Job Accountabilities:
Ensure all activities are member-focused and individualized, resulting in personalized attention to each patient's unique needs.

Maintain a knowledge of government programs, appropriate paperwork needed for County/State programs, local housing alternatives, employment assistance, etc.
Maintain current list of support groups available for various diseases/conditions, and coordinate utilization of community resources available for members/families.
Maintain knowledge of new laws/policies that affect member care and care management practices.
Complete special projects as assigned by Case Management leadership, and all other duties as assigned.
Job Requirements:
The primary role of this position is focusing on enrollment for members who are losing their U-Care coverage.
RN unrestricted license in the State of Minnesota is required. Experience with MNChoices/MN Choices Certified Assessor is preferred.
Minimum of two years of clinical experience required, one year of experience working with case managers or in case management is preferred.
Ability to read and interpret insurance benefits and experience with computer programs including Microsoft Office (Word, Excel), email, etc.
Working knowledge of internet based resources and strong background in customer service and care delivery.
Strong organizational skills and ability to prioritize tasks, along with excellent oral/written/interpersonal communication skills. Problem solving skills.
Ability to function independently and adapt to change, perform critical analysis, plan, organize and evaluate effectively.
Maintain a valid driver's license for minimal travel.

Job Accountabilities:
Ensure all activities are member-focused and individualized, resulting in personalized attention to each patient's unique needs.

Job Requirements:

Position Summary
This position provides general nursing support for the Adult Psychiatry Department in collaboration with the psychiatric providers and other medical staff to perform patient-centric, safe, effective, timely, and efficient patient care. Responsibilities include providing telephone triage services for the patients of four psychiatric providers, utilizing nursing assessment and intervention in that role, coordinating with psychiatric providers to address the triage calls, accurate and timely documentation, responding to medication refill requests, addressing prior authorization requests for medications, administering long acting injectable medications to a small subset of our patient population, and managing the tracking of things like sample medications and lab results. Additionally, providing coverage and assistance to the rest of the nursing team as needed, including being able to do vitals on patients seen in clinic, and providing support to the psychiatric providers in the clinical management of patients.
Minimum Qualifications
* Associate degree or higher in a nursing program
* Current Minnesota license as a registered nurse
* Minimum of three years of psychiatric nursing experience or triage nursing experience
Preferred Qualifications
* Bachelor's degree or higher in nursing program
* Certification as a Psychiatric and Mental Health Nurse (RN,C) by the American Nurses Credentialing Center
* Experience working with and/or delivering services to diverse populations.
* Five years of psychiatric nursing experience
