Job Title & Role Description:
The Care Specialist is a key member of the team responsible for delivering chronic care management to high-complexity patients. At Catalytic Health Partners (CHP) our Behavioral Health Specialist (BHS) are dedicated to meeting with members in their home focusing on their coping skills, comfortability with behavioral health clinics and counseling through complex life situations. CHP is an integrated population management company, and our BHS will work with physical health and social needs team members to address the holistic needs of our members. We engage with members who are high cost, high risk and have low engagement with the health care community and our performance is based on how well we as a team address their complex holistic needs. We do not replace their community providers and clinics but help guide members on how to effectively use their community resources. All our team is remote and, in the field, seeing 4 to 5 members per day in the member's home setting for in person visits. BHSs are provided AZBBHE clinical supervision to further their licensure, if desired.
Area Covered
This position is a field position, and you will be in East Maricopa which covers: Queen Creek, Apache Junction San Tan Valley, Mesa, Gilbert, Chandler
Day to Day
A typical day for a BHS will start with them traveling from their home to their first member's home. You will see 4 to 5 members per day in their home setting with and office team helping to set your recurring appointments with members. You will meet with members for about 1 hour addressing their goals, barriers, life situations and plans for achieving their goals. We focus on the member's holistic needs so every team member will be addressing and asking members about their social, behavioral, and medical needs. After your appointment you generally have a 30-minute time frame to make phone calls, respond to emails and other such administrative needs for your member population before the next appointment. A BHS has a member population of approximately 40 members that you are accountable for as a lead provider, with other team consultations to assist with urgent needs. Many times, BHSs are following up on their members in BHRFs, coordinating intakes for members and following up on member and team phone calls. In your role you are part of a team all working collaboratively to ensure your members' needs are met with you leading the way. Your work schedule is a standard Monday through Friday with flexibility in your appointment times based on member needs and your needs.

A Crisis Counselor provides a clinically sound crisis assessment to clients who are in crisis at a hospital or office setting. Crisis Counselors will respond in a timely manner to requests from physicians, nurses, nurse case managers, or others to complete a thorough crisis assessment and make recommendations for discharge or level of care. A Crisis Counselor must have the ability to assess and provide immediate crisis intervention services. What You'll Do:
The Behavioral Health Specialist (BHS) is responsible for managing and maintaining the safety and well-being of individuals and staff within the unit, ensuring high standards of care, and support for both adults and youth. What You'll Do:
The Behavioral Health Specialist (BHS) manages and maintains the safety of the individuals and staff. New Freedom's Behavioral Health Professional (Therapist) performs a variety of therapeutic services to provide psychological care to members receiving treatment. This treatment includes a focus on various behavioral challenges such as substance abuse, post-traumatic stress disorder, post-incarceration syndrome, and co-occurring behavioral health needs. The BHP works collaboratively with all members of the treatment team including the members and family. The level of care for the program is Intensive Outpatient Services, with specific curriculum and program design.
Duties and Responsibilities:
Observes members, listens to their concerns, records their observations, and reports to mental health professionals about any unusual behavior(s)
Provides individual psychotherapy to members on their assigned caseload
Develops and implements service/treatment plans
Provides quality care for members on an outpatient basis
Assesses members' changes in mood, and determines whether it is positive, negative, or potentially dangerous, taking the appropriate steps to modify the members' actions and alert the leadership team

Maintains safety for staff and members, including effective securing of documents and files
Works with other treatment team members to plan, market, and implement new programs
Facilitates groups that are 2-3 hours in length, five (5) days per week, with pre-determined and pre-approved curriculum
Completes monthly individual counseling sessions with members of assigned caseload
Completes staffing meetings with assigned treatment team(s)
Completes all necessary and required documentation within 24 hours of service(s) being provided

