What You Will Do
Applies mutually shared and lived experiences navigating the systems of care as a caregiver. Serves as a role model, advocate, and mentor for children and families
Assists with obtaining and maintaining clients' entitlements, including Medicaid, Public Assistance, SSI as well as assisting with problems related to housing or utilities
Interviews clients and significant others, when appropriate, in order to obtain personal histories and to understand the social and emotional factors related to the client's mental health problems
Participates in frequent active engagement with families to engage them in receiving services and assists in empowering them to take the steps necessary to maximize their quality of life
Develops inventory of resources that will meet the clients' needs as identified in the assessment process
Advocates for and assists clients in utilizing adjunctive community resources. Arranges transportation and accompanies clients to appropriate facilities/agencies, as necessary
Provides linkage, coordination with, referral to and follow-up with other service providers. Monitors appointments medical facilities, social service agencies or other providers of concrete services
Collaborates with interdisciplinary team members to discuss care needs and identify solutions to support clients/consumers/members
Attends planning meetings with service providers to coordinate service plans

The goal of the Family Enrichment Center (FEC) is to enhance child and family well‑being by providing access and support to enable the strengthening of family protective factors inherent in its residents and community by building mutual community connections, capacity, and resilience.
In partnership with FEC staff, families will drive the planning, development, and sustainable co‑ownership of the centers. Families who participate in the FEC programming will be encouraged to share their expertise by volunteering at the center and given meaningful opportunities to engage with neighbors and peers. The goal of the FEC is to strengthen families' protective factors and build community connections, capacity, and resilience.
Position Summary
The Family Advocate is responsible for contributing to the day‑to‑day operation of the FEC including building connections with other staff, residents, and community partners; co‑designing and delivering relevant programming and activities (offerings); providing advocacy and making referrals for participants; and creating a warm and inviting gathering place for families and individuals using the protective factors model to support community resilience.
Responsibilities
Coordinate meetings, provide support, and collaborate with a team of local parents, subject experts, and other residents to deliver monthly events and activities (offerings) held at the FEC.
Foster a culture of respect built on the belief that families and communities possess the inherent strengths and knowledge needed to succeed given the right access to opportunities and support.
Provide advocacy, outreach, and referrals to connect participants to service providers and other programs that have resources to offer.

Qualifications
Minimum qualifications include a bachelor's degree and three (3) years of professional experience; or a high school diploma and five (5) years of professional experience in community-driven programs working directly with children, youth, and families.
Reside in or near the FEC community, or reside close enough to arrive promptly, in the event of an emergency.
Bi‑Lingual either /Mandarin &/or Cantonese is required.

· Collects pertinent social data upon admission of each resident and places
it in the medical record, including information about the personal and family
problems related to the resident's illness and care, support network, actions
taken to meet the resident's individual needs and eventual discharge to an
appropriate level of care based on functional capacity. Pertinent social data
shall be made available to the attending physician and other appropriate staff
members;
· Acts as a liaison between residents, families, outside agencies, and the
facility Administrator to ensure that the resident's rights are maintained;
· Functions in a manner that adheres to all policies/procedures of the
facility, as a representative of the Windsor Health Care, LLC's Center
administration;
· Documents and prepares any reports requested by the administration; also
councils and provides assistance when change in financial status occurs for
residents in the Windsor Health Care, LLC's Center;
· Coordinates discharge planning and assists with the discharge planning and coordination of community

Develop inventory of resources that will meet clients' needs as identified in the assessment process
Advocate for and assist clients in utilizing adjunctive community resources; arrange transportation and accompany clients to appropriate facilities/agencies as necessary
Provide linkage, coordination with, referral to and follow‑up with other service providers; monitor appointments to medical facilities, social service agencies or other concrete service providers
Collaborate with interdisciplinary team members to discuss care needs and identify solutions to support clients/consumers/members
Attend planning meetings with service providers to coordinate service plans
Initiate written (including clinical and progress notes) and verbal communications, and ensure all patient record‑keeping is in compliance with agency, state, city and federal requirements
Participate in agency and community programs and education as requested
Participate in interdisciplinary team meetings, case conferences, staff meetings, quality assurance meetings, and supervision meetings as required
Qualifications
Licenses and Certifications:
Valid driver's license (i.e., license matching state of residence) required
Credentialed family peer advocate preferred
Education:
Bachelor's Degree in social work, health and human services or a related field preferred
Two or more years of equivalent and relevant experience in a social service agency OR an equivalent combination of the education and experience preferred
Work Experience:
Minimum of one year of experience in a mental health or social service setting with emphasis on family support services and/or family psycho‑education required
Lived experience as a caregiver for children with behavioral health needs in accordance with the New York State Office of Mental Health and the New York City Department of Health and Mental Hygiene required
Bilingual skills may be required as determined by operational needs
Pay Range
USD $20.98 - USD $26.23 /Hr.

Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
Monitor patient progress and adjust treatment plans as needed.
Requirements:
Active and unrestricted license to practice as a Licensed Marriage and Family Therapist in State applying for.
Effective oral and written communication in English
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.

Full/Part Time: Part Time Number of hours/week: 4-17 Number of months per year: 12 (only while classes are in session) Basic Function and Scope of Responsibilities: Tutors provide individual and/or small-group tutoring to students in the Center for Learning and Academic Success. They assist students in improving academic achievement by meeting with them on a regular basis to clarify learning issues, work on study skills and review content. Tutoring sessions are designed to supplement in-class work and focus on providing opportunities for active learning, self-reflection, and collaborative study. Tutors, acting as educational mentors rather than instructors, focus not on teaching content and completing homework assignments, but on posing problems and putting course subjects into practice through critical thinking and re-examination. Subject Specialists focus on advanced level courses, and act as mentors for other tutoring staff.
Other assistance might include reviewing class material, discussing the text, helping students to predict test questions, helping students formulate ideas for research, working on solutions to problems, and/or assigning practice exercises. Tutors are not expected to know everything about the subject for which they tutor, and they are expected to continue developing their tutoring skills.
Principle Responsibilities:

Education Requirements:

* Passed content course with a grade of B or better in the subject area which the candidate desires to tutor.
* Be recommended by at least one faculty member or academic dean in the content area.
* Possess good oral communication skills (demonstrated in the interview) and ability to explain concepts well.
* Availability to work at least 4 hours per week during CLAS's regular hours.
* Demonstrate interest and enthusiasm for teaching and tutoring.
* Be available for weekly tutor training in the form of Mentor Groups and Tutor Development meetings and workshops.
Additional Requirements
* Maintain accurate records and return them to the supervisor in a timely fashion.
* Strict adherence to the policies and procedures outlined in the CLAS Handbook.
* Possess the utmost respect for the confidentially of the tutor/students that are tutored relationship.
* Report concerns about students that are tutored to the Director. Areas of concern to report are absenteeism, lack of class/tutor session preparation, emotional stress, questions pertaining to academic integrity, or any other indications of obstacles to academic success.
* Complete and turn in periodic evaluations of tutoring sessions as requested.
Training, Skills, Knowledge, Experience:
* Attendance at ongoing Tutor Development Meetings and/or Mentor Groups is mandatory.
* Priority is given to LIU-Brooklyn student applicants.
* Previous tutoring and/or teaching experience is preferred, but not required.
