JOB DESCRIPTION AND QUALIFICATIONS (This position will travel within the Columbus and surrounding areas) This position can be part-time, full-time or even contract We are seeking a compassionate and skilled Therapist to provide high-quality mental health services to residents live live in skilled nursing and assisted living environments. The ideal candidate will be a Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or another equivalent licensed mental health professional. The Assessor will provide biopsychosocial, treatment planning, and case management, to clients in a supportive and professional environment. In this role, you will work closely with clients to assess their mental health needs, develop personalized treatment plans, and provide evidence-based therapeutic interventions. You will also collaborate with a multidisciplinary team to ensure that clients receive holistic care tailored to their individual needs. Key Responsibilities: Individual Therapy (small caseload): Provide therapeutic services through individual counseling sessions to clients experiencing mental health challenges. Assessments: Conduct comprehensive assessments (including intake, diagnostic, and risk assessments) to understand client needs and determine appropriate treatment. Treatment Planning: Develop individualized treatment plans in collaboration with clients, addressing their mental health needs and goals. Regularly evaluate and modify treatment plans as necessary.
