Responsible for integrating healthcare and health education in the Continuum of Care process. Order/perform and interpret routine laboratory tests. Develop a health promotion and maintenance plan, including disease prevention, health education, and counseling. Provide care for selected complicated patients as directed and supervised by department physician(s). Arrange referrals as needed to other members of the health care team in accordance with SHS policies/procedures and/or third-party payers Successful Candidates will have: Master of Science in Nursing or a Doctoral Degree Program 2-3 years experience in Geriatric setting preferred. Registered Nurse in the State of Missouri and Kansas with an ARNP License. As a Day Center CNA for Swope Health Services, you will be part of a team of caregivers to provide care for the people who need it most. The Day Center CNA provides quality care to participants, under the supervision of the Supervisor of the Day Center and nurses in accordance with PACE KC policies and procedures. You will also perform related tasks in accordance with the participant's plan of care. Primary Responsibilities: Consults daily with nurses and providers in managing participant flow. Assists in prioritizing the scheduling of participants. Schedules necessary appointments for participants for clinic, specialists, and outside medical appointments as requested. The Registered Dietician designs and implements the participant nutrition care and education program. The Registered Dietitian evaluates the participant's nutritional status, and needs, develops and implements a nutritional plan of care in collaboration with the interdisciplinary team.
