What You'll Do: Conduct intake with all beneficiaries, including CalOMS forms, consent forms, ROI, and assessments, as well as orientation to the program Manage bed assignments Maintain documentation in the beneficiary record, which includes intake notes, updated case notes, discharge notes, and shift reports Oversee the self-administration of medication Receive briefing from prior shift personnel on recent developments Observe and report beneficiary behavior to case managers and document in the beneficiary chart as well as shift report in HER Ensure beneficiaries attend all meetings and groups if physically able Conduct walk-throughs of facility and beneficiary rooms every 30 minutes and provide documentation of walkthrough Knowledgeable of Confidentiality laws, policies, and procedures Answer telephones and become knowledgeable about referral procedures Monitor and record vitals of withdrawal management beneficiaries as instructed, as well as conduct 30-minute close watch observation and document in the beneficiary record Complete continuing education and trainings as required by Cedar House, County, and State mandates Provide transportation of beneficiaries as needed Maintain a professional demeanor at all times Perform other duties as assigned or required Requirements: What You'll Bring: Understanding of the nature of alcohol/drug addiction and the recovery process Completed twelve years of education or have a GED Agree to enroll and complete a drug studies certificate program Will have 5 years to become certified or must be registered upon hire under an accredited drug and alcohol certification program Ability to address stressful situations in a calm manner and be able to communicate in a positive way Maintain current Adult CPR/First Aid/AED Maintain current certification and/or registration with CAADE or CCAPP Ability to work with diverse groups of people Resourcefulness and strong problem-solving skills Driving Requirements Hold a valid California Driver's License Four years of driving experience Must be at least 21 years of age No more than one at-fault accidents in the last three years No more than three moving violations in the last three years No major violation in the last 3 years CULTURAL COMPETENCIES Uncompromising integrity and ability to maintain the strictest confidentiality. Job Summary: Under the direct supervision of the assigned Supervisor, the Family Support Counselor has responsibility for providing support services to individuals and families as identified in the designated program and/or individualized plan. Essential Functions: Provides individual and family support services in accordance with the individualized plans. Services may include: providing referrals, advocacy and support to individuals and families (clients), linkage and case management services, support groups, educational services and support, transportation, home management assistance, recreational activities, and behavioral interventions and management, etc. Able to effectively engage and develop relationships with appropriate boundaries with assigned clients. Effectively manages a flexible community-based schedule. Completes all required documentation according to Agency and program standards within required time constraints. These may include: client contact forms, CANS, service activity summaries, data tracking, daily progress notes for all mental health services provided, mileage documentation, and other administrative duties, etc. Is an active member of assigned treatment teams. Assumes assigned responsibility within the team in accordance to program needs which may include operating effectively within a Teaming Model and act as a liaison with other community agencies and resources. Meets Agency service percentage standard reflecting expected time spent with clients, if applicable. Completes other assigned program operational tasks in accordance with supervisory directives. Minimum Required Education and Experience: Must possess a high school diploma/GED and two years' experience working with children, youth, adults and/or families preferred. Position/Program Requirements: Must possess a reliable mode of transportation to travel to assigned client homes, schools, offices, etc. If personal vehicle is the mode of transportation, must meet Agency's driving requirements. Must be physically and mentally fit in accordance with the Agency's Personnel Policy: Physical Fitness Standards and Examinations.
