As a Social Worker at US Renal Care, you will be an integral part of a cross-functional team, working to help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation.
What you will be doing
Advocate & Support. You will be part of an interdisciplinary team working to ensure patients receive the best care, including conducting all required patient assessment and care planning activities such as assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and all state/CMS regulations. You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies.
Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care. You will participate in all required continuing education and staff meetings. You will collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community.
Safety & Quality. You will help with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes. You will also participate in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with federal, state, and local laws and regulations.
What we're looking for
Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
Current licensure (in good standing) in applicable state is required unless employed in the state of AZ, PA or Guam. Must meet any practice requirement(s) for the applicable state.
Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
Basic computer skills, including Microsoft Office (Word, Excel, Outlook).
Proficiency in all USRC clinical applications required within 90 days of hire.
Preferred
Previous experience in providing social services to dialysis patients preferred.

Job Description
This position is contingent on contract award.
International SOS is looking for an individual who is an experienced Licensed Substance Abuse and Rehab Counselor to provide comprehensive support and therapy to individuals struggling with addiction and substance use disorders, facilitating recovery through individualized treatment plans and therapeutic interventions. They also educate clients about coping strategies, relapse prevention, and resources for maintaining long-term sobriety, promoting overall mental health and wellness.
Key Responsibilities
Perform a full range of Certified Addiction Counselor duties, in accordance with assignment and consistent with the scope of practice for profession, including but not limited to, medical care treatment and patient education in a knowledgeable, skillful, consistent, and continuous manner.
Obtain or provide relevant information that will facilitate treatment and assist in developing programs.
Provide assessment of substance abuse and addiction.
Identify and document medical consequences of alcohol and other substance use, including withdrawal and detoxification.
Facilitate the placement and management of patients in appropriate levels of care within the continuum of care model of the treatment of addictive disorders.
Assess each patient and perform triage duties as assigned.

Qualifications
Basic Requirements/Certifications:
Maintain a national certification credential by the National Association of Alcohol and Drug Abuse Counselors or National Board of Certification Counselors or International Certification & Reciprocity Consortium/Alcohol and Other Drug Abuse.
Possess a minimum of one year experience within the last three years as a Substance Abuse and Rehabilitation Counselor working in an out/inpatient setting.
Current, full, active, and unrestricted license to practice as a Certified Addictions Counselor.
Ability to acquire and use basic computer skills with such programs as Microsoft Office, and general data entry functions to enter patient data, and extract patient information via the electronic health record.
Education Required:

Examples of Duties
* Oversee and monitor the work performance of behavioral health specialists, school social workers, and other assigned staff regarding the provision of school-based behavioral health services within a complex or district.
* Responsible for evaluating the work performance of assigned staff and completion of their performance appraisal (PAS) within the required timeframe.
* Monitor activities and services of staff for conformance to existing laws, policies, procedures, standards, and guidelines, etc. Take corrective action/steps to remedy non-conformance in order to comply with legal requirements.

Minimum Qualifications
Basic Education Requirements: Possession of a Master's degree from an accredited college or university with a major in counseling, marriage/family therapy, psychology or social work.

Salary: The advertised salary is based on full-time employment and includes shortage and school year differentials, if applicable.

Responsible for evaluating the work performance of assigned staff and completion of their performance appraisal (PAS) within the required timeframe.
Monitor activities and services of staff for conformance to existing laws, policies, procedures, standards, and guidelines, etc. Take corrective action/steps to remedy non-conformance in order to comply with legal requirements.
Ensure that appropriate services are delivered to students by program staff as specified in student Individual Education Plans/504 Modification Plans (IEP/MP). Provide technical support/assistance to principals, counselors, teachers, and other school personnel regarding integration of school-based behavioral health services into educational programs, and meeting the requirements of federal laws.
Assist the district educational specialist (DES) with program development and the overall plan for the implementation of the district's/complex's school-based behavioral health services program including an evaluation strategy to effectively monitor and utilize behavioral health and the contracted provider resources.
Administer policies and procedures pertaining to the implementation and operation of school-based behavioral health services.
Conduct budget surveillance and utilization procurement review for the SBBH DES to assist in identification of resources needed to implement a system of school-based behavioral health services.
Review requests for contracted services and monitors for appropriate utilization of services and resources in conjunction with the DES and district contracts specialist.
In consultation with personnel from the DOE district and State level offices, the Department of Health Family Guidance Center, and Children's Community Council (CCC), develop a community-based system of care.
Plan and implement a complaints resolution process for behavioral health-related services. Monitor sentinel events/incident reports, investigate and collaborate with the agencies involved to resolve the situation. Refer unresolved system issues to the appropriate district resources.
Develop and implement education and/or training programs to address staff development needs related to behavioral health evidence-based practices and IDEA/504.
Train subordinates and assigned staff.
Review IEP/MP quarterly reports, direct and oversee the recordkeeping systems.
Sign documents for Medicaid reimbursements.
May provide direct services.
Minimum Qualifications
Basic Education Requirements: Possession of a Master's degree from an accredited college or university with a major in counseling, marriage/family therapy, psychology or social work.
Experience Requirements: Applicants must have had progressively responsible experience of the kind and quality described in the statements below and in the amounts shown below or any equivalent combination of training and experience.
Three (3) years of progressively responsible professional post-masters work experience providing counseling and/or therapy to individuals and/or their families which included assisting individuals to address emotional, behavioral, and/or mental health problems. This experience must have involved counseling or guidance in situations where individuals had difficulty in grasping or understanding the nature of their problem(s), choosing between various possible courses of action, adjusting to environmental conditions and/or acquiring or sustaining appropriate individual control or motivation.

