Under the direct supervision of the Family Advocate II and the indirect supervision of the Site Manager, the Family Advocate will provide direct participant services to all participants in the Shingle Springs Tribal TANF Program (SSTTP), including eligibility determination, continuous case management and helping participants gain access to all services allowable under Federal and State regulations, supporting employment and becoming self-sufficient. The Family Advocate applies the principles, knowledge, and practice of professional social work to provide culturally relevant social diagnosis, treatment, research, consultation, and preventive social service programs for individuals and families.
ESSENTIAL FUNCTIONS
Uses social case work, therapy techniques, administration, community organization, consultation, teaching, research, planning, health education and welfare services to assist eligible participants.
Conducts and participates in research appropriate to professional training and expertise.
Develops and maintains an active information program designed to apprise the community of their social need and the resources available to assist with these problems.
Understands and is proficient in Federal Tribal TANF program regulations, the SSTTP Plan and the SSTTP Policies and Procedures.
Conducts initial participant intake interviews, conducts annual recertifications, and updates case information as changes occur for eligibility requirements. Maintains communication with participants regarding determination status.
Determines case family eligibility, including gathering required verification pertaining to income, resources, expenses, etc.
Facilitates orientation for new participants.
Calculates and processes participants monthly grants and supportive services.
Develops and updates individual participants Wellness Plan to assess and evaluate employability of participants, taking into consideration work history, skills, strengths, education and training, interests, and current barriers.
Develops and updates individual participants Compliance Plan to avoid sanctions, supportive service denials, and case closure.
Completes Fraud Report when fraud is suspected and submits report to Site Manager and Quality Assurance Officer.
Case staffing for the following: Employment, Education, Wellness and Compliance Plans.
Maintains required documentation of participants services and daily program activities by entering case management data into the software system and maintaining both electronic & hardcopy files.
Cross trains with other Tribal TANF team positions.
Communicates regularly with participants through in-person meetings, telephone, email and standard mail.
Maintains ongoing case family assessments, employment plans, job and resource referrals, progress monitoring, supportive services requirements, problem solving, job or other work activity monitoring and support/encouragement.
Assists participants in accessing resources.
Participates in case consultations with participants and other organizations to determine the best needs and services for the participants.
Determines when assessment tools are needed; administers or recommends appropriate assessments and in-house referrals.
May facilitate workshops or classes for participants and or general public.
Provides information to participants, community organizations and the general public about the SSTTP.
Conducts periodic home visits to determine needs of the participants.
Provides various support to other TANF departments as needed.
May be required to adjust schedule to support TANF events.
Performs other duties as assigned.
MINIMUM QUALIFICATIONS
AA Degree in Social Services and/or two (2) years of relevant case management experience.
Four (4) years case management experience in Tribal TANF Program can suffice for Education requirement.
Must be proficient in the use of Microsoft Word, Excel and Outlook

Must be able to work evenings, weekends and occasionally on short notice in response to crisis situations.
Ability to work independently with minimal supervision.
Ability to organize work and time; establish priorities; work under pressure and meet deadlines.

