JOB OVERVIEW
The Wellness Nurse is responsible for monitoring the health and well-being of our residents and provides leadership support and assistance with resident care and services. Responsibilities include but are not limited to overall well-being of our residents, assisting in training and monitoring of medication care administration, communication with resident support services and families and maintaining clinical quality assurance while leading and demonstrating the Mission for Sunrise Senior Living, "to champion the quality of life for all seniors" in accordance with federal, state/provincial, and local standards and Sunrise Senior Living policies to promote the highest degree of quality care and services to our residents.
RESPONSIBILITIES & QUALIFICATIONS
Essential Duties

Coordination of Health Needs:
Identify on-going needs and services of residents in promotion of the highest quality resident services to be delivered.
Ensure proper follow through, assessment, and documentation for residents with a change in health care services and status.
Assist Resident Care Director (RCD) in conducting resident assessments at time of move in, 30 days post move in, semi-annually and with any change in status.
Conduct monthly wellness visits for all residents.
Assess, monitor, and follow up with residents who have experienced any fall, accident, or incident and document and report as required and directed.
Document all pertinent information in the resident wellness file.
Maintain communications with resident's family and or responsible party to ensure medical needs for the resident are being met.
Contact resident's attending physician when necessary and/or upon family request.
Ensure weights and vital signs are obtained monthly for each resident.
Assist in maintaining wellness files according to Sunrise policies and federal and state/provincial regulations.
Maintain medical supplies and emergency kits for the community.
Provide clinical support and assistance to community team members as needed.
Understand and follow infection control practices.
Partner with the RCD and community team to promote infection control standards including any resident testing, training, and skills checks.
Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials in compliance with universal care precautions.
Medication Programs
Monitor each resident's medication and treatment profile regularly to ensure each medication and treatment is administered as ordered and documented accurately.
Transcribe physician orders Medication Administration Record Treatment Administration Record (MAR/TAR) to include initials, date and time 'noted' and, date faxed to pharmacy accurately and promptly.
Complete MAR/TAR audit each month.
Demonstrate and is knowledgeable of the Six (6) Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
Act as liaison for pharmacy services to ensure effective services for residents.
Monitor the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
Complete Medication Observations and skills checks for medication care managers at the direction of the RCD.
Quality Assurance and Regulatory Compliance
Demonstrate and is knowledgeable in the following key quality improvement areas:

Resident Assessment/Evaluation process in accordance with state/provincial regulatory requirements.
Review Physician Reports of all new residents as directed by RCD.
Order medications and equipment as directed by RCD.
Demonstrate and is knowledgeable of Sunrise practices and guidelines in accordance with resident care programs to include skin, nutrition, behavior, falls, and bowel/bladder.
Assist in implementation, training, and monitoring of documentation related to resident's clinical health and wellness.
Ensure resident changes, concerns, and/or solutions are reported to RCD as appropriate.
Contribute to the overall engagement programs and processes (customer and team member engagement) including participating in the team member engagement survey and engagement improvement planning workshops.
Attend regular meetings; Town Hall, Department Team, Cross Over, and others as directed by the Supervisor/Department Coordinator.
Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.
Perform other duties as assigned.

Graduate of approved college/school of nursing
Maintains a current state/provincial license as a professional Registered Nurse (RN)
Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment
Demonstrates knowledge of nursing practices, techniques and methods applied to health and wellness resident care, and federal, state/provincial, and local regulations
Demonstrates knowledge of good assessment skills
Demonstrates proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) and Sunrise applications with the ability to learn new applications
Ability to work weekends, evenings, and flexible hours to be available to our customers at peak service delivery days and times

