ESSENTIAL FUNCTIONS: * Responsible for the coordination of care of individuals in the Community Behavioral Health Clinic [CCBHC]. * Facilitate ongoing collaboration and communication between providers. * Supports the intake/access processes through completion of risk screening and behavioral health/SUD screenings with individuals. * Links individuals with needed resources and services and regularly ensures services are meeting needs. * Complete timely and accurate documentation of all contacts and activities utilizing an Electronic Health Record. * Completes comprehensive surveys with individuals at intake, discharge and every six months while in care. * Responsible for tracking and administering surveys as well as data entry. * Conducts primary care screening and monitoring processes and procedures.
