Provides direct psychosocial services to mentally ill patients in the community who are experiencing psychosocial difficulties. Provides linkage, coordination with, referral to and follow-up with appropriate ongoing service providers. Provides information and consultation to other community agencies and other disciplines, including other services of the Agency. Assists in the overall functioning of the Program. Works under general supervision.
• Responsible for screening telephone referrals. • Performs psychosocial evaluation and assessment of mental health service needs of identified patients and their families through professional knowledge, skills of observation and interviewing. • Develops and implements short-term service plans for patients, in conjunction with other members of the Program. • Provides counseling, care management and appropriate referrals for long term and supplemental treatment. • Prepares case histories and prepares and maintains case records, in accordance with the Program record-keeping mechanism. • Encourages resistant clients to accept mental health services through interventions with clients and/or family members and friends concerned with the client's welfare. • Participates in interdisciplinary team meetings and case conferences of the Program. • Provides referral and provision of information to appropriate long-term mental health services and social services and social services providers, or long-term residential facilities. • Coordinates and follows up on linkages made between clients and other service agencies and mental health providers to ensure continuity of care. • Liaison with, and consultation to, community agencies. • Provides outreach services as part of the Program to mentally ill individuals in the community who are experiencing, or are at risk of, psychosocial difficulties and require mental health intervention in their home. • Serves as resource person to the program and other components of the Agency, when requested, pertaining to psychiatric nursing. • Participates with the team and other appropriate Agency staff in the development and implementation of an in-service training component. • Assists and collaborates with the Program Coordinator in the overall functioning of the service. • Assumes Program Coordinator's functions in his/her absence, as requested. • Participates in community programs and education, as requested. • Contributes to the formulation of clinical and administrative policies and procedures and the preparation of policy and procedure manuals, as required. • May provide clinical supervision for Mental Health Technician, graduate SocialWorker students or junior staff SocialWorkers. • Monitors mental status of Senior Citizens accepted to the program. • For Mobile Crisis - Adult (4711) FLOAT Team Only: Assists other Mobile Crisis Agencies across the five boroughs with 24-hour notice. This means you'll be part of a flexible response team, ready to provide crisis intervention and psychosocial support wherever needed in NYC. This requires adaptability and strong coordination skills. • Participates in special projects and performs other duties as assigned.

• Utilizes appropriate community resources and serves as a liaison between VNS Health and other community agencies.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Supervision Responsibilities
Ensures the extent, kind, and quality of counseling performed is consistent with the education, training, and experience of the supervisee.

Patient Education

With other members of the interdisciplinary team, provides appropriate information about all treatment modalities.
Facilitates the transplant referral process and collaborates with interdisciplinary team on transplant waitlist management.
Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available.
Reviews patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them.

Patient Admission and Continuity of Care
Reviews Patient Rights and Responsibilities, Grievance Procedure & Important Numbers Handout, FKC Non-discrimination policy, DNR Statement (if applicable) and address any immediate needs/concerns.
Understands the referral and admission process and supports the clinic in regard to the patient needs for scheduling to maximize adherence and adjustment.
The SocialWorker will interview the patient to identify root causes or concerns for the discharge request, (i.e. transfer to hospice, relocation, dissatisfaction with services or staff) and share causes/concerns with operational leadership.
Insurance and Financial Assistance
Collaborates and functions as a liaison for patient with Insurance Coordinators to address issues related to insurance.
In collaboration with Financial Coordinators, provides information and education to patients about payment to dialysis (federal, state, commercial insurance, state renal programs, AKF HIPP, and entitlement programs).
Collaborates with the Insurance Coordinator of any changes to patient state that impacts insurance i.e. transplantation, discharged, loss of coverage, or extended travel.
Refers patients to patient billing solutions (PBS) department for questions/concerns in regard to treatment related bills.
Staff Related
Assists with interview process and decision to hire new personnel if requested by SW Manager/Senior Manager.
Works with the administrative support staff to maintain updated patient resource lists (e.g. maintain updated list of transportation resources).
Provides training to staff pertaining to psychosocial topics as needed.
Contributes and participates with weekly team huddles. Discusses any urgent patient issues with staff.
Adheres to work defined caseload guidelines based on state regulatory requirements.
Performs other related duties as assigned.

Qualifications:

We require:

Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.

Requirements:
Active and unrestricted license to practice as a Licensed Clinical SocialWorker in State applying for.
Effective oral and written communication in English
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.

Job Summary:

