Lead, facilitate, and advise the Center Quality Council and internal performance improvement teams:
* Set the agenda and maintain meeting minutes
* Ensure reporting of all mandatory and center specific monthly and quarterly reports for trends/areas for improvement to the Quality Council and Medical Executive Committee/Governing Body a minimum of quarterly:
* Medical Record Audit reports; Monthly or quarterly data collection from ongoing systematic chart review to assess quality of documentation.
* Infection Control reports
* Hospital Transfer/Complication reports
* Patient Safety; measurement of key measures of patient safety and hazard analysis/process redesign (adverse events, root cause analysis).
* Life safety (environment of care); Provide for a detailed assessment and evaluation of the Environment of Care (EOC) and the associated conditions, staff education and readiness and the various processes. Framework for the EOC includes the management processes and systems that affect safety, security, hazardous materials, emergency preparedness, life safety, medical equipment, and utilities management.
* Risk Management (incident reporting)
* Adverse Drug Reaction reports
* Cancellation logs
* Service Satisfaction reports (patients, staff and physicians)
* Center specific quality indicator reports as appropriate

Provides strategic oversight of proactive and reactive patient safety activities:

* Sentinel Event Alerts
* Identification and data collection of center specific quality indicators based on high risk, problem prone procedures as appropriate.
* Review and revision of the PI Plan on an annual basis and preparation of the annual report of the PI program to the Medical Executive Committee/Governing Body.
* Documentation of all Performance Improvement activities and maintenance of records for a minimum of three years.
Provides strategic oversight of proactive and reactive patient safety activities (continued):
* Coordination of the center policies/procedures and processes to be in compliance with the current standards of applicable regulatory and accrediting agencies, and mandatory SCA Corporate policies.
* Working with the Administrator/designee to ensure currency of all physician files, medical staff appointments and/or privileges and compliance with credentialing policies and procedures. Coordinating as appropriate the peer review process and aggregate individual peer review data for presentation and review by the Medical Executive Committee and Governing Body at reappointment.
* Working with the Administrator/designee to ensure currency and completeness of all human resource and education files for center employees and contract personnel.
Maintain Center Survey readiness:
* Assess center compliance with accreditation standards and regulations in collaboration with leadership and staff.
* Identify areas of vulnerability and direct the development of strategies to enhance compliance.
* Provide the overall direction necessary to ensure that clinical services provided are evidence-based, in accordance with standards established through state and federal regulations and applicable accreditation standards, including the National Patient Safety Goals.
Communicate Effectively Throughout All Levels of the Organization:
* Proactively educate and train the leadership and staff regarding regulatory issues, new statutes/guidelines, and safety/quality/performance improvement activities and their respective responsibilities in carrying out the performance improvement program.
* Maintain effective communication on current center activities related to Safety/Quality/PI and Accreditation and seek consultation as needed for support from the Regional Quality Coordinator or assigned Group Director.

Summary

The Certified Diabetes Care and Education Specialist will have different areas of responsibility depending on the environment of which they primarily perform their job duties:
Inpatient education staff will be responsible for the provision of diabetes education primarily in the inpatient setting.
Outpatient diabetes education staff will be responsible for the delivery of diabetes education primarily in the outpatient setting. Individual and group education delivered either in-person or via telehealth will be required. Outpatient educator will also be responsible for facilitating Diabetes Prevention Program (DPP) sessions.
Certified Diabetes Care and Education Specialists stationed within the adult Endocrinology offices will primarily educate patients within the practice settings. Registered dietitian credential is required for these roles.
Regardless of the primary location of the role, the Certified Diabetes Care and Education Specialist may be requested to assist at other locations depending on department needs.

Under the direction of the Environmental Standards director, this position works independently collecting water samples and monitoring water quality parameters, performing sampling to assess employee exposures, measuring indoor air quality, and other environmental sampling. Conducts air and contact sampling for microbial contaminants in cleanrooms, water fixtures and ventilation systems. Audits construction projects for compliance with infection prevention policy and conformance with infection control risk mitigation measures. Assists with the training of construction and renovation contractors. Assists with audits for environmental infection prevention. Performs ventilation measurements to certify laboratory hoods, verify pressure relationships and airflow, and inspects HEPA-filters. Works in clean rooms, soiled workrooms, construction sites, plant mechanical spaces, patient rooms, and out of doors. Work may require wearing scrubs, cleanroom garb, hardhat, protective eyewear, and other personal protective equipment.