About the Position:
The Peer/Family Advocate will serve as a member of a multi-disciplinary team to provide peer and/or family support in collaboration with the Service Team. Their focus will be to assist with accessing program services and other community resources to support wellbeing based off their prior personal or familiar experiences. The Peer/Family Advocate will function as a role model exhibiting competency in personal recovery and use of coping skills as well as assisting in helping clients learn coping skills and develop wellness plans congruent with cultural norms and expectations. The Peer/Family Advocate will serve as an advocate for client choice and voice.
Essential Duties & Responsibilities:
Support clients and/or family members with active listening, demonstration of empathy and carry a message of hope and recovery and empower clients in meeting their identified goals.
Collaborate with Service Team members in coordinating and providing services for clients.
Observe, assess, intervene and refer (within one’s scope of practice) in situations with clients regarding mental health symptoms, physical health symptoms and substance use issues.
Assist clients in developing and following a wellness plan unique to their needs and desires.
Assist member with various independent living skills: house cleaning, laundry, food preparation, hygiene, yard work, budgeting money, shopping, medication management and safety.
Using personal experiences and interactions with mental health systems and other community partners, to guide and support client in advocacy; as well as provide hope.
Provide/Assist the clients with transportation in personal vehicle as necessary.
Meet with clients and/or family members in the community and their homes, and attend meetings to ensure continuity of care for clients.
Provide positive public relations, community referrals, information and support by linking clients to needed resources; advocate with and on behalf of clients and significant support persons the clients have identified.
Facilitate planning of social, cultural, educational, supportive and recreational group activities.
Accurately complete all necessary forms, reports and paperwork required by the program on a timely basis (e.g., progress notes, special incident reports, discharge summaries, Medi-Cal billing, EHR forms).
Obtain Peer Certification within 6 months of employment
Act as a designated Mandated Reporter for the observed or suspected abuse and/or neglect of children, disabled individuals, dependent adults and elder/senior citizens. Mandated Reporter also includes “Duty to Warn” which includes the responsibility to notify authorities and the intended victim(s) of physical harm.
Regular and predictable attendance required.
Use motivational interviewing, trauma-informed care, and harm-reduction practices.
Qualifications:
Lived experience as a mental health consumer or a family member of a mental health consumer or as someone who has experienced homelessness.
Ability and willingness to work with people experiencing homelessness where they are in encampments, on the street, or in shelters.
Work experience with persons who have substance abuse problems, preferably individuals who are dually diagnosed (mental health & substance abuse) is highly desirable.
Knowledge of 12-Step Programs and/or personal experience with substance abuse recovery is highly desirable.
Demonstrated knowledge of General Assistance, Food Stamps, Temporary Aid to Needy Families, Unemployment Insurance, and SSI/SSDI regulations
High School Diploma or GED required
Certificates, Licenses, Registrations:
Possess a valid California Driver’s License and current vehicle insurance. Driving records must meet, and continue to meet, the underwriting standards established by the generalized driving guidelines used by our insurance broker. Must provide proof of insurance.

You will evaluate donor reaction(s), which occurs at the facility as outlined in the SOPs. Follow applicable SOPs for medical emergencies including the development of Center Physician's standing orders and donor transport to emergency care facilities.
You will refer to the Center Physician or Medical Affairs (as applicable) when in need of providing unacceptable findings to donors, or guidance concerning medical or technical issues, including donor safety and eligibility.
You will support the Hepatitis B and Seasonal Flu vaccination programs for employees as applicable.
You will manage employee incidents and determine whether further evaluation is required by occupational health/ER. Refer to EHS guidance regarding employee incidents.
You can be a Pandemic Coordinator when authorized by EHS and support investigations associated with pandemic threats within the local community as indicated by EHS or Medical Affairs.

In addition to the Typical Duties noted above, duties for the below noted areas of assignment include (the below list does not represent all areas of assignment):
Athletics:Review academic performance of student athletes and make appropriate referrals; assist in follow-up of unsatisfactory reports, and complete eligibility unit checks.
Call Center:Intake inquiries and refer students and public to appropriate resources; assist with coordination and support of call campaigns for recruitment, retention and completion efforts; guide students to complete application process, academic program selection, additional enrollment processes, financial aid application, and college visits.
Career Center/Internships:Research current labor market for career and transfer opportunities, with emphasis on college program areas; refer qualified students to employers or employment services for placement and conduct follow-up interviews with students; provide assistance with mock interviews, resume critiques, and job searches; administer career planning assessments; provide vocational and educational guidance training for students.
Student Life:Assist with the development and supervision of student organizations in collaboration with student leaders; plan and coordinate social and cultural activities for students; attend and assist with coordination of college activities sponsored by the student association and other student groups; coordinate the student election process.
Minimum Qualifications
EXPERIENCE:Two years of experience related to the duties of the position.
EDUCATION:Completion of an Associate's Degree or 60 semester units (90 quarter units). NOTE: An additional year of qualifying experience may be substituted for up to one year of college education.

(Experience Requirement: One year of experience is equal to 12 months of experience at 40 hours per week. Applicable part-time experience will be converted to the full-time equivalent for purposes of meeting the experience requirement.Education Requirement:One year of education is equal to 30 semester units.)
Education must be from an accredited institution.
