Serve as the point of contact for coordination and communication with other systems where clinical knowledge of the case is crucial.
Assist in screening and coordinating primary care services for clients.
Requirements:
Combination of ONE of the following education and licensure levels required
High school diploma or GED and active CDCA license
CDCA - Preliminary licensure acceptable
Associates degree in Behavioral Science and active LCDC II license
Bachelor's degree in Science and active LCDC III license
Master's degree in Behavioral Science and active LICDC license
Behavioral health experience preferred

WellBe LCSW - Social Worker - What to Expect:
Practices the WellBe mission: To help our patients lead healthier, meaningful lives by delivering the most Complete Care.
Performs psychosocial assessments and provides therapeutic services to patients and their families.
Work closely with Behavioral Health Medical Director to manage complex patient cases and transitions of care
A resource for the WellBe communities in managing complex Behavioral Health cases.
Manages complex behavioral and psychosocial needs that result in improved clinical and financial outcomes
Maintains constant communication with patients when addressing their concerns and goals and helping them keep positive attitudes
Builds rapport with patients and their families and relay all concerns to WellBe provider/IDT

Monitors patient progress and adjusts patient treatment plan as needed

Involves the patients' families and primary caregiver(s) as needed to achieve the best care decisions and outcomes.
Effectively collaborates with all those involved in the members' care, including health services contractors (i.e., Home Health, Hospice, Community Agencies), to meet the patients' care goals.
Consults with and advises the Clinical Team regarding the relationship of social, emotional, and cultural factors to health and medical care, and to the availability of social services in the community.
Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to their supervisor/team. Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors and others.

Master's Degree in Social Work
5+ years clinical social work experience with the geriatric population

Duties: In this role, you will be provide crisis intervention services to individuals in community settings in Licking County. Services being a Public Health Officer, with the ability to write involuntary admissions, and completing Hospital Pre-Screens for Psychiatric admissions or community safety plans, as well as mobile crisis services for children and adults. Each staff is designated a specific shift and location, for a 40 hour work week, with the addition of a one (1), twelve (12) hour On Call shift a month, and on a rotation basis to provide a required holiday coverage, once or twice a year. Staff are compensated for On Call hours and overtime for any worked hours during that On Call shift. Each staff has access to the Crisis Supervisor 365 days of the year, 24/7, for consultation purposes, as well as to team members working the same shift. Position Summary
Provide supportive services to children, adolescents, and their families in the home, school, and community setting. Performs psychosocial assessments and provides therapeutic services to patients and their families.
Work closely with Behavioral Health Medical Director to manage complex patient cases and transitions of care
A resource for the WellBe communities in managing complex Behavioral Health cases.

We are currently seeking a full-time Case Managers for multiple departments in Lancaster, Ohio.
POSITION DESCRIPTION:
DIRECT CLINICAL SERVICE: Provides on-going primary and secondary community services to Adult SMD's, serves as an advocate, liaison, mediator, broker for SMD's, provides transport for clients to services, promotes a client/driven, strength-oriented service, adheres to the agency's mission statement, policy and procedures, follows guidelines to ensure quality assurance, maintains productivity and documentation standards, keeps current licensing credentials. Develops and implements initial and revised Individual Service Plan. Communicates with other service providers to improve quality of care to client.
Manifest a commitment to and Recovery Model philosophy and standards, and foster an environment that supports recovery for persons served. Possess a working knowledge of community resources. Be sensitive to the cultural needs of the individual and/or family served.
DOCUMENTATION: Completes all appropriate case documentation and case planning information; completes all necessary reports per agency policy and protocol; appropriate documentation of collateral contacts; participates in clinical supervision.
Performs related administrative duties; attends staff meetings, supervisions, trainings, assists in program planning and evaluation. Non-billable outreach to clients not engaging.
Other duties as assigned.

Requirements
QUALIFICATIONS:
Ohio driver's license, proof of automobile liability insurance (minimum $100,000), Bachelor's degree preferred. Preference given to holders of Ohio Counselors and Social/Work Board licensure. Experience in working with adults with SMD, exhibits respect, compassion, warmth, caring and friendliness, non-judgmental of varying cultural beliefs, ability to assess, teach and model skill development techniques in home maintenance, interpersonal-social and pre-vocational interests, ability to set limits, confront behaviors and redirect.

Crisis Intervention Specialist
Duties: In this role, you will be provide crisis intervention services to individuals in community settings in Licking County. Services being a Public Health Officer, with the ability to write involuntary admissions, and completing Hospital Pre-Screens for Psychiatric admissions or community safety plans, as well as mobile crisis services for children and adults. Each staff is designated a specific shift and location, for a 40 hour work week, with the addition of a one (1), twelve (12) hour On Call shift a month, and on a rotation basis to provide a required holiday coverage, once or twice a year. Staff are compensated for On Call hours and overtime for any worked hours during that On Call shift. Each staff has access to the Crisis Supervisor 365 days of the year, 24/7, for consultation purposes, as well as to team members working the same shift. There are no productivity requirements or on going caseloads, and each day is varied, can be fast paced and often very interesting. Opportunities for additional training and advancement in professional growth are available, and encouraged.

Qualifications: Candidates must possess strong written and oral communication skills and the ability to collaborate with other service providers. Minimum of Bachelor's Degree in Social Work or Counseling with one to three years of social work experience. Independent licensure preferred. Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC); State of Ohio Driver's License. BLS/CPR certification required. Basic computer, phone and typing skills are necessary for all positions.