About this Position:
HealthPark Medical Center is a 326-bed Acute Care Facility that provides specialized cardiovascular care, comprehensive womens and childrens services, and advanced rehabilitation programs. As a Respiratory Therapy Supervisor, you will assist the director in supervising personnel and overseeing the daily operations of the Respiratory department. Helps to recommend and implement policies and procedures. Understands and follows human resources and health system policies and procedures. Assists the director in hiring, orienting, developing, coaching, counseling and termination of employees. Coordinates equipment maintenance needs, performance improvement, and quality control activities specific to the department. Performs assessment of staff blood gas competencies if qualified as a Key Operator. Measures ongoing performance improvement efforts. Manages supplies and payroll to operate within the department budget. Participates in the interview and decision process for new hires. May also participate in delegating daily assignments and participates on various committees.
Observes activities of staff and provides input for competency evaluations and performs annual evaluations for direct staff. Helps to mentor lead Respiratory Therapists in their development. As appropriate and when needed, performs in the capacity of a Lead or bedside Respiratory Therapist. May represent the department and/or director on campus committees and meetings as designated. Educational Requirements:Bachelors Degree required.

Neuro Interventional Radiology:
The NIR Technologist must be competent as the assistant to the Interventional Neurologist for diagnostic and interventional procedures in roles that include scrubbing, monitoring, and circulating. The technologist must have knowledge of the X-ray equipment in use, hemodynamic monitoring, cerebral and peripheral anatomy, and sterile technique.
Shifts for this position begin at 7a with a shift of (4)10hr or (5) 8hr work schedule. No weekend rotation, only call. Call is 1-2 nights/week & 2 out of 6 call weekends per 6-week schedule.
Responsibilities And Duties:

Cath Lab Specific: (Level 3 Lab- Ohio Admin Code)
The Senior Cath Lab Radiologic Technologist must be competent as the assistant to the Interventional Cardiologist for coronary, peripheral, and structural heart procedures. The Senior Technologist must meet all requirements of the procedural job code: knowledge of the X-ray equipment in use, hemodynamic monitoring, coronary and peripheral anatomy, sterile technique and knowledgeable of all inventory items and equipment in the department.
Requirements:
1. The Senior Cath Lab Radiologic Technologist must be an active, effective preceptor for new associates and students.
2. Competent to scrub and circulate all coronary cases including complex PCI and brachytherapy; Ventricular assist devices (IABP, IMPELLA and ECMO); all peripheral vascular cases including venous procedures (IVC reconstruction) and all Structural Heart Procedures including aortic and mitral valves and ASD/PFO. Must rotate through all labs including the HVL at least twice a month.
3. Competent to float to Arrythmia Services and Hybrid Lab to assist with procedures.
4. Must be FT or PT rostered FTE
5. 1-3 years' experience in hospital CVL department
Arrhythmia (EP) Specific:
The Senior EP Radiologic Technologist must be competent as the assistant to the Electrophysiologist for non-invasive, invasive, and structural heart procedures. The Senior Technologist must meet all requirements of the procedural job code: knowledge of the X-ray equipment in use, arrhythmia monitoring, cardio structures, anatomy of the heart chambers, sterile technique, and be knowledgeable of all inventory items and equipment in the department.
Requirements:
1. The Senior EP Lab Radiologic Technologist must be an active, effective preceptor for new associates and students.
2. Competent to scrub and circulate all complex ablations including afib and VT, laser lead extractions, and Structural Heart Procedures.
3. Competent to float to Hybrid Lab to assist with lead extraction procedures outside of our unit.
4. Must be FT or PT rostered FTE
5. 1-3 years' experience in hospital EP department
Vascular Interventional (VIR) Specific:
The Senior VIR Radiologic Technologist must be competent as the assistant to the Interventional Radiologist for minimally invasive, image-guided vascular procedures. The Senior Technologist must meet all requirements of the procedural job code: knowledge of the imaging equipment in use, vascular anatomy, sterile technique, and be knowledgeable of all inventory items and equipment in the department.
Requirements:
1. Competent to scrub and circulate all special angiographic procedures including but not limited to: Y-90, (Trans jugular Intrahepatic Portosystemic Shunt) TIPSS, Chemoembolization, Uterine Artery embolization (UAE) and GI Bleeds
2. Must be FT or PT rostered FTE
3. 1-3 years in hospital IR department
Neuro Interventional Specific:
The Senior Neuro Interventional Radiologic Technologist must be competent as the assistant to the Neuro Interventional Radiologist for diagnostic and interventional procedures.
