Assist with medication support, crisis prevention planning, and skill-building activities. Promote autonomy and recovery by meeting clients where they are-physically and emotionally. Qualifications Required: IM+CANS certification (Illinois Medicaid - Community and Supports Assessment). Bachelor's degree in social work, Psychology, Human Services, or a related field. At least 1 year of experience working with individuals with serious mental illness. Valid Illinois driver's license, auto insurance, and reliable personal transportation. Strong interpersonal skills and the ability to work independently in community settings. Tech-literate; comfortable with electronic documentation and mobile communication.
