Intervention Specialist Key Responsibilities:
Lesson Plan Development: Create effective lesson plans that include accommodations and consultation with general education teacher as needed. Plans should accommodate various student levels, include procedures for assessing knowledge, outline the content standards addressed, and specify the curriculum used. Adapt the general education curriculum according to needs outlined in the IEP.
Supervision of Paraprofessionals: Oversee and direct paraprofessionals assigned to the classroom. While Program Supervisors and Site Administrators will assist in supervision, it is the teacher's primary responsibility in the classroom.
Implementation of PBIS Program: Manage the classroom's Positive Behavior Intervention and Supports (PBIS) program, including tracking behavior points, maintaining the student store, and communicating with parents/guardians.
Classroom Management: Maintain a classroom environment that prioritizes student engagement and safety, implementing the behavior plan, modifying state-adopted curriculum, and employing strategies to effectively teach students with various disabilities.
IEP Meetings and Documentation: Attend IEP meetings for students assigned to the classroom. Write updates for each student's annual IEP in coordination with the state standards, school district expectations, and enter all IEP updates (Present Levels, BIP, ITP, and Goals) into the dedicated IEP documentation system prior to the meetings. Maintain documentation on IEP goals, student academic and social growth, prepare progress reports. Ensure documentation is accessible and provided upon request in a timely manner. Ensure confidentiality of records according to district and FERPA regulations.
Collaboration: Work collaboratively with other staff and outside agencies to effectively educate each student.
Assistive Technology: Support implementation and use of assistive technology including, but not limited to, augmentative communication devices.
Instructional Planning: Plan instruction and design learning experiences based on data. Conduct summative assessments with each student in accordance with IEP goal reporting timelines. Administer, transfer, and interpret educational assessment data for supporting educational decision making and goal development.
Professional Development: Participate in ongoing professional development activities as directed by the district including teacher and all staff trainings.

Summary:

Completion of all CQI data collection and implementation of all CQI corrective actions as assigned. Monthly participation in the Utilization Review and Peer Review programs. Other duties as assigned.

ESSENTIAL FUNCTIONS:
Assess the well being of clients and mitigate any identified risk with direct supervisor and other appropriate team members.
Visit and call clients to assure the quality and appropriateness of services provided to each client.
Submit documentation of calls and visits within 48 hours of the event to the Supervisor.
Provide services to each client based on the service needs defined in the Individual Service Plan.
Participate in the development of an Individualized Service Plan (ISP) to meet program requirements and to establish the services to be provided for each client.
Provide written progress reports on each client served that specifically address elements of the ISP.
Coordinate services to assure effective communications with the treatment team.
Utilize various assessment tools to prioritize and target problem behavior for use in the ISP.
Attend professional conferences and seminars to comply with state regulations regarding training hours.
Meet with county/other mental health personnel to provide a report on clients progress.
Maintain productivity requirement based upon employment status as defined in Bair policy.
Qualifications
JOB QUALIFICATIONS:
High school diploma or GED with 3 years of experience with at-risk youth.
Bachelors degree in Social Work or related field preferred.

Essential Functions:
Monitor client symptoms as well as response to treatment; communicate changes with treatment team (consists of social workers and case managers) to ensure immediate and appropriate interventions
Develop and maintain a service relationship with client, family, and caregivers to provide them support as appropriate.
Coordinate community resources including school, referral sources and outpatient behavioral health providers to ensure continuity of care; acts as an advocate for the client as needed.
Provide crisis intervention consultation in times of emergencies for coordination and assistance in crisis management and stabilization as needed.
Facilitate appointments to ensure overall physical well-being including basic, mental health and substance use needs.
Review Diagnostic Evaluation and additional assessments, to aid in the ongoing development and implementation of a client's individualized treatment plan (ITP).
Attend and participate in agency/ team/clinical meetings, including mandatory trainings, and treatment coordination.
Perform related administrative responsibilities; maintain client information, reports, records, and other documents; respond timely to emails and phone calls; attend and participate in various trainings and meetings; communicate with other agencies, departments, and vendors regarding required paperwork, contract compliance and consult with other county agencies and/or supportive services as necessary.
Accurately and promptly, document and maintain the confidentiality of client and family interactions including, legal, financial, and clinical documentation within agency guidelines.
Meet with Supervisor for weekly supervision.
Maintain ongoing frequent contact with other assigned OhioGuidestone professionals, parents or outside agency professionals and social service agencies assigned to case.

