Carries case load up to 25 clients, per State and contract requirements.
Collaborates with clients to develop discharge summary and continuing care and relapse prevention plans for supervisor's approval to integrate aftercare treatment with referral resource services.
Coordinates required services to resolve emergency problems in crisis situations in support of continued recovery.
Collects and maintains treatment records and reports to ensure organizational, regulatory, accreditation, and contractual compliance.
Performs office administrative duties, such as answering telephones; relaying messages; responding to inquiries regarding program services and policies; filing, photocopying and faxing documents; planning special events, assembling intake packets, etc.
Job Requirements:
Must have a mimimun of a Bachelor's Degree in a helping profession from an accredited college or university
Some previous counseling experience, experience in substance use disorder or mental health treatment is preferred
Must obtain certification as identified by state within 24 months of the first date that the counseling services are provided. Acceptable Certifications include Certified Addiction Counselor from the Georgia Addiction Counselor's Association (GACA), Certified Addiction Counselor from the National Association of Alcoholism and Drug Abuse Counselors (NAADAC), Certified Drug and Alcohol Counselor from the International Certification and Reciprocity Consortium (ICRC), and Certification as a Certified Clinical Supervisor (CCS). Acceptable Licensures include Licensed Professional Counselor (LPC), Licensed Associate Professional Counselor (LAPC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), and Certified Professional Counselor Supervisor (CPCS).
Must be able to obtain Georgia Department of Corrections clearance to work in the correctional institution

JOB DESCRIPTION
GENERAL DESCRIPTION:
The mission of Metrocare Services is to serve our neighbors with developmental or mental health challenges by helping them find lives that are meaningful and satisfying. We are an agency committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, development disabilities, and co-occurring disorders. Metrocare programs focus on the issues that matter most in the lives of the children, families and adults we serve. The Metrocare Services Mental Health and Primary Care Division includes a Crisis Services Program that provides 24/7 services to those in need via the crisis hotline and mobile crisis outreach teams. Crisis services are offered for to anyone, anywhere, and at any time the need arises. Crisis services will be provided regardless of an individuals ability to pay or whether he/she is homeless or unable to produce verification of residence. The Crisis Interventionist Specialist-QMHP plays a vital role on a team comprised of a Licensed Practitioner of the Healing Arts and a Mental Health Peer Specialist.
Work Hours: 11am-7pm (2nd Shift)
ESSENTIAL DUTIES AND RESPONSIBILITIES:
The essential functions listed here are representative of those that must be met to successfully perform the job.
Provide crisis intervention services in a variety of modalities, including telephonic, telehealth, and face to face intervention.
Ensure follow up is provided within 1 hour of the emergent crisis
Assists inpatient facilities, including emergency departments, behavioral health hospitals, state hospitals, residential treatment facilities and the criminal justice system, in continuity of care arrangements for consumer referred to Metrocare
Hotline staff screen/assess the crisis, provide an evidence-based intervention, and stabilize the crisis. The caller is then linked with service coordination assess the level of further stabilization needs and appropriateness for other services in the crisis continuum
When Hotline interventions are not able to adequately stabilize the situation, the MCOT team to further assess the crisis. If screening and assessment indicates that there is an emergent/imminent risk of harm to self or others, the Hotline staff notifies 911 and attempts to stay on-line with the caller until 911 personnel arrive on site where the individual is present.
Develop person-centered crisis plan and assist the individual in executing the plan during and post crisis intervention.
Ensure continuity of care post crisis intervention to appropriate outpatient services, including but not limited to, outpatient mental health, substance abuse services, and/or primary care services.
Be accessible for referrals and communication through our electronic health applications, by phone and email to provide a timely and comprehensive response.
Interacting with other team members to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving services
Report all clinical crisis staffing to supervisor or designee
Facilitate communication between providers within a schedule that meets outlined timeframes
Develop close working relationships with internal and external referral resources
Maintain confidentiality of information concerning individuals and family members
Attends and participates in all scheduled team meetings.
Assist clients in linking with resources and a system of natural supports.
Assists individuals in adjusting to new environments.
Completes paperwork requirements and maintains quality, up-to-date clinical records. Documents any clinical or support services within 24 business hours of providing the service. Documentation requirements will include Release of Information and Consents to expedite the transition of care.
Works with family members/support system to gain support in everyones success.
Is knowledgeable about and remains in compliance with internal and external policies, procedures, regulations, and standards, including, for example, all Metrocare policies and procedures and Quality Assurance Plan, all applicable regulatory standards.
Performs other duties as assigned.

JOB DESCRIPTION GENERAL DESCRIPTION: The MAT LCDC provides interventions to assist consumers with substance abuse issues, specifically related to chemical dependency assessment, intensive outpatient (individual and group), and support outpatient (individual and group). The LCDC assures services as defined on chemical dependency service plan, and the consumer is actively working toward stated objectives and the family and natural supports are involved as appropriate. The MAT LCDC solely works with those clients who are enrolled in MAT services at Metrocare to assist in safe and successful recovery. ESSENTIAL DUTIES AND RESPONSIBILITIES: Provide individual and group treatment as part of the intensive and supportive outpatient programs at Lifenet or the Bridge. Possess knowledge of substance abuse issues and symptomology, DSM V diagnostic criteria and treatment methods. Maintain performance benchmarks Conduct CD Assessments, Service Authorization Requests, and Service Planning. Accurate and timely data entry into the state system, CMBHS, including CD Assessments, Service Authorization Requests, Service Planning, Progress notes, and Psychoeducation notes. Complete documentation of services provided to individuals within 24 hours of service. Performs other duties as assigned. COMPETENCIES: The competencies listed here are representative of those that must be met to successfully perform the essential functions of this job. Analytical skills, professional acumen, business ethics, thorough understanding of continuous improvement processes, problem solving, respect for confidentiality, excellent communication and presentation skills. QUALIFICATIONS EDUCATION AND EXPERIENCE: Masters degree with 2 (two) years experience, OR Bachelors degree with 4 (four) years experience in MH/MR or related field. LCDC licensure required. REASONING ABILITY: Ability to be responsive to consumer, family and community needs. Ability to conduct CD assessments Ability to provide CD/substance abuse intervention/prevention in group and individual settings Ability to assist in CD discharge planning with consumers Ability to work flexible hours Ability to thrive in a high stress environment Ability to provide Community Support Services to consumers. Ability to maintain positive attitude. Ability to successfully use an automated clinical record keeping system. COMPUTER SKILLS: Intermediate level on MS Excel, Email and word processing programs Ability to utilize Internet for resources. Organizational & time management abilities.
