Ministry Program The Behavioral Health Specialist organizes and implements behavioral health services to Senior CommUnity Care participants and families. Provides behavioral health assessments, individual and group counseling in formats appropriate to clinical populations served. Collaborates as part of an interdisciplinary team and promotes professional working relationships with both internal and external customers. QUALIFICATIONS: Education: Master's Degree from an accredited school of social work or counseling required. Experience: A minimum of one year's experience working with frail or elderly population required. ESSENTIAL FUNCTIONS: Upon referral from Interdisciplinary Team (IDT) or Social Worker, performs in person initial assessments of Senior CommUnity Care participants to obtain a complete psychosocial history, which may include descriptions of cognitive status, social supports, family dynamics mental health and substance dependency and other issues and needs. Coordinates with the IDT to develop a comprehensive plan of care for each participant receiving behavioral health services. Reassesses participants as needed to assess progress in therapy. Keeps IDT apprised of changes and progress in treatment. Functions as a consultant to the IDT. Attends IDT when necessary to present updates related to significant changes with participant status, education related to care planning for participants receiving behavioral health services, or in absence of the social worker. Ensures that Social Workers are kept informed of behavioral health services offered to participants and coordinates care in conjunction with the Social Work Team. Provides ongoing support, counsel, and education to participants and family regarding a variety of issues, including but not limited to: mental health disorders, coping strategies, grief and loss, the aging process, and treatment for behavioral health. Presents recommendations to IDT for recommended services to support participants with mental health needs and psychiatry referrals. Supports Social Workers with coordination of care with psychiatry and external counseling services. Supports participants with access to community support regarding mental health services, self-help and other resources related to behavioral health. Follows all mandated reported guidelines regarding reporting of suspected abuse or neglect of an adult and coordinates with social worker as needed. Acts as a resource to other team members, including nursing, transportation, day center and home care staff, and therapy staff regarding topics such as severe and persistent mental health diagnoses, dementia, difficult behaviors, and difficult personalities. Completes and ensures completion of documentation of clinical service, in participant's medical records including initial assessments, re-assessments, counseling sessions, home visits and other significant events according to Senior CommUnity Care documentation requirements. Assists participants and caregivers in filing grievances. Advises on day center or other programming offered via Senior CommUnity Care to meet the behavioral health needs of participants and caregivers. Acts within scope of his or her authority to practice. Follow all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines. Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants, and families. Maintains safe working environment. Follows Senior CommUnity Care Safety policies and procedures. As appropriate, participates in and supports Quality Improvement Initiatives. Position: Full Time - Substance Use Counselor Department: Women's Health Project Job Description: CDPHE RW Part B - Women's Health Project (WHP). The WHP Substance Use Therapist position works full-time with women who are living with HIV/AIDS. Comprehensive, individualized substance abuse services are provided to these women. The WHP Substance Use Counselor works with the participant's care managers who utilize a participant-centered process that links clients to housing, health care, mental health, substance abuse, and other support services to produce more positive and sustained outcomes. The Family Services Outpatient Clinician will provide individual, family, and group therapy to at-risk children ages 8-22. Utilization of an episode of care model is essential. Hours would include either a 4/10s schedule (working from 8:00 AM-6:00PM 4 days/week) or a 5/8s schedule (working from 10:00 AM-6:00 PM 5 days/week), using a hybrid (online and in-person) model. Essential Duties: * Provide individual therapy as the primary modality as well as family and group therapies. * Engage client and family in the treatment process and goal-setting. Identify risk factors including lethality, and effectively safety plan with client and family involvement. * Work constructively with client and family to reach agreed upon outcomes, and coordinate care with internal and external providers. * Provide case management services including consulting with psychiatrists on clinical issues, and consulting with all parties essential to client welfare including but not limited to: school district, child welfare system, juvenile justice system, primary care physicians, and other community resources. * Utilize a trauma-informed lens. Supervision and Continued Learning & Development * Attend mandatory team meetings, trainings, and consult groups. * Attend optional trainings of interest, Center town halls, and other events. * Attend supervision at times and intervals agreed upon with supervisor and participate in supervision by coming prepared with an agenda. Report high risk/problem cases, along with what is going well and engage in reflections on successes. * Utilize peer and team supports, within FSOP and across other teams. * Stay abreast of current trends and changes in the field. * Review challenging cases with peers and supervisors in monthly consult meetings. * Be open to supervisors observing sessions and providing developmental feedback about your approach and cases. Administrative and Compliance * Submit 90% of all Progress Notes within 3 working days (72 hours). * Maintain 90% of updated annual CCARs throughout the year. * Complete 90% of case closings within 60 days after last contact. * Complete 90% of all treatment plans within 6 months of the last update. * Complete 90% of monthly Relias (online) trainings on time. * Ensure that fee collections are timely if clients have co-pays or pay out of pocket. * Meet required number of client-facing hours as determined by the Center (currently 50% of a clinician's time is required to be client-facing). * Effectively document medical necessity, treatment progress, safety risks, and other required information (informed by Center and peer audits). * Comply with all corporate compliance and HIPAA standards. * Complete data tracking required by grant funding sources, including required assessments such as the NOMS and required monthly reports to Department of Health and Human Services for open Dependency and Neglect cases. * Effectively use technology to maintain an efficient working environment.
