Job Title & Role Description:
TheCare Specialistis a key member of the team responsible for delivering chronic care management to high-complexity patients. The Care Specialist primarily works in patients' homes and communities (90% of the time) and engages in virtual care (10% of the time). This role involves direct outreach to patients, guiding them through the enrollment process, facilitating virtual appointments with providers, and ensuring that patients adhere to care plans. By building relationships, coordinating care, and providing education, the Care Specialist plays a critical role in promoting health, preventing unnecessary hospitalizations, and improving patient outcomes.
Skills Required:
Minimum of 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant, etc.).
High school diploma or GED required.
A valid drivers license, auto liability insurance, and reliable transportation to travel within the assigned territory.
Experience in chronic care management or working with chronically ill/elderly patients.
Technologically proficient with basic computer skills (typing, using EMR systems).
Experience with motivational interviewing, trauma-informed care, and care coordination.
Strong interpersonal communication skills with the ability to engage patients and team members effectively.
Ability to prioritize tasks, manage schedules, and work independently in an unstructured environment.
Multi-lingual skills are a plus but not required.
Prior home care experience is beneficial.

Job Summary
Odyssey House Detox Program is currently looking for a full-time bilingual (English and Spanish) Licensed/Licensed-Eligible Social Worker or Counselor. The schedule is M-F 8am to 5pm, rotating holidays and weekends as needed. The Counselor will provide specialized services for the assigned caseload, provide core and ancillary group therapies weekly, individual counseling and when appropriate, family therapy. The Counselor has the main responsibility of maintaining communication with the referral source, court systems, and other involved human service providers as related to the client's ongoing treatment and discharge planning. The Counselor is responsible for working within the multidisciplinary team to develop treatment planning, treatment case reviews, and all written documentation as required by the agency, state and federal courts, and all funding sources.
Responsibilities and Duties
It is the duty and responsibility of the Counselor to:
provide Clinical services to assigned clients; conduct weekly group therapies, individual counseling and/or family therapy as outlined in treatment plans; co-lead group therapies; conduct family assessments/therapy;
function as a liaison to outside agencies, participate in case planning conferences, and treatment meetings and, when necessary, attend court hearings and represent the agency and client(s);
maintain contact with and is available to Agency personnel, client's drug court officer, and family members regarding client's progress, problems and crises;
maintain clinical case file for each client to include: assessments, Individual Treatment Plans,
Case notes, and discharge summaries;
apply ASAM Criteria to treatment planning;
maximize units of services and maintain contract units of service by working with clients to stay engaged in treatment;
provide crisis management when indicated by client need;
assist with the planning and organization of therapeutic client actives both at and outside of the facility;
ensure all written reports are up-to-date in case file;
ensure completion of electronic medical record documentation;
attend staff and other meetings;
attend regularly scheduled in-service training;
participate in quarterly training with staff;
complete all Incident Reports prior to the end of scheduled shift as required by Incident Policy and Procedure;
adhere to all Louisiana Department of Health and Agency contract requirements;
maintain COA standards and participate in COA site audits;
collaborate with other departments;
work with Bayou Health providers for initial authorization and concurrent authorizations;
ensure that all sign in sheets are printed for group and all groups are batched for billing; and
perform all other duties as assigned.
Qualifications and Skills
Minimum qualifications include the following:
Master's Degree in Clinical Social Work or Counseling
A valid Louisiana State issued license in either counseling or social work
At least 1 year of substance use disorder treatment modalities (emphasis on Minnesota Model) and DSM-V experience

Provides individual and group treatment to address addictive behaviors by using Evidence Based practices and approaches such as: Trauma Informed Care, Motivational Interviewing, Harm Reduction, and 12 step group sessions
Conduct group and schedule individual substance use disorder counseling meetings with patients to assess triggers, cravings, withdrawal symptoms, substance use, motivation to remain in recovery
Assist the client in developing skills required to achieve sobriety and better enhance his/her abilities to function as a productive member of the community.
Provides ongoing support for clients in areas of their daily living situation; assist client to build resilience through focusing on interpersonal skills, maintenance of mental needs with particular focus on substance abuse issues, educational needs, connecting to work training, fulfilling their legal obligations in order to remain in the community.
Facilitates appropriate needed referrals as recommended by BTs and Prescriber to other treatment programs, i.e., detox, inpatient or residential treatment facilities and medical care in conjunction with the client's insurance and primary care provider. The Federal Work-Study Program provides you with part-time employment to pay your education expenses. The program emphasizes work that is in the public interest, and when possible, related to your major. Students can work on campus or off-campus at an approved organization off-campus.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following and other duties may be assigned:
· Smile and be productive in your work
· Must dress in a professional manner (no low cut blouse, tank tops, flip flops, beach attire)
· Academically excel to maintain your employment
· Report to assigned supervisor at scheduled work times
· Clock in/out using a PC equipped with Time and Labor program
· Understand falsification of time worked constitutes payroll fraud
· Type material assigned and make necessary photo copies on copy equipment
· Fax material from facsimile equipment as assigned
· Prepare, type, key and/or photocopy bid quotations for mailing as assigned
· Separate and label outgoing mail as assigned
· Assist in time-stamping and separating incoming mail as assigned
· Be cordial to students, faculty, staff and visitors entering the office
· Assist students, faculty, staff and visitors conducting business in the office
· Assist other purchasing personnel in the {enter department} related functions when requested
· Answer and screen telephone calls in the absence of supervisor or secretarial personnel
· Transfer telephone calls to appropriate personnel or record appropriate message
· Perform other tasks as may be assigned by supervisor
SUPERVISORY RESPONSIBILITIES:
None
QUALIFICATIONS
ü Experience preferred (may be for Team Leader or Event Leader type of positions).
ü If particular field of study is preferred, list it.
ü Familiarity with the office equipment (list equipment you prefer them to have experience with).
ü Good computer skills (list programs your department uses, for example Microsoft Word or Excel).
ü Excellent communication skills and detail oriented.
ü The ability to handle confidential matters and to be professional.
ü Ability to work well with others.
ü Willingness to tackle new projects.
ü Ability to work between 15 - 20 hours per week (indicate if evenings/weekends/holidays are
ü required).
ü Flexible schedule (if required).
ü Travel to sites via your own transportation (if required) if assigned Off Campus - Tutor
ü Student must be awarded Federal Work-Study through Financial Aid and Scholarships.
ü Must be enrolled at least ½ time
ü Must maintain at least a 2.0 cumulative GPA.

· Strong keyboarding skills, working knowledge of Microsoft office suite.

· Ability to communicate and work well with a diverse staff and other students.

The Clerk's Office of the Louisiana Supreme Court has an immediate opening for a part-time student worker. Successful candidate must be a full-time student at an Accredited College or University (i.e., 12 credit hours for undergrads) and be able to work approximately 15 hours per week during the school year, with potential for higher hours during school breaks. Hours of operation are 8:30AM - 4:30PM weekdays. Work assignments will include telephone coverage, scanning documents, data entry, assistance with various administrative tasks associated with legal work, organizing and completing large copy jobs, filing, and other special assignments as needed.
Required skills for this position include but are not limited to: Microsoft Office Programs, Outlook, excellent customer service and communication skills, ability to work independently, strong organizational skills and attention to detail. Knowledge and/or experience with databases, also helpful. Candidate must be able to pass criminal background check.
$10.00/Hour, plus either paid parking in the French Quarter, or a bus pass.
