How you will change lives
As a Social Worker at US Renal Care, you will be an integral part of a cross-functional team, working to help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation.
What you will be doing
Advocate & Support. You will be part of an interdisciplinary team working to ensure patients receive the best care, including conducting all required patient assessment and care planning activities such as assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and all state/CMS regulations. You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies.
Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care. You will participate in all required continuing education and staff meetings. You will collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community.
Safety & Quality. You will help with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes. You will also participate in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with federal, state, and local laws and regulations.
What we're looking for
Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
Current licensure (in good standing) in applicable state is required unless employed in the state of AZ, PA or Guam. Must meet any practice requirement(s) for the applicable state.
Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
Basic computer skills, including Microsoft Office (Word, Excel, Outlook).
Proficiency in all USRC clinical applications required within 90 days of hire.
Preferred
Previous experience in providing social services to dialysis patients preferred.
Other Requirements
Must meet any practice requirement(s) for the applicable state.

About the Role
The BCBA (board-certified behavior analyst) is primarily responsible for conducting functional behavior assessments and developing behavior plans based on these assessments. The BCBA is to ensure the effective implementation of all treatment plans and programming for their clients on their caseload as described by the fundamental responsibilities listed below. This position will work primarily in the function of providing services and expertise in a Community Residential Alternative (CRA) environment for adults. The secondary function of this position will be to provide behavioral supports for other adults in the IDD program.
Responsibilities
Adheres to professional and legal requirements, the principles of behavior analysis and demonstrates sound professional judgment at all times.
Collaborates with families, caregivers and all service providers as necessary to ensure continuity of care.
Performs direct and indirect assessments to identify the function of client's behaviors.
Develops a treatment plan (including behavior support plan) based on the functions of the behavior and treatment methodologies that are evidence-based.
Attends all relevant meetings for clients, including treatment team meetings, ISP (as allowed/needed) and staffing.
Adequately demonstrates their abilities to work effectively as a part of a clinical team.
Collects data before and during the implementation of the plan so that the plan's efficacy can be assessed and data-based decisions can be made.
Provides direct instruction/ongoing supervision, training and support to clients, staff members and family members during regularly-scheduled visits.
Evaluates the effectiveness of the implemented treatment plan and modifies the plan as necessary.
Upon receiving information from the Vice President, the BCBA will initiate services with new clients in a timely manner.
Manages caseload and prepares and submits all required documentation on time. This includes, but is not limited to: assessments, treatment plans/BSPs, session notes after every session, at least monthly analysis of the data, updating the treatment plans as needed, participation in calls with insurance companies, etc.
Generates reports mandated by DBHDD in a timely manner.
Supervises Registered Behavior Technician's (RBTs), BCaBAs, and graduate students, if assigned to their caseload.
Performs other duties as requested/assigned.
Qualifications
Must have MA degree in related field and BCBA certification number. Certification must be kept up to date.

The Youth Worker/Transportation for La Jornada works to make a meaningful difference in the lives of the families being served by the National Youth Advocate Program by working closely with the youth and family to provide advocacy and support towards targeted goals.
RESPONSIBILITIES
The Youth Worker for La Jornada will perform duties including, but not limited to:
Perform all work in a manner consistent with the National Youth Advocate Program's Mission, Values and Philosophies.
Interact with the youth to develop a trusting, supportive relationship while assisting the youth in achieving identified goals.
Provide youth with the appropriate leadership during the program.
Provide a warm, caring and safe atmosphere for youth.
Perform other duties as assigned.
Report any problems which arise to the Out of School Coordinator.
Responsible for all activity and room preparations for the program.
Complete all relevant training on a timely basis as required by state, contract, and/or licensing agency standards.
Complete all required documentation in a timely manner.
Maintain a positive attitude toward all youth, co-workers, families and others involved with the program.
MINIMUM QUALIFICATIONS
High School Diploma or GED equivalent.
Excellent Interpersonal and communication skills, with the ability to engage and interact effectively with minors.
Patience, empathy, and the ability to provide emotional support to minors as they navigate various developmental stages.
Flexibility and adaptability to work in a dynamic and fast-paced environment.
Previous experience and proficiency in the use of various office machines, including computers.
Proficiency with Microsoft Office, including Excel and Word.
Bilingual English/Spanish fluency.
Must have a valid driver's license, reliable transportation, automotive insurance, and a good driving record.
Minimum automobile insurance coverage of $100,000/300,000 bodily injury liability.
Proficient use of desktop and laptop computers, internet search, people management programs, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
21 years of age, valid driver's license.

Ability to travel frequently and drive vehicle while sitting for extended periods, with frequency varying based on program demands.

Job Description:
Provide clerical support to the Atlanta Registrar's Office. Primary job responsibilities will include scanning files and data entry. Additional responsibilities may include front counter, phone support and assisting Registrar staff with other tasks as needed.
Requirements:
Must be proficient in basic computer usage and office equipment. Must be able to pay attention to detail with a high degree of accuracy and exercise good judgment and discretion in handling confidential information.
