Summary:
The Mental Health Therapist I apply graduate-level counseling skills/social work under the direct supervision of an independently licensed staff member as stipulated by the Ohio Counselor, Social Worker, and Marriage and Family Therapist Board. The duties performed include the psychosocial assessment of individuals, adolescents, families, and groups and the provision of graduate-level clinical social work/counseling interventions as appropriate to client needs in the context of the department. Responsibilities:
Conducts initial comprehensive patient and family psychosocial assessment, and triages care accordingly.
Communicates with staff regarding assessment, treatment/interventions and interdisciplinary care plan through documentation and meetings such as patient rounds.
Demonstrates the knowledge and skills necessary to provide competent care for the physical, psychological, social, educational, and safety needs of the patients served regardless of age.
Provides case management for assigned patients that involves consultation and collaboration with hospital and community professionals and programs in order to provide the most appropriate and comprehensive care.
Consistent use of evidence-based trauma and risk assessment tools and application of crisis intervention skills when indicated, and determine formally the need for a referral to the Emergency Department or more suitable community agency.

Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.

Requirements:
Active and unrestricted license to practice as a Licensed Marriage and Family Therapist in State applying for.
Effective oral and written communication in English
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.
Psych or Addictions experience is a plus.

Position Overview
In collaboration with the Abdominal Transplant Team, the Transplant Psychologist plays a critical role in supporting patients through all phases of the transplant process: pre-evaluation, post-operative care, and long-term psychological support. This position involves working with transplant candidates (liver, kidney, pancreas) including living organ donation.
Primary Responsibilities:
- Pre-Surgical Evaluations:
Conduct psychological assessments for transplant candidates and living donor patients to determine readiness and identify any potential risks or barriers
- Clinical Consultation:
Provide both inpatient and outpatient psychological consultations to patients undergoing evaluation or treatment for transplant
- Therapeutic Support:
Offer individual and family therapy to assist with understanding illness and procedures, emotional adjustment to chronic disease, quality of life concerns, and adherence to medical regimens.
- Post-Surgical Follow-Up:
Deliver ongoing psychological care to patients experiencing emotional or behavioral health challenges following transplant
- Team Collaboration:
Partner with multidisciplinary transplant teams to enhance care coordination, optimize patient outcomes, and support family involvement.
Key Duties and Areas of Focus:
* Screen for substance use disorders and facilitate treatment as needed both pre and post-transplant
* Screen for psychiatric conditions and ensure timely access to care both pre and post-transplant
* Evaluate health literacy and identify cognitive impairments that may impact treatment
* Assess family and social support systems and connect patients with resources
* Evaluate transplant candidacy for liver, kidney and also pancreas transplants with a focus on psychosocial readiness and capacity for adherence
* Provide brief, evidence-based treatments that align with interdisciplinary care goals
* Address and manage psychopathology and substance use risks
* Assess patient motivation, barriers to adherence, and stage of behavioral change
* Collaborate with social workers to develop comprehensive mental health care plans
* Offer expert input to the transplant team regarding risk for recidivism or non-adherence to complex medical regimens
Qualification Requirements:
* Ph.D. or PsyD Psychology
* APA-accredited clinical internship
* Licensure or eligible for licensure as a Psychologist in Arizona
* Experienced candidates preferred; however new graduate psychologists welcome to apply

Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
Monitor patient progress and adjust treatment plans as needed.
Requirements:
Active and unrestricted license to practice as a Licensed Professional Counselor in State applying for.
Effective oral and written communication in English
Basic proficiency with technology, including electronic health records (EHR).
Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
Full Vaccination and Booster Status may be required in some facilities.
Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
History of treating Anxiety, Depression and Adjustment Disorders.
Psych or Addictions experience is a plus.

The Specialty Care Neuropsychologist will conduct comprehensive outpatient neuropsychological evaluations of older adults, primarily those receiving care in our Cleo Roberts Memory and Movement Disorders Center. Full psychometry support is provided for test administration and scoring. Common referral reasons include diagnosis of mild cognitive impairment and dementia syndromes, differential between neurodegenerative disorders, pre-deep brain stimulation evaluations, and assessment of psychiatric comorbidities. Consultation takes place at weekly interdisciplinary case conferences and among neuropsychologists. The Neuropsychologist is also engaged in the education and training of psychology graduate students, psychometrists, and other staff across departments. About the Position
We have an exciting career opportunity for an Intensive Supportive Housing Therapist who possesses a combination of skills and QMHP eligibility to provide clinical services for our Southern Oregon supportive housing programs in Medford, Oregon. The Intensive Supportive Housing Therapist position requires the ability to conduct mental status examinations and DSM-5 diagnoses. They will conduct comprehensive psychosocial mental health assessments and complete assessment reviews and updates every six months or as clinically indicated. Swing Lane Supportive Housing is an exciting, newly constructed housing program in Medford, Oregon that provides permanent, intensive Supportive Housing, as well as transitional Room & Board units for adults who have a serious mental illness.
Rogue Ridge Apartments is a brand-new, 28-unit, permanent Supportive Housing program in Ashland, Oregon. Rogue Ridge serves adults who are houseless and living with a mental illness. The program is in a beautiful three-story apartment complex with elevators, ADA units, and a community/activity room for residents to enjoy. Supportive services include on-site staff who provide peer support, skills training, tenancy assistance, community navigation, and more; 24/7 access to housing support; and tenancy services and support around move in and out, rent collection, maintenance, and repairs.

About the Position
We are looking for a Residential Treatment Therapist to join our team for our Southern Oregon region. The Residential Treatment Therapist will serve as a role model through the provision of recovery-oriented interventions to promote and nurture a healthy treatment culture. In collaboration with the Clinical Manager and Residential Therapist, they will provide individual and group treatment services such as, but not limited to, skills training, activity therapy, group therapy, and case management as indicated in the treatment plan.
The Residential Treatment Therapist will develop the competency necessary to perform comprehensive mental health assessments, including diagnosis and mental status exam. They will also develop treatment plans which center individuals' stated goals, address needs identified in the assessment, and prescribe appropriate interventions. The Residential Treatment Therapist will document treatment interventions provided, including rationale for services and progress towards intended outcome. They will also complete all documentation, including assessments, treatment plans, and individual service notes in a timely fashion in adherence with Federal standards.
