Job Summary
Under the supervision of the Director of Foster Family Adoption (FFA) and Intensive Services Foster Care (ISFC), the FFA/ISFC Social Worker is responsible for a caseload of children and their families. Primary duties include conducting initial assessments and placements for children. The FFA/ISFC Social Worker also provides ongoing training, evaluation, and monitoring of foster and adoptive parents. They are responsible for completing all paperwork in accordance with County and agency regulations.
Essential Functions
Provide case management and coordination of services as required by contract and Title XXII regulations for safety and health.
Participate in recruitment of foster parents and provide orientation and training to applicants.
Complete and regularly update case related documentation, quarterly treatment plans, discharge summaries, initial quarterly and termination reports and files of foster parents and children in specified time frames as required.
Provide support to foster parents as well as enforcement of safety and health per Title XXII regulations.
Coordinate office duty and after hours duty coverage for emergencies as scheduled.
Assess referrals for appropriateness of placement
Provide initial and on-going training, evaluation and monitoring of foster and adoptive families
Responsible for intake assessments and placements of children
Interface with agency staff to complement the client's therapeutic milieu experience
Provide appropriate discharge and after care planning

Conduct lithium-ion battery cell tests, including electrical, mechanical, and electrochemical assessments.
Help and support standard operating procedures (SOP) documentation, prioritize tests, and track and communicate test progress to various teams of cell engineers and scientists.
Develop and execute automated test scripts using cyclers and chambers to streamline testing processes.
Collaborate with data engineers to process test data, maintain testing databases, and create scripts for automated data analysis and report generation.
Improve post‑processing pipelines to enhance data accuracy, consistency, and overall data quality.
Design and optimize lab automation (hardware and software)

About the Role
The Social Work Assistant is responsible for assisting and managing a caseload of participants working in close coordination with the Seen Health Interdisciplinary Team (IDT) and Social Worker. The Social Work Assistant functions as a liaison between the participants, their family members, and the IDT assist participants by offering advice, implementing treatment plans, coordinating community services, and completing necessary paperwork. The Social Work Assistant collaborates effectively with colleagues and stakeholders to promote the Seen Health values, team culture and mission.
Responsibilities
Assist Social Workers (MSWs) with daily tasks that may arise. Escalate to MSW or identified appropriate staff member as needed.
Establish rapport with participants and families to provide psychosocial support.
Complete IDT care plan reviews with participants and family members within regulatory time frames.
Deliver and document social work interventions as agreed upon in the participants' care plans including but not limited to check in calls, arranging necessary resources and services, and assisting with care transitions, and case management.
Participate in IDT adhoc and care conference meetings to provide input, and function as the administrative support to MSW to include transcribing meeting notes and documentation as needed.
Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT/Social Worker promptly of any changes in participants' condition or psychosocial status.
Connect participants or families with social and community services. Arrange meetings with relevant agencies, while serving as the advocate for our participants every step of the process.
Assist with completing grievances, appeals incident reports, APS reports, and other quality and compliance requirements as needed.
Maintain an up-to-date roster of relevant agencies.
Complete discharge planning with participants and or family members when participants disenroll from the program, present to IDT, and provide appropriate documentation to medical records timely.
Provide administrative support to MSW for educational in-services to staff, participants, and caregivers/families regarding psychosocial aspects of aging and related issues
Participate in end-of-life care coordination and support.
Conduct home visits, hospital visits, skilled nursing visits, as necessary.
Demonstrate ability to assist participant or family member with completion of identified application and community assistant programs. Will include but limited to benefit renewals, housing, DMV needs, utility bills, and other community support needs.
Complete all required documentation in a timely and accurate manner.
Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
Follow Seen Health policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
Qualifications
High School Diploma or Equivalency.
Associate's Degree in Social Work, Human Services, Social Science, or relevant field preferred.
Valid State Driver's License with a clean DMV record.
Minimum one year experience working with frail and elderly population. (3+ years social work experience preferred).

All new hires will be placed at the starting salary step of this classification ($28.04). Employees are eligible for a step increase on their anniversary date each year, up to the maximum step 5 of this salary range ($32.52). ABOUT THE JOB: This eligibility list will be used to fill mostly full-time (6 hour) assignments. Persons in these positions work Monday - Friday, during the school session. Under the general direction, provides intensive behavioral intervention to students who have severe behavioral disorders; and performs related duties as assigned. JOB DUTIES: Provides intensive behavioral intervention to designated students who exhibit behaviors such as biting, kicking, hitting, throwing, running, spitting, verbal aggression, pulling, and pushing in a variety of educational settings, including classroom and community settings, as directed by supervisor or other specialist; follows documented therapy plans for intensive behavioral treatment; confers with instructional personnel and provides input regarding student progress; documents student progress and behavioral data; assists instructional personnel with development of behavioral and reinforcement strategies; utilizes approved district restraint training to assist with restraining students as appropriate; utilizes Applied Behavior Analysis (ABA) strategies as directed; assists in training students in personal hygiene and developing self-sufficiency; and assists students with toileting.

EMPLOYMENT STANDARDS Education and Experience: High school diploma or equivalent, and the equivalent of at least six months of part-time experience working with students or clients with intensive behavior disorders is required. Any other combination of training and experience which would likely provide the required skills, knowledge and abilities may be considered. All candidates must meet the federal Every Student Succeeds Act (ESSA) requirement for this position. You can meet the ESSA requirement through possession of an associate's degree or higher or have completed at least 48 full semester units. Knowledge of: Special needs and requirements of students with intensive behavioral disorders; Applied Behavior Analysis (ABA) techniques and strategies; appropriate restraint procedures using district training techniques; correct English usage, grammar, spelling, punctuation and vocabulary; oral and written communication skills; interpersonal skills using tact, patience and courtesy; recordkeeping and report preparation techniques; appropriate hygiene standards; and health and safety regulations as related to the position. Ability to: Provide intensive behavioral intervention to students; maintain accurate records and data; demonstrate a patient, receptive and understanding attitude toward students; work independently with little direction; maintain confidentiality of sensitive and privileged information; determine appropriate action within clearly defined guidelines; complete tasks in a timely manner; establish and maintain effective working relationships; use good judgment in making independent decisions to respond to student requests and needs; assist students with personal care such as dressing and toileting; follow education and behavior plans for students; and communicate effectively in English.