The
Resource Specialist
assists with the development of individual success plans for each family, provides referrals to other community services/resources, and serves as a liaison with outside entities to advocate for the needs of the family. The Resource Specialist works with each family to identify a community rental unit and negotiates the lease and payment process with each landlord. The Resource Specialist also works closely with homelessness liaisons from school districts to ensure that federal education regulations regarding children that are homeless are followed.
The successful candidate will be a compassionate service professional, who is deeply motivated by helping families and children achieve sustainability through the attainment of their goals, is adaptable to change and able to work at a faster-than-average pace, and is thorough, structured, and detail-oriented.
DUTIES AND RESPONSIBILITIES
(Work assignments may vary depending on the department's needs and will be communicated to the applicant or incumbent by leadership):
Provide direct Resource Specialist services to families in the Family Pathways program including:
Screening of potential participants and orienting new families to the program.
At participant entry, complete intake paperwork, administer housing assessments, create and continuously maintain accurate HMIS/Service Point electronic records for all participants, tracking and ensuring data is entered within 24 hours of each participant-related contact or activity.
Ensure all intakes of participants are entered in Shelterpoint within 1 hour
Establish a file for each participant in programs, ensuring proper documentation at intake as required by Primavera guidelines and outside funding sources.
Create family success plans and meet with participants on a weekly basis, monitoring progress and completing referrals to community partners.
Coaching and guiding families, providing life skills resources, and ensuring follow through on referrals, and advocating for families as appropriate.
Coordinate with Primavera programs and other community permanent housing providers to assist families with stable housing.
Maintain contact with school homelessness liaisons/coordinators, ensuring the needs of the children are met, including transportation
Arrange for short-term motel stays as needed for families in immediate housing crisis.
Provide referrals, information, and guidance to families for housing search.
Negotiate the lease and all related activities with the landlord.
Completing Habitability Standards inspection of units and finalizing follow-ups.
Create the Temporary Occupancy Agreement for the adult participants, which is to include a copy of the lease signed by Primavera and the landlord.
Purchase and ensure delivery of furniture as necessary depending on the needs of the family.
Create individual success plans and meet with each participant at least monthly, monitoring progress and making referrals to ensure participant's success, and adjust as necessary.
Develop, plan and teach job readiness, financial education and housing search skills to program participants.
Ensure participant files and tracking are properly maintained and confidentiality requirements.
Provide resources such as laundry services, household supplies, and other necessities.
Complete periodic home visits and weekly check-ins to ensure a safe living environment for each family.
Conduct post-shelter exit follow-ups and tracking.
Maintain tracking information and statistical data as required internally and by grant funding sources.
Sort, store and distribute donations received for Family Pathways families.
Attend training and meetings as required, including but not limited to Resource Specialists, 1:1, employee meetings, and all employee meetings.

The Job/What You'll Do:
The Crisis Specialist I is responsible for receiving, responding to, and triaging crisis and healthcare requests. Screens and triages incoming requests, completes an assessment and intervention, ensures safety, coordinates care, and documents the interaction in an electronic health record (EHR). Promotes and maintains an organizational culture that is focused on Solari's core values. This is realized by demonstrating the following outcomes: safety, recovery and resiliency, engagement, crisis assessment and intervention, hope, efficiency, quality, accuracy, and service excellence.
Responsibilities:
Respond to incoming contacts and requests for crisis and healthcare services across all contracts within the scope of employee work; completes outbound calls, emails, text or chat to effectively coordinate care and resolve crisis situations; operates with accuracy, compassion, and efficiency.
Demonstrate effective engagement skills; inspire hope and promote recovery and resiliency.
Screen and triage incoming crisis requests, resulting in safety and crisis resolution; effectively use the risk assessment tool to guide crisis intervention based on acuity and risk.
Utilize knowledge of healthcare industry standards and clinical assessment and intervention skills, resulting in crisis or contact resolution.
Provide health education and community resources based on the intervention provided.
Participate in required training, supervision, meetings, and clinical oversight; demonstrates enhanced knowledge and skill over tenure at Solari for of new contracts, suicide intervention and prevention, the behavioral health and medical healthcare systems, Medicaid or Medicare managed care, mental health and diagnostics, community resources, effective communication, active listening, speech analytics, contact center industry standards, and recovery and resiliency practices.
Demonstrate effective documentation skills; ensure all interactions are documented in the designated electronic health record in accordance with policies, procedures, and industry or organizational standards of practice.
Adhere to call center policy and procedures.

Knowledge, Skills, Abilities:

Or a comparable combination of education/experience and/or training will be considered equivalent to the education (Master's degree / Bachelor's degree + 1 year BH experience / Associate degree + 2 years BH experience)

Behavioral Health Specialist - PRN POSITION SUMMARY: The Behavioral Health Specialist (BHS) is responsible for the safety of all clients and the facility, supporting clients towards their treatment goals, and reporting any required maintenance of the facility. Assist in maintaining a safe, recovery-oriented therapeutic environment
Provide intervention and support including listening, redirection, and guidance

Document daily client activity in milieu progress notes
Observe and document client head count in Bed Check log
Document and submit incident reports in a timely manner
