The Family Therapist/Shadow shall prepare and complete all necessary treatment plans and documentation in a timely manner, including: Psychosocial within 72 hours of admission A treatment plan within 10 days of admission, A mid-program (4 week) treatment plan review and update, Complete weekly progress notes for all sessions with clients and caregivers within 5 days, A discharge summary and aftercare plan given to participants and caregiver/parents at the end of the field program. The Family Therapist/Shadow shall Provide clinical support to TrailWalkers in the field, Participation in PIQA meetings, direct care team meetings and clinical staffing meetings Participation in weekly on call schedule.
