Summary

The Certified Diabetes Care and Education Specialist serves as a clinical expert and provides education and consultation services to internal and external groups. Collaborates with other professional staff to facilitate care across the continuum.
The Certified Diabetes Care and Education Specialist will have different areas of responsibility depending on the environment of which they primarily perform their job duties:
Inpatient education staff will be responsible for the provision of diabetes education primarily in the inpatient setting.
Outpatient diabetes education staff will be responsible for the delivery of diabetes education primarily in the outpatient setting. Individual and group education delivered either in-person or via telehealth will be required. Outpatient educator will also be responsible for facilitating Diabetes Prevention Program (DPP) sessions.
Certified Diabetes Care and Education Specialists stationed within the adult Endocrinology offices will primarily educate patients within the practice settings. Registered dietitian credential is required for these roles.
Regardless of the primary location of the role, the Certified Diabetes Care and Education Specialist may be requested to assist at other locations depending on department needs.

Education: Associate degree required in health-related profession.

The Transport RRT willprovide safe, timely, high-quality care during the transport of all neonatal and pediatric patients, and patients determined by the transport medical directors needing transported by the team.Perform all duties stated in Registered Respiratory Therapist-Critical Care job description listed above. Provide family-centered care to infants from birth to pediatric patients during inter-hospital transports. Initiate emergency protocols independently following ACLS, PALS, NRP, and STABLE guidelines. Communicates effectively with team members. This position functions as an expert resource for direct care throughout the system and supports all staffing levels. Responsibility for the delivery of Respiratory Care to all patients, to include all therapeutic treatments, oxygen therapy, arterial blood gas draws and analysis, ventilator management, airway management, and high risk deliveries and to include intubation with proper certification.
Requirements
Education: Associate degree required. Bachelors required to perform duties of a Technical Consultant.
Experience:Three years critical care in Pediatric Intensive Care Unit and/or Neonatal Critical Care Unit, or three years Pediatric Emergency Department experience, 2000 hours in a level 2-3 NICU. Critical care transport experience preferred.
Certification:BLS required. Credentialed by the National Board for Respiratory Care as an ACCS (Adult Critical Care Specialist) or NPS (Neonatal/Pediatric Specialist) as required by specialty. ACLS, PALS, NRP and STABLE required as appropriate to population served and/or specialty. The Music Therapist provides direct services to hospice and palliative care patients and their families in homes and inpatient and outpatient units. ESSENTIAL RESPONSIBILITIES
Assesses hospice and palliative care patients of all ages to identify needs and provide interventions that meet individual needs. Collects data in collaboration with the patient and/family as demonstrated by the documentation on the patient record.
Performs the initial assessment of needs during first visit and documents those needs within appropriate time frame. Assesses the emotional well-being, physical health, social function, communication abilities and cognitive skills through musical responses. Communicates findings and problems to appropriate team members.
Plans and implements care for patients of all ages based on needs assessed to ensure individualized care. Identifies and documents problems, and sets developmentally appropriate goals, time frames, and interventions.
Provides and documents direct patient care according to the Care Plan, within set target dates, and according to all departmental policies. Designs music sessions for individuals and groups based on client needs. Provides basic and specialized teaching as required in the Plan of Care and documents the response of patients and/or family in the patient's record.
Regularly evaluates patient's progress, takes prompt action when any change in the patient's condition is noted or reported, and terminates care when goals of management are attained for patients of all ages. Participates in treatment planning, ongoing evaluation and follow-up. Re-evaluates and documents the patient's response to music therapy. Based on patient response does appropriate modifications to the Care Plan in a timely manner. Recognizes changes in the patient's physical and mental status and informs health care professionals within a reasonable timeframe.
Identifies own learning needs and seeks direction/approval from supervisor to meet those needs as documented on the goal section of the performance appraisal.
Educates staff about the role of the Music Therapist and the use of music therapy interventions with hospice patients and families.
Assesses and monitors program development and evaluation of music therapy services.
Attends all mandatory Hospital initiated training
Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.
Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements.