Therapeutic Behavioral Service (TBS) service activities include, but are not limited to the following:
Consultation with a licensed practitioner or an eligible provider, to assist with the individual's needs and service planning for individualized supports or care coordination of healthcare, behavioral healthcare, and non-healthcare services and development of a treatment plan;
Case management, referral and linkage to other healthcare, behavioral healthcare, social service agencies and non-healthcare services to avoid more restrictive levels of treatment;
Interventions using evidence-based techniques;
Identification of strategies or treatment options;
Restoration of social skills and daily functioning; and,
Crisis prevention and amelioration.
Education/Experience:
High school diploma or equivalent required, Associates or Bachelors degree preferred
Three years relevant experience and able to demonstrate competencies in basic behavioral health skills, for example:
understanding of human behavior, mental illness, psychiatric symptoms, and impact on functioning and behavior;
understanding how to therapeutically engage a mentally ill person, concepts of recovery/resiliency
understanding of the mental health system; de-escalation techniques
Understanding of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of emotional and mental dysfunctions.
Understanding of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, and cultures and their history and origins.
Relevant experience or course of study in behavioral healthcare helpful

Job Description
Required Education, Experience and Qualifications
High school diploma or equivalent required; CPR, First Aid, AED, and Narcan certifications preferred.
At least 1 year of experience with survivors of trauma, mental health, or addiction preferred.
Valid Ohio driver's license and insurability; ability to pass background checks (Ohio Bureau of Criminal Investigations and Federal Bureau of Investigation)
Role and Responsibilities
Provide confidential, trauma-informed support to survivors, including but not limited to: crisis intervention, advocacy, hotline responses, shelter intakes, referrals, safety planning, and follow-up care.
Assist with shelter operations, including: providing shelter coverage, safety, transportation, conflict resolution, assist with cleaning/food prep as needed, cleaning responsibilities, facilitating workshops for survivors on self-sufficiency skills, and provide daily survivor engagement.
Complete timely and accurate documentation in accordance with agency policy.
Collaborate with agencies to secure services for survivors and support case managers and H&H staff in individualized survivor plans.
Participate in professional development and community representation.
Always adheres to Service Excellence standards.
Complies with agency policies and procedures, federal and state requirements, CARF standards and educational/certification/registry requirements.

Job Description The Family and Community Engagement Specialist will recruit families and support providers, families, and the community by providing; parent cafés, resources, referral services, parent training, parent advocate home visits, provider updates and technical assistance to their caseload of child care providers around parent engagement and parent community consumer education.
Duties
Recruit families and identify opportunities to reach families with resources related to:
-High-quality early care and education settings - SUTQ, Accreditation, etc.

-Fully implement the CCRC mission through a caseload model of service delivery and meaningful relationships.
Maintain an accurate database (Work Life Systems) of all providers on your caseload within the CCRC service delivery area. Must ensure provider updates are completed every six months.
Refer families to providers in database and contact parents to verify program effectiveness, questions or additional information. Referrals are emailed, mailed or provided over the phone.
Provide Parent Advocate Home Visits that empowering the parent through:
-Parent-child relationships; home-school relationships; co-coordinator-parent relationships; and parent-parent/ community relationships
-Educating parents re: children's physical and emotional development and realistic expectations of child behavior
-Role-modeling appropriate parenting techniques including positive communication, listening and age-appropriate limit setting and discipline
-Enhancement of home-management skills such as, budgeting, organization and nutrition
-Support in times of crisis
Provide services to families related to agency grants or projects (i.e. strengthening families, Parent Café's, Parenting Wisely, Triple P,Foster Families, Trainings etc.)
Help educate families, and community organizations on child development issues, nutrition, and other CCRC services through outreach, consultation, meetings, and newsletters.
Contribute to leadership in the community concerning advocacy for quality child care.
Represent the agency at various meetings as assigned.
