follows-up with facility and/or payer daily, if authorization is not obtained within 24 hours * Contacts referral agencies to make post discharge arrangements for patients, including verification of bed availability * Confirms actual and projected discharge dates with patient, family, and/or patient representatives; ensures transportation is arranged * Updates post-acute providers of patient's discharge condition and final discharge plans * Reassesses and documents discharge needs throughout the patient stay at minimum every 3 days, or as patient condition changes; This is a rewarding position helping youth in need with the potential for continual career advancement in a fast-growing community-based health care youth facility. The Youth Empowerment Specialist works directly with youth ages 10-17 in a Therapeutic Group Home setting. RESPONSIBILITIES: Performs basic youth care procedures and related task-oriented activities. Assumes the responsibilities of providing youth care in a therapeutic environment. Provides the safety and security of the group home and the youth while performing job duties. Reliability is of paramount importance as the youth and other staff rely on you to be there. FLSA STATUS: Non-Exempt RESPONSIBILITIES: Perform basic youth care procedures and related task-oriented activities. Assist youth with activities of daily of living and engages in 1:1 interaction. Assist with admitting, discharging, and transferring of youth. Provide direct youth care assigned by the Program Manager. SUPERVISORY RESPONSIBILITIES: This position does not supervise other staff members, however this position is responsible for youth and youth activities. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION AND/OR EXPERIENCE: A bachelor's degree in human services field and minimum of 6 months previous experience working in a therapeutic group home preferred. REASONING SKILLS: Ability to define problems, collect data, establish facts, and draw valid conclusions using critical thinking skills.
