Job Responsibilities: The APC will ensure the delivery of high-quality clinical and home-based patient care, while promoting optimal health outcomes and providing cost-effective solutions that adhere to Alignment Health?s policies, as well as state and federal regulations.
You?ll play a vital role in assessing, developing, and coordinating care options tailored to the needs of our members, who are typically 65 and older. With home and virtual visits ranging from 45 to 60 minutes, you?ll have the opportunity to provide focused, compassionate care, whether it?s an initial visit, an annual check-up, or when additional support is needed to address medical and social challenges.
Willingness and ability to do home visits in the field 3-4 days per week (depending on the program), with remaining days being remote from home is required

Qualifications:
2 year previous experience as a Physician Assistant preferably in cardiology.
Familiarity with medical tools and equipment.
Ability to build rapport with patients.

Excellent written and verbal communication skills.
Physician Assistant Must maintain board certification as required for licensure.

The Physician Assistant functions to assist the physician in the delivery of medical care; obtains historical and physical data on patients; interprets and integrates findings to determine appropriate diagnostic and therapeutic procedure under the supervision and guidance of the physician. The Physician Assistant also assists in management of treatment plans, providing technical assistance and assuring quality patient care. The Physician Assistant must maintain positive relationships with patients, family members, visitors and fellow employees.
PREFERRED QUALIFICATIONS:
This position requires formal training which will probably be indicated by graduation from an accredited program of physician assistant studies. The PA must possess a Physician Assistant certification and current KY board licensure.

Physical guidelines include the ability to move, traverse, position self, remain in a stationary position and negotiate steps for up to eight hours per day; lift, move and transport patients, equipment, supplies; motor coordination and manual dexterity; ability to communicate effectively; ability to read/understand orders; visual acuity.
