The successful candidate will be responsible for providing high-quality mental health services, psychosocial support, and guidance to patients transitioning from hospital care to skilled care facilities settings or long-term care facilities. Stay informed of the latest research and best practices in social work and post-acute care, integrating new findings into practice to enhance patient outcomes. Qualifications: Master's degree in Social Work (MSW) from an accredited institution. Valid LCSW license in the state of Colorado. A minimum of 2 years' experience in a clinical setting, with preference given to experience in acute or post-acute care. Demonstrated ability to conduct thorough psychosocial assessments and develop effective care plans. Excellent counseling and crisis intervention skills. Strong advocacy and negotiation skills, with the ability to coordinate resources and services for patients. Effective communication and interpersonal skills, with the capability to work collaboratively within an interdisciplinary team. Knowledge of healthcare systems, Medicare, Medicaid, and other social services programs. Compassionate and patient-centered approach to care. Ability to maintain confidentiality and adhere to ethical guidelines and standards of practice. Working Conditions: This position requires flexibility in hours and locations, including the requirement to travel within the Southern Region-Specifically Colorado Springs, Pueblo, and Canon City. Is sensitive to the patients' needs throughout the procedure. Performs radiologic exams that produce quality images and ensures patient and staff safety. Thoroughly and accurately completes all required documentation, including time stamps and image storage. Basic Life Support certification (BLS) for healthcare providers. Competency in at least one of the following areas: Chest, Extremities, Spine, Skull, Podiatry, Bone Densitometry (DEXA). Preferred Qualifications One year of experience in an ambulatory outpatient clinic environment Physical Requirements: Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies. Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations. Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc. Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items. Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment.
