Your Job as a Substance Use Counselor:
The Substance Use Counselor is instrumental in our patient's treatment and recovery from opioid use disorder. Substance Use Counselors provide high quality, compassionate care through various mediums which include both individual and group counseling sessions.
Job Responsibilities:
Provide high quality, compassionate guidance in both individual and group counseling sessions.
Plan, oversee, facilitate and document patient's recovery.
Co-facilitate assigned group or family sessions as needed.
Ensure all documentation regarding patient care, treatment, and incidents is completed timely and in a clear, concise manner.
Prepare individual treatment plans for each assigned patient.
Initial assessments as well as follow up assessments.
Evaluate patient needs and determine if referrals to other programs or facilities are needed.
May plan for aftercare for assigned patients.
Provide crisis intervention to patients, as needed.
Provide case management duties for patients, ensuring individualized quality care as needed.
Act as a liaison between referral sources and patients, as needed.

Required Education, Skills and Qualifications:
Minimum of an Associate's Degree in social or health services field required;
Prefer Bachelor's or Master's degree in social or health services field;
Degree must be from an accredited college or university.
Previous experience in addiction recovery or behavioral health settings, such as outpatient, residential, or correctional facilities.
* Familiarity with Medication-Assisted Treatment (MAT), including methadone, buprenorphine, and naltrexone.
* Experience conducting individual and group counseling sessions focused on substance use recovery.
* Knowledge of evidence-based practices, such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and relapse prevention techniques.
* Documentation experience, including maintaining clinical notes, treatment plans, and progress updates in accordance with state and federal guidelines.
* Familiarity with state regulatory standards (e.g., 42 CFR Part 2, HIPAA, Joint Commission standards).
* Experience collaborating in a multidisciplinary team with medical staff, case managers, and peer support specialists.
Licenses/Certifications:
* Required: CADC or tCADC

Clinical Excellence/Patient Care
Provides evaluation, counseling and therapeutic services to chemically dependent patients/families assessing patient's social, psychological, emotional, financial, cultural problems and chemical abuse history.

Qualifications

Therapy

Develop treatment plans in accordance with program requirements, state regulations, etc. and communicates pertinent treatment plan information to others as needed. Updates and revises client treatment goals/objectives on a regular basis consistent with House of Mercy and regulatory expectations.
Establish rapport and develops a therapeutic relationship with assigned clients. Provides counseling services in individual and group sessions. General Requirements:

We are looking to add an Outpatient Counselor to our team in Cedar Rapids at both our Main location and at The Way Home transitional housing. This role will be responsible for conducting comprehensive assessments and facilitating group and individual sessions for patients seeking assistance with substance use disorders. The perfect candidate for this position would have an interest or skill in working with women and women's services.
As the Outpatient Counselor you will:
Conduct comprehensive substance use evaluations for new patients to determine their individual needs and the level of care required.
Collaborate with community partners to conduct evaluations in person and via telehealth for incarcerated patients.
Complete engagement activities and partner with Supervisor to ensure patients exiting incarceration are assigned a regular Counselor upon release.
Lead group counseling sessions for patients; providing education, support, and guidance in a supportive group setting.
Collaborate with the clinical team on patients' individualized treatment plans based on assessment findings, outlining specific goals and objectives.
Maintain timely, accurate and confidential records of patient sessions, including assessments, treatment plans, progress notes, and group attendance records, in compliance with agency policies and regulations.
Provide urinalysis monitoring on an as needed basis.
Requirements
· A minimum of a bachelor's degree in counseling, psychology, social work, or a related field, and a tCADC or CADC is required.
· Knowledge of substance use disorders, addiction treatment modalities, and group counseling techniques.
· Ability to work daily using standard office equipment, computer system, software and to work well with technology.
· Ability to use independent judgment, analytical/decision-making skills, verbal and written communication skills, human relations skills, teamwork skills, and organizational skills.
· This position consists of frequent sitting and/or standing for extended periods of time.
