Supportive Concepts for Families, Inc, an Affiliate of Apis Services, is immediately looking for a PART-TIME dedicated and hard-working Health Services Coordinator/Licensed Practical Nurse (LPN) to join our medical services team in our Berks county & surrounding areas. In this role, you will provide comprehensive nursing care and perform duties under supervision of the Director of Medical Services to individuals in our Behavioral Health and IDD Residential Program. This role requires traveling from CLA's to CLA's along with providing training to the CLA Staff in the Northeast/Central region.

Education & Experience Requirements:
Active LPN license in the state of Pennsylvania.
Minimum two (2) years of nursing experience in a residential, community, or similar setting.
Strong patient assessment skills.

Knowledge of Intellectual Developmental Disabilities (IDD)& Behavioral Health.

Work within the scope of Pennsylvania Nurse Practice Act for LPN's in providing for all operation.
Follow standardized system to monitor regulatory guidelines and guidance to support consumers for the following areas:
Medication.

Complex Health Issues.
Specific tasks to be performed include, but are not limited to:
Review medication logs as needed and consult with Program Coordinator/On Call person for medication issues, errors, refusals in all IDD community living arrangements
Assist support staff in preparing for doctor appointments as needed
Assist with complex medical appointments in preparing support staff and/or attending appointments
Develop approaches to educate consumers about health care procedures, diagnoses, and treatments
Educate support staff in consumer health care procedures, diagnoses, and treatments
Offer input for development of medical/psychiatric policies and procedures
Participate in preparation and revision of medical protocols
Work to promote and provide staff education and understanding of psychiatric and medical health care policies and procedures
Documentation as needed
Participate in the coordination of service delivery to new and current consumers
Site visits to consumers as scheduled and as needed
Serve as a liaison with PCP/Specialists around healthcare needs of consumers
Provide direct services to consumers such as administration of injections, wound care, etc. as needed
Participate as assigned by the Vice President of Health Services in medical emergencies
Act as a point person in communicating with hospital health care professionals, SCFF supervisors, and support staff
Participate in consumer discharge meetings
Provide health training as required following discharge
Act as a resource for the Positive Approaches Resource Team (PART) to offer medical knowledge when needed
Special projects as assigned by the Vice President of Health Services

The Medical Coordinator (MC) conducts medical appointments to ensure that the individual receives all the treatments as ordered by medical professionals. Ensure that consumer body weights are taken weekly, on Sunday, and ensure data is maintained.
Ensure that each consumer have all the annual, semi-annual, and follow up requests appointments, labs, PPD, vaccines, EKG, etc.
Check the treatment, referral and recommendations on the Medical Visit Report (MVR) and report summary to respective team.
Ensure all Medical Visit Reports are completed by the physician or PA, including signature.
Maintain organized and well-kept medical records and medication administration records in accordance with agency policy and regulation.
Accompany individuals to medical appointments, as necessary. Ensuring that all relevant questions are asked to ensure that there is a clear understanding by all.
Schedule medical appointments with all physicians to ensure adequate follow-up as ordered by physicians.
Ensure that all medications are ordered timely and as ordered by the physician.
Effectively communicate with staff and management to ensure that all physician's orders occur. This includes, but is not limited to: in-home treatment, medication, follow-up tests and appointments.
Maintain proper filing in medical records, consultation review records and medication administration records.
Maintain effective conversation with physicians, doctors' offices, support coordinators, and pharmacists to ensure that all medications are ordered and received accordingly.
Must advocate for the best medical services for the individual by health care personnel.
Promote the individual to be involved during medical assessments.
Should understand and should be able to communicate and transcribe the physicians' instructions.
Be the liaison between the IDT team and medical offices.
Maintain HRST evaluation ratings for each individual.
Ensure the MAR is in compliance with ODP-approved medication curriculum.
Must be medication trained by the ODP-approved medication curriculum.
Ensure that all medications have prescriptions on file and available for staff review.
Ensure all MARs are reviewed by the 7

of every month, reconciled with the pharmacy web connect and ready to be sent to the pharmacy for monthly MAR delivery.
Ensure all medication administration equipment is operational (blood pressure machines, thermometers, etc.)
Maintain Medication Trainer certificate to provide staff with training and observations.

Attend all ISP meetings, complete annual outcomes/procedures and all needed paperwork and submit to the Supports Coordinator
Ensure the resident's home is a safe, functional living space to meet individual needs
Participate in medical appointments (especially psychiatric or hospital discharge) as needed to ensure additional services are requested if needed and that information is relayed appropriately
Participate in on-call rotation, as assigned
Create, monitor, and update house books for all individuals on caseload
Coordinate and participate in all evaluations including, PT, OT, speech, sensory, and any other required support
Work in conjunction with Referral Team on coordination with pending placements, as well as with the families and present provider of these individuals
Coordinate and participate in all evaluations concerning wheelchairs, medical and adaptive equipment