The Substance Use Counselor (SUC) is responsible for providing low threshold opioid use disorder (OUD) treatment services to the following focus population: persons who inject drugs (PWID), men who have sex with men and inject drugs (MSMID), and transgender person who inject drugs (TGID). Treatment services will also be provided to participants of HHHRC's Syringe Exchange Program (SEP), at-risk individuals engaged during outreach efforts and interested HHHRC participants currently engaged in other agency services. S ummary of Program and Position:
Voluntary Case Management (VCM) program serves families on Oahu who have been referred by Child Welfare Services (CWS) after a report to the Child Abuse and Neglect Hotline has been made and the family has been identified as moderate to moderate high risk. Each family will be assigned a case manager who will address the range of family system issues that place children at risk of child abuse and neglect. They may also be assigned a skill builder who will work with the family to increase skills such as, but not limited to, parenting skills, co-parenting skills, problem solving skills, and stress management skills.
The Case Manager provides voluntary, culturally appropriate, and strengths-based services related to child safety concerns to families referred by the Department of Human Services who are moderate to high risk for child abuse and neglect. These services are focused on addressing all reported concerns, increasing parents' ability to establish and maintain safety for their children, meet their children's needs, problem solve, and access community resources. Services are provided at the home or other “natural” environments preferred by the families. The Case Manager works with referred clients to engage families, refer, and coordinate services, plan, ensure children's safety, provide parenting and family strengthening activities including persistent and creative outreach, monitor progress and document family status. This position is under the direct supervision of Program Supervisor.
Essential Duties:
Ensures that the delivery of service is done in a manner consistent with the standards and guidelines established by the funding agencies, state law, and the Parents And Children Together policies and procedures.
Provides assessment and lead case management services that ensure that families gain access to appropriate resources in an efficient and timely manner.
Assesses each family's strengths and weaknesses, identifies their needs, and develops goals and objectives as part of the Family Partnership Plan and Family Partnership Plan Activities developed with the family. Completes required assessments (Child Safety Assessment and Comprehensive Strength and Risk Assessment Tool), including the Adult-Adolescent Parenting Index according to required timelines.
Provides social services consisting of parenting skill building, child development education, alternatives to corporal punishment, increasing knowledge and skills in meeting child and family needs, assistance in developing a support network, preparation for longer-term services, and other areas as needed to reduce risk factors for child abuse and neglect and increase protective factors.
Links families to needed community resources and conducts follow up activities as needed, teaching the family to identify and access resources throughout the process. Ensures all services on the Family Partnership Plan and the Family Partnership Planning activities are in progress and/or completed.
Meets at least once a month with the Program Supervisor to discuss progress on all families, and notifies their supervisor of progress, challenges, and any significant and critical issues, including incident reports. Consults weekly with Program Supervisor on top tier families and as needed for a family in crises. Case consultation with supervisor includes following recommended actions and the review and approval of all reports by supervisor.
Completes clear, concise, and non-judgmental written and electronic documentation, such as initial assessments, monthly updates, and closing reports, and ensuring that all paperwork is completed and submitted in a timely manner for review and approval by the Program Supervisor.

Provide appropriate prompting and, when necessary and appropriate to their treatment plan, directly assist consumers, with ADL's, including assistance with personal hygiene and self-care, cooking, cleaning, social skill development, and participation in activities.
-Work as a team with other Group Home program staff to ensure a consistent, structured, and therapeutic environment is maintained at all times.
-Provide medication monitoring services as needed or assigned.
-Maintain knowledge of established rules governing participation, behavior, and general expectations concerning consumers. Reinforce rules and expectations as necessary. Promptly report issues or concerns and seek support or clarification from the Site Manager(s) and/or Residential Manager.
-Document pertinent observations and any other relevant information in the record on a daily basis. Work closely with the assigned Case Manager as necessary, in order to maintain assigned records; ensuring that required documentation is completed and/or updated in a timely fashion.
-Catch bus with/Drive consumers (in company owned vehicles), as necessary, to facilitate participation in off-site activities, attend appointments, or to assist with shopping or other personal needs. Complete a vehicle safety inspection and complete the vehicle mileage log each time the vehicle is used. Promptly report vehicle maintenance or safety needs to the Site Manager.
-Participate in the daily general maintenance of the residence and yard. All residential sites are to be maintained in a neat, orderly, and clean condition that will ensure they will pass an inspection by any contracting or regulatory agency.
-Ensure that appliances and safety equipment are maintained in working order. Promptly report needs to the Site Manager for follow-up.
-Follow-up, as necessary, on needs addressed in the weekly inspection reports. May make minor repairs as necessary. Promptly report major repair needs or problems to the Site Manager.
-Maintain a thorough knowledge of emergency procedures and appropriate evacuation procedures and ensure they are followed. Maintain a clearly posted evacuation route in case of emergency, emergency contact information next to each phone, and clearly posted first-aid information. Ensure that appropriate safety equipment such as flashlights, smoke detectors, and the first-aid kit are properly functioning and/or stocked with necessary supplies. Promptly report needs to the Site Manager for follow-up.
-Assist in ensuring that adequate supplies and equipment necessary to support the day-to-day needs of the Group Home program are available. Promptly report needs to the Site Manager for follow-up.
-Any other duties as assigned and appropriate to the position.
