to establish, modify, and maintain environments that best support communication. Identifies resources and potential communication tools or collaborating with a licensed speech and language specialist to gain assistance with communication tools.
