Summary:
A Registered Nurse (RN) in a long-term care (LTC) facility provides skilled nursing care to residents, ensuring their health, comfort, and well-being. RNs oversee patient care plans, administer treatments, monitor for changes in condition, and collaborate with healthcare teams to provide high-quality, resident-centered care. They also play a key role in supervising nursing staff, educating residents and families, and ensuring compliance with healthcare regulations and facility policies.
Essential Work Functions:
Assess patient or resident health, monitor for changes, and take appropriate action to ensure their well-being
Develop, update, and implement personalized care plans in collaboration with residents, families, and the patient care team
Administer medications and treatments safely and accurately
Observe residents for any medication reactions or changes in condition and report concerns to the healthcare provider
Provide treatments including wound care, IV therapy, catheter management, and other skilled procedures within scope of practice
Supervise and support LPNs and CNAs, delegating appropriately and ensuring residents receive quality care
Educate residents and families on medications, treatments, and managing chronic conditions
Document care provided, resident progress, and any health changes in a timely manner
Respond quickly to emergencies, delivering critical care and coordinating with medical teams when needed
Work closely with physicians, therapists, and other healthcare professionals to ensure coordinated, comprehensive care
Perform other duties as assigned within the scope of practice
Adhere to facility safety protocols, infection control guidelines, and regulatory standards

Summary:

PURPOSE AND SCOPE:
Functions as part of the Home Therapies dialysis health care team. Ensures provision of quality home patient care in accordance with company policies, procedures and training. Actively participates in process improvement activities that enhance the likelihood that patients will achieve the company's Quality Enhancement Goals (QEP).
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Staff Related:
Provides safe and effective training and oversight of the delivery of home dialysis therapies to patients in compliance with standards outlined in the facility policy and procedure manuals, as well as regulations set forth by the Corporation, state and federal agencies.
Rotates with other licensed staff to ensure adequate coverage.
Participates in staff training and orientation of new staff as assigned.
Participates in all required staff meetings as scheduled.
Quality:

Dialysis Treatment:
Provides safe and effective delivery of home therapy support care to patients with ESRD.
Assesses patients' response to home dialysis treatment therapy by following predetermined protocols.
Communicates patient related issues to the physician as needed.
Reports adverse patient events at home or in the clinic as well as equipment and technology related problems to the Clinical/Home Therapies management and physician including the correct documentation of such events.
Coordinates the transfer of patients to the hemodialysis clinic when needed and appropriate.
Laboratory Related:
Reviews, transcribes, and enters physician lab orders accurately into the Medical Information System.
Follows company policies and government regulations related to the collection, packaging, and delivery of lab specimens and reporting of results.
General Duties:
Maintains competency with all emergency operational procedures, and initiate CPR and emergency measures in the event of a cardiac and/or respiratory arrest when the patient is in the medical office.
Administers medications as prescribed including following algorithms (as appropriate), and document appropriate medical justification if indicated and maintain records on controlled substances as required by law.
Maintenance/Technical:
Maintains appropriate skill level in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone.
Directs patients in the preventative maintenance of home therapies equipment, including water systems and analyses as appropriate.
Coordinates with the company products division on the delivery, operation and maintenance of all home therapy dialysis equipment.

Documents all relevant data including physician orders, lab results, vital signs, and treatment parameters, and patient status and any treatment related data appropriately, and enters into Medical Information System.
Participates in and ensures all company policies regarding patient admission, transfer, and discharge are appropriately implemented.
Completes all monthly and annual reporting as required by company policies and government regulations.

* Tax Shelter Annuity (TSA): If eligible, Department employees are able to participate in a tax shelter annuity program. The TSA plan allows eligible employees to contribute pre-tax dollars to an employee-selected investment service provider authorized to provide TSA products.
* Deferred Compensation: The State offers employees a tax deferred (sheltered) retirement investment program. This program is an opportunity to build a retirement nest egg through voluntary payroll deductions. Several investment options are available.
* Temporary Disability Insurance: Qualified employees may be eligible to receive benefits for a disability caused by a non-work related injury or illness under this program.
* Social Security: As an employer, the State also contributes to an employee's social security account.
* Workers' Compensation: If an employee is injured while on the job, benefits covering medical expenses and partial wage support may be available through workers' compensation.
* Union & Collective Bargaining: Employees have the right to join the union for collective bargaining with the State and employee representation. Some employee unions provide discounts, insurance and other opportunities. Employees may also decide not to join a union, however, employees are required to pay union service fees unless the employee's job is excluded from collective bargaining.

WORK ENVIRONMENT
Nurse Corps Officers may serve at any one of more than 250 Navy and medical facilities around the globe, from Hawaii to Japan, Germany to Guam and Washington, D.C., to Washington state. As a Navy Nurse, you could work at one of the highly acclaimed National Naval Medical Centers in Bethesda, MD, Portsmouth, VA or San Diego, CA. Assist with the activities of daily living, such as bathing, dressing, grooming, transferring, range of motion exercises, assisting ambulation with wheelchair, walker, or cane, incontinence care, toileting, taking vital signs, housekeeping, meal preparation, daily paperwork/charting.
Requirements for employment:
State of Hawaii LPN or RN current license
Current Adult CPR for everyone (Child CPR for those who will be caring for children)
Current First Aid Certification
Current TB clearance + must have had a TB 2-step done at some point in their life
Criminal Abstract will be reviewed
Upon hire, must present 2 valid forms of identification to satisfy I-9 requirements.
If you are willing to work at a lower level (Nurse Aide) you may apply as a Nurse Aide.
Please note Compensation is based on the level at which you work. (Working at the Nurse Aide level = pay at the Nurse Aide rate of pay. Working at the Skilled Nursing level = pay at the Skilled Nursing rate of pay.)