Assist the Sales and Marketing team in maintaining a thorough working proficiency in the customer relationship management (CRM) lead tracking system.
Support the Sales and Marketing team by effectively maintaining all current prospect records in the CRM lead tracking system.
Support the Executive Director (ED), DSM, and DOS in building a trusting and positive relationship with prospects by understanding the Sunrise product and how Sunrise can meet the customers' needs.
Assist the ED, DSM, and DOS by ensuring model suite(s)/apartment(s) are well maintained and presentable.
Support the ED, DSM, and DOS in setting, tracking, and accomplishing sales goals in a timely manner.
Demonstrate effective telephone skills producing qualified leads and appointments.
Prepare the resident's Administrative File according to Sunrise and state/province specific regulatory requirements where applicable.
Manage the move-in process as outlined in the Resident Move-In Checklist.
Meet with the resident and/or family on the day the Move-In Fee is placed and review the Move-In Packet ensuring all information is explained.
Coordinate and facilitate the Resident Assessment with the Resident Care Director (RCD) and Assisted Living Coordinator (ALC)/Reminiscence Coordinator (RC).
Ensure all Sunrise and required state/province forms are completed by the resident and/or family on or before move-in date.
Schedule the Resident Agreement Signing on Move-In day, collect any remaining paperwork, and review the welcome packet with the resident and family member(s).
Orientate the resident and family to the community, present the room to the resident and family, and explain the telephone and emergency procedures systems and other services (hair care/salon, transportation, activities, etc.).
Arrange lunch or dinner for the resident and family members on Move-In day.
Ensure the suite/apartment is ready for Move-In using the Suite Readiness Checklist.
Communicate any necessary suite/apartment readiness needs with the Maintenance Coordinator.
Maintain an open and ongoing dialogue with the resident and/or family from the time the Move-In Fee is placed through Move-In to ensure a smooth transition for the resident and to identify resident/family needs.
Initiate communication with resident's physician and/or family to ensure completion and delivery of the Physician's Statement and any state-specific forms.
Assist the resident and family members in making the emotional connection to the new community through awareness, integration, and socialization techniques and engaging activities.
Maintain and protect the confidentiality of resident information at all times.
Facilitate the Sunrise Founders Club ensuring all Founders Club benefits are received by the resident and/or family as applicable for new communities.
Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety, promoting Risk Management programs and policies, and adhering to safety rules and regulations.

Provide supervision to the assisted living/long term care team and support and assist the Assisted Living Coordinator and/or Resident Care Coordinator.
Assist with scheduling and assigning daily tasks to the assisted living/long term care team.
Create schedule in the absence of coordinator to be reviewed by another coordinator.
Assist with staffing and scheduling according to operational and budgetary guidelines and federal, state/provincial, and local laws.
Assist in daily review of timekeeping practices and payroll reports to ensure all hours worked are recorded and appropriately paid.
Take an active role in Crossover meetings to remain informed; brief incoming assisted living/long term care team regarding resident updates and community news and concerns.
Maintain visibility throughout the assisted living/long term care neighborhoods and take initiative to assist team members.
Resident Services
Review clinical alerts to document and learn about pertinent information and any resident's physical and behavioral changes.
Recognize resident changes in condition and take appropriate action.
Communicate regularly with families as appropriate.
Assist with monthly and quarterly in-house social and family events.
Assist in maintaining a robust public relations program in support of the assisted living/long term care neighborhood(s) and community operations.
Assist with the inventorying, tracking, and ordering of necessary resident care supplies.
Resident Care and Programs
Supervise and assist the assisted living/long term care team as well as provide direct resident care and services.
Support the leadership activities of planning, organizing, facilitating, and assisting in the overall assisted living/long term care operations and/or directly supervise assigned assisted living/long term care neighborhoods as directed by supervisor in accordance with federal, state/provincial, local laws and Sunrise quality service standards.
Support the all-embracing quality delivery of the Resident Centered model.
Assist in developing resident Individualized Service Plans (ISP).
Assist with developing the Daily Assignment Sheets and monitor daily tasks progress and flow.
Review the resident's file prior to move-in and assist if appropriate in care plan meetings, reviewing the Resident Lifestyle Story and ISP according to Sunrise quality service standards.
Assist with adding tasks to the electronic health record for point of care documentation.
Assist the Resident Care Director (RCD) with the assessment process to support the levels of care with the appropriate staffing levels.
Partner with the Activities and Volunteer Coordinator to ensure a variety of appropriate and individualized activities are available through the day and evening and care managers are actively involved.
Monitor and assist in the dining experience daily.
Quality Assurance and Regulatory Compliance
Assist in ensuring the community team follows all federal, state/provincial, and local laws and regulations and Sunrise quality service standards as it pertains to resident care and services.
Demonstrate proficiency and understand the Quality Service Review (QSR) and essential quality service standards pertaining to resident care and services; partner with community team in striving for excellent quality care and service delivery.
Assist in the development and implementation of appropriate plans of action to correct identified deficiencies in compliance with Sunrise expectations and other regulatory compliance audits.
Assist in ensuring standards of cleanliness, hygiene, proper attire, and health standards are maintained at all times by the assisted living/long term care team.
Risk Management and General Safety
Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.
Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, SDS Safety Data Sheets), and Lockout Tagout procedures.
