As a Licensed Clinical Therapist / Independent Licensed Therapist, you will treat a wide variety of mental health conditions that reflect the needs of our diverse patient population. You will provide patients with behavioral and mental health treatments, including individual and group therapy, medication management, and intensive outpatient programs.

Primary Responsibilities:
Form excellent provider-patient alliances and coordinate care with external providers
Conduct outpatient psychiatric evaluations and provide ongoing medication management
Participate in enriching the training experience of our graduate students
Screen and assess patients for common mental health and substance abuse disorders
Provide treatment for a variety of mental health conditions using treatment approaches including cognitive behavioral therapy, dialectical behavioral therapy, and other evidence-based methods
Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications

Maintain accurate and up-to-date electronic medical records (Athena) and clinical documentation, ensuring compliance with all regulatory requirements

Required Qualifications:
Master's degree in psychology, social work, or a related counseling field
Clear, active and unrestricted license (LPC or LMFT) in the state of practice
Enrolled or eligible to be enrolled as a provider in the Medicare program
5+ years professional social work / clinical experience post Master's degree
Preferred Qualifications:
Experience providing direct and group psychotherapy services to children, adolescents, adults, and families
Experience working with computers for professional communication and medical documentation - Excel, Outlook, Athena RMS (or other EHRs)
Proven ability to work both independently and collaboratively with equal effectiveness

The salary range for this role is $59,500 to $116,600 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable. Registration with the Virginia Board of Counseling as QMHP A (Adult) preferred.

A Qualified Mental Health Professional - Trainee (QMHP-T), is defined as a person receiving supervised training in order to qualify as a QMHP-A/C in accordance with 18VAC115-80 and who has met the minimum requirements and is registered with the Virginia Board of Counseling.

Responsibilities:
Behavioral Assessment:
Conduct thorough assessments and reassessments (Functional Behavior Assessments (FBAs), skills assessments) to identify target behaviors.
Develop behavior intervention (BIPs) based on assessment results.
Program Development and Implementation:
Design, implement, and modify individualized treatment programs for clients based on ABA principles.
Train and supervise direct care staff (BTs and RBTs) and other team members to ensure the successful implementation of behavior intervention plans.
Monitor progress towards treatment goals and adjust interventions as needed to ensure effectiveness.
Supervision and Training:
Ensure accurate and timely data collection for all target behaviors and treatment outcomes.
Analyze data regularly to tract client progress and make data-driven decisions about treatment modifications.
Offer regular training to families, caregivers, and other stakeholders in behavioral interventions and strategies.
Collaboration and Communication:

The Case Manager works under the direction of the clinical director of care management, providing coordination of care for patients at Anne Arundel Medical Center to support safe, seamless, timely transitions across the continuum. Utilizing a collaborative process, will identify (using quantitative and qualitative methods), assess, plan, implement and evaluate the options and services required to meet an individual's health and health related needs, including social- determinants that affect ones' overall wellbeing. Promotes the right resources, at the right time and at the right level of care and is responsible for engaging and supporting patients that are in need of care management services; is able to determine, using evidence based guidelines, the correct initial and ongoing level of care for patients and is able to submit appropriate denial review for Medicare, Medicaid and commercial insurers.
Essential Job Duties:

1. Identifies and prioritizes patient in need of care management services, using a holistic approach inclusive of biopsychosocial, functional, cultural, spiritual, and financial factors; uses a multi discoplinary approach to assess/plan for care needs.

3. Utilizes evidenced based guidelines (such as InterQual or other agreed upon evidenced based guidelines) to promote quality care, decrease variation and mitigate waste. Verifies appropriate level of care; enters clinical review and authorized days in Epic; documents actions to avoid denied days; refers cases to Physcian Advisor as appropriate.

Develops and coordinates transition plans for patients transitioned to home with home health, community care coordination program, Hospice or Palliative care, home infusion and routine sub-acute and skilled post-acute providers; completes all necessary documentation and necessary handovers. Involves and prepares patients and families for transition from the ED, Peds, Clatanoff or Observation unit as indicated.
6. Maintains clear and concise documentation in each patient record to reflect physical and functional limitations, psychosocial characteristics, educational needs of patient & family, family/social support systems, financial, economic, and transition needs. Initiates referrals to disciplines as indicated.
7. Participates in nursing unit and department clinical outcome projects as well as process improvement initiatives of care management.
8. Identifies potential or current patient situations which require referral to other members of the health care team such as infection control, risk management, or quality management. Assures plan of care is adjusted as appropriate and that follow-up occurs. Keep leadership abreast of potential issues.
9. Utilizes all risk and predictive analytic tools such as the readmission risk tool. Applies tailored interventions to mitigate potential barriers or risk, prolonged unnecessary hospitalization and readmission prevention.
10. Maintains compliance with all regulatory standards (CMS, commercial insurers etc)
Educational/Experience Requirements:
BSN or ADN with equivalent experience. BSN must be achieved within 5 years of start date in the role
Three years of experience in a clinical setting, ambulatory or post-acute.
Care coordination experience preferred.
Licensure/Certification:
Current licensure as a registered nurse by the Maryland Board of Nursing.
Adherence to the credentialing requirements of AAMC as stated in the nursing bylaws.

The mobile crisis staff responds to all calls from members of the community and from the police. Persons in crisis needing MCT services may be in the community or seen at Grassroots. Primary Responsibilities:
Works assigned shift and arrives to work 5-10 minutes prior to shift starting.
Triages requests and responds to a variety of sources up to 40 hours a week as scheduled.
Completes a thorough assessment in partnership with other MCT member, police and collateral others for each new client to determine their needs and the best intervention possible.
Demonstrates ability to work with individuals (adults and adolescents), as well as couples and families.
MCT provides follow-up services to identified clients as well as family and or friends of identified client.
Completes iCarol reports for all calls at the end of each call. All calls are documented before the end of shift.
Provides crisis intervention counseling using a variety of interventions including Emergency Petition, voluntary hospitalization, conflict mediation, access to resources, etc.
Secondary Responsibilities:
Assist MCT Program Manager, and Director of Crisis Services with projects or other tasks, as requested.
Participates in program evaluation by sending surveys to recipients of service as per protocol.
Represent Grassroots at health fairs and other community events, as requested.
Uses counseling skills, empathetic listening and warmth to establish rapport with all callers and clients.
Counselor uses the crisis intervention model with all clients as is appropriate.
Communicates with peers on shift all necessary information relating to operations and client updates.
Actively participates in the various formats of Crisis Team and MCT “meetings” (videos to watch, emails to read, webinars, and or face to face meetings) to ensure that up to date information is being communicated to clients.
Positively contributes to the morale of peers and proactively problem solves client cases as well as systemic concerns, offering suggestions for addressing concerns.
Provides training and orientation to new staff as requested in a helpful and thorough manner.
Keeps workplace including MCT vehicle clean and orderly.
Agency-wide Responsibilities:
Participate in supervision, in-service training and staff meetings.
Maintain good working relationship with co-workers and contribute positively to group morale.
Complete all administrative duties including time sheets, leave requests etc.