Interviews patients or family members of residents when application for admission is made.
Explains cost of care involved in appropriate level of care as determined or required by the admitting physician.
Gives appropriate forms necessary to request adequate medical and social information, which will be returned for review.
Works as liaison between families and hospital discharge planners, social service agencies, and physicians.
When appropriate bed is available and all records are in order, gives notice of time patient admission in line with admitting time in Facility policies.
Notifies the family of approval for admission, the room number and time of expected arrival of the patient.
Notifies all appropriate departments of admission.
Greets patient and family at the door. Assists family and patient in settling in room as needed.
Completes cover sheet of MDS form.
Chairs the Admission Conference/MDS meeting for new admissions.
Holds patient-family conferences as deemed appropriate.
Attends and participates in required meetings.
Assists families or residents with discharge placement and discharge information as needed. Maintains list of community resources.
Receives complaints from residents or families investigates and reports to the Department Director.
Receives and reviews change of room requests. Notifies appropriate departments of room changes.
Refers all residents or families to the appropriate agency or resource if service cannot be met within the Facility.
Works closely with Unit Directors/RN Supervisors, administrative nursing and other SocialWorkers (if applicable) for input regarding admissions and room changes.
Writes correspondence as necessary following established correspondence procedures
Monitors to assure appropriate sections on MDS completed within 14 days of patient admission. Monitors through Quality Assurance monitors per calendar.
Assures that Social work progress notes and electronic assessments completed according to documentation policies.
Assures that SocialWorker meets residents' social needs through scheduled periodic visits.
Assists with Family Council, providing topics for discussion, offering support, etc. May be required to conduct resident council.
Receives notification of residents on which there are notes precautions regarding a resident's precautionary condition and follows Facility procedure.
Participates in Quality Assurance committee by performing monitors and reporting to the Q.A. Committee.
Meets with SocialWorkers (if applicable) on a monthly basis to receive concerns, and/or suggestions to improve department. Reports concerns, solutions and suggestions to Department Director.
Encourages department in a professional manner keeping Facility objectives in focus.
Assists in providing annual services to staff.
In case of fire or fire drill
Shut off all electrical equipment.
Shut doors.
Report to Nurses Station for further instruction.
Performs other duties as assigned.
Job Requirements:
Must be a high school graduate. Must have one year of experience in the health care or long-term care field or have an equivalent combination of education and experience.

MedicalSocialWorkers focus on goal setting to establish compliance with treatment plans and overcome barriers prohibiting the appropriate level of treatment. The MedicalSocialWorker helps patients learn how to cope with chronic illnesses, new diagnoses and terminal illness. The MedicalSocialWorker assists with providing support, education, and resources for patients and families undergoing chronic or terminal illness. The MedicalSocialWorker may also assist with goal directed counseling, group intervention, conferences with patients, financial needs assessments, and discharge planning. The MedicalSocialWorker serves as a community resource liaison providing information and advocacy on the patient's behalf with appropriate local, state, and federal agencies and programs.
Specific Job Duties:
Assist patients and their families with a multitude of complex services during the episode of care including navigating the health care system and systematic levels of resources.
Provide support for patients and families who are at end of life.
Collaborate with the interdisciplinary team to provide on-going care planning for patients with complex social/environmental situations including but not limited to:
Conducts a comprehensive bio-psychosocial assessment
The bio-psychosocial assessment includes: family/support system, education, employment, living arrangements, financial/insurance, significant medical or psychosocial history, and psychosocial treatment plan and goal setting.
Assess patient's family support system, healthcare literacy, current living arrangement, and financial resources.
Assess caregivers willingness to assist in the oversight of the patients care.
Review documentation from the medical record to maintain an understanding of current clinical/medical diagnoses.
Review psychosocial history and current mental health diagnoses.
Review for barriers that may be preventing the patient from meeting personal and plan of care goals.
Conduct on-going assessments to ensure the plan of care reflects the current needs of the patient.
Participate in the daily interdisciplinary huddles.
Assessment of the patient and family's emotional needs, coping skills and level of understanding chronic or terminal disease process.
Evaluating and put efforts in place to help strengthen the patient's family support system
Provide support and education for patients and families that require or desire additional services to improve health and well-being.
Provide assistance to connect patient and family with resources in the community
Informs patients of their right to execute an advanced directive, and explains relevant state laws regarding advanced directives
Assist with completing advance directives, medical power of attorney worksheets, and POLST Forms. Provide assistance and guidance for families to establish legal guardianship of a patient, deemed incompetent to make their own health care decisions.
Assisting with adjustment to treatment plans as well as counseling and referrals
Serve as a liaison between patients and their families to help ensure they understand clinical medical information being provided to them by their primary care physician or specialist.
Finding community resources for transportation, meals on wheels, respite care and non-emergent transport.
Assisting patients with maintaining or obtaining insurance, as well as applying for financial aid. This includes interface with the VA, SSI, Medicare, Medicaid and Private Insurance, Personal assistance.
Assist families with the grieving process after loss of a loved one.
Provide families with appropriate resources available to assist with bereavement, support services and grief counseling.
MedicalSocialWorkers assist with:
Nursing home or assisted living placement to facilitate a safe care plan and meet the medical needs for patients that no longer can remain in their home.
Assist with transfers to outside hospices.
Coordinate with other medical facilities to ensure DME is available and or emergency services if traveling.
Coordinates with the hospice chaplain to arrange faith based support for patients during end of life.
Qualifications: Minimum of a masters degree in social work from an institution accredited by the Council on Social Work. Preferably be a Licensed Clinical SocialWorker. At least one year social work experience in a healthcare setting required. Knowledge of acute, chronic disease, and terminal illness preferred. Experience in community-based programs, home health and/or hospice preferred.
Aptitudes :
High degree of interpersonal communication and negotiation skills.
Ability to work effectively as a health care team member.
Vocal, visual, auditory and manual ability adequate to perform clinical social work intervention. Must be able to communicate effectively in verbal and written form in the English language.

Monitor patient progress and adjust treatment plans as needed.
Requirements:
Active and unrestricted license to practice as a Licensed Clinical SocialWorker in State applying for.
Effective oral and written communication in English
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Certified as an Alcohol and Drug Counselor is Preferred
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting
History of treating Anxiety, Depression and Adjustment Disorders.
Psych or Addictions experience is a plus.
Excellent time management and organizational skills.
