Coordinate care with team members in MH/COD/AOD, DCS, jail-based services, and other community partners/referral sources as needed to meet client need. Communicate with judicial team members as needed and appropriate. 3. Provide quality case management and assist clients with reaching specified goals Maintain and document client progress toward their identified goals on appropriate program forms. Facilitate/coordinate client treatment entry and/or engagement in services related to their treatment goal attainment (including assessing client's and family's needs, providing linkage and referral to appropriate resources.) Engage or re-engage client in relevant services as appropriate. 4. Complete all documentation in compliance with accepted standards and practices on time. Complete all necessary paperwork including assessments, intakes, daily and weekly notes, treatment plans and transition plans, plans of care, ASAMS, etc. based on program and agency guidelines/expectations. Follow all HIPAA requirements and federal confidentiality laws related to PHI and transportation of PHI. Review client Treatment Plan as needed, document client progress on objectives and update as needed. Provide data as applicable for progress reports. Maintain documentation as required and specified by the program grant. 5. Assist Program Coordinator/IOP Masters Clinician in collection of Quality Assurance Indicators. Complete/submit accurate time sheet daily in e-Datis. Case Managers will do call backs and other follow up to assist with evaluation of clients post-discharge. Ensure client satisfaction surveys are offered anonymously on a regular basis. 6. Case Managers will develop a plan with supervisor regarding professional development. Staff will have a valid driver's license and have or be willing to obtain ?f? endorsement. Will attend trainings as requested or assigned. Staff will attend individual supervision with Program Coordinator on an ongoing basis for additional help/support. Staff will attend treatment team meetings on a regular basis to present cases and to coordinate with other members of the team. 7. Attends all staff meetings and training, as required by standards, policies, procedures, and state health department code requirements. Minimum of four (4) hours in-service per month. Maintains vigilant surveillance of the pool and notices a potential victim in 10 seconds, and reaches/effectively rescues within 20 seconds. Maintains effective relationships with the members, participants, and other staff. Communicates rules and policies while being "firm, fair, and tactful." Reports suspicious and inappropriate behaviors and policy violations. Follows incident reporting requirements. MARGINAL JOB FUNCTIONS Respond to guest inquiries using the company's service recovery guidelines Presents professional appearance and attitude at all times and maintains a high standard of customer service. QUALIFICATIONS Minimum age of 15 years. Demonstrate lifeguard skills per standards, policies, and procedures. Current YMCA Lifeguard or Red Cross Lifeguard certification Current/or must obtain before first scheduled shift: ASHI certifications for CPR for the Professional Rescuer, Basic First Aid and Emergency Oxygen (Administration), Workplace Harassment and Bullying, Universal Precautions, and New Staff Orientation Assessment. Ability to relate effectively to diverse groups of people from all social and economic segments of the community. Ability to read, interpret, retain and teach safety rules, maintenance instructions, and aquatics education manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of children, parents, members, and other staff, as well as one-on-one situations. Possess excellent organization and project management skills, leadership skills, and the ability to manage and complete multiple tasks simultaneously. Possess exceptional guest relations skills Weekend hours are required Ability to react calmly and effectively in emergencies. Skill in the application of lifeguarding surveillance and rescue techniques. Ability to pass a pre-employment physical skills evaluation as stipulated by the department. Ability to prepare routine administrative paperwork. Ability to follow routine verbal and written instructions. Possess adequate vision and hearing to safely monitor and aquatic environment and children. Ability to remain alert, continuously scan all pool areas with a clear vision, and perform strenuous physical tasks necessary for a water rescue. Ability to hear voices, distinguish distress signals and communicate verbally, including projecting the voice across distance in both every day and loud situations. This position is an extremely physically active position.
