Full-time Description Provides in-home, community-based and/or telehealth treatment services to a caseload of individuals aged 55 and over who have a mental health diagnosis. Essential Duties and Responsibilities: Responsible for providing clinically appropriate psychoeducation, advocacy and coordination of services to assigned clients. Engages clients in the development of annual treatment goals, clinical reviews and completion of all other documentation related to the client's treatment. Maintains communication with clients, caregivers, and helping professionals/agencies serving the client through phone calls, written communications, and in-person meetings. Provides necessary direct service hours to meet the performance standards set by the Behavioral Health Services Department.
