PACE KC is in search of a compassionate and dedicated Home Health Aide to be a part of a team of care givers that provide care for those who need it the most. The Home Health Aide provides quality care to participants, under the supervision of the Home Care Coordinator and nurses in accordance with PACE KC policies and procedures. The Home Health Aide performs related tasks in accordance with the participant's plan of care. Maintains flexibility in schedule and responds to unexpected participant care needs and changes in workload and location of work as directed.
Primary Functions:
1. Possesses effective communication skills to act as a companion and provide emotional support to participants and caregivers.
2. Possesses interpersonal skills that promote interdisciplinary collaboration and communication.
3. Identifies need for intervention by other members of the IDT on an ongoing basis and reports to primary nurse.
4. Provides input into interdisciplinary evaluation of the effectiveness of care
5. Performs and/or assists the patient/family with home maintenance activities as according to Plan of Care including light housekeeping and meal preparation.
6. Performs support activities as indicated on the plan of care including but not limited to supporting independence, providing companionship, and providing respite for families
7.Establishes therapeutic relationships with participants and families from a variety of cultural, ethnic and socioeconomic backgrounds.
8..Provides emotional support to participants and families in a manner that fosters independence and affirms existing strengths.
9.Maintains up-to-date and appropriate documentation in the participant's electronic medical record.
10.Participates in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.
11.Participates in in-service training and staff meetings as required.
12.Performs other duties as assigned.
Job Related Competencies:

Qualifications and Experience:
1. Medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact.

3. Previous experience working in hospice, home health, or long-term care.
4. (1) year working with the elderly population.
Certification & Licensure Requirements:
Current American Heart Association Basic Life Support (BLS) with Automated External Defibrillator (AED) certificate or obtain within two weeks of hire, before having direct participant contact.

Primary Functions:

POSITION SUMMARY: The Advanced Practice Professional (APP) has coordination and clinical skills required for the comprehensive management of cardiology patients, (including biological, social and psychological assessments).
The APP coordinates involvement of medical/surgical specialty teams necessary to develop a comprehensive plan for the patient and reviews the plan of care with the other members of the clinical team, participates in directing that care, performs diagnostic/therapeutic procedures, peri-operative services, and coordinates ambulatory management and/or discharge planning/follow-up (as applicable). The APP participates in the education of residents and medical/nursing students.
Provide healthcare services consistent with state law and the requirements of the professional licensing or certification authority and/or hospital privileges, where applicable. Such service may include but are not limited to.
MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Graduate of a CCNE accredited nursing program with Master's Degree in Nursing
2. National Certification by one of the recognized APRN Certification Centers

1.Manages comprehensive and individualized care for obstetrics/gynecology patients and their families in the inpatient phase of hospitalization (as applicable) and in the ambulatory setting.
2.Completes daily rounds as applicable; consults and/or pre-procedure/evaluations (as applicable). Provides both urgent and chronic management of OB/Gyn patients in the ambulatory setting.
3.Completes integrated assessments.
4.With the collaboration of obstetrics/gynecology faculty or by following established clinical Guidelines and Protocols, orders additional tests or consults as deemed appropriate.
5.Performs and communicates results of diagnostic tests following established clinical guidelines and scope of practice.
6.Consults and communicates with clinic and hospital staff regarding abnormal findings in the evaluations of patients.
7.Performs succinct comprehensive clinical evaluations and documents findings in medical record.
8.Facilitates patient discharge and/or follow-up by coordinating appropriate services and tests necessary for outpatient follow-up appointments in the clinic (as applicable).
9.Performs necessary procedures following established clinical guidelines and Protocols and scope of practice (SOP).
10.Develops a comprehensive individualized plan of care for obstetrics/gynecology patients, which includes teaching, disposition planning and necessary referral to other members of the health care team as necessary.
11.Demonstrates expertise in caring for patients across the age continuum of their educational experience and certification.
12.Participate in physician rounds, as applicable.
13.Identifies unstable or complex obstetrics/gynecology patient care situations and facilitates appropriate intervention in the clinic and/or hospital setting.
14.Recognizes emergency situations and initiates effective emergency care.
15.Evaluates obstetrics/gynecology patients and serves as a coordinator for follow up procedures and tests in the ambulatory setting (as applicable).
16.Assists in the collection of outcome measures (as applicable).
17.Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation and management of the individual needs of obstetrics/gynecology patients/families.
18.Demonstrates the ability to utilize equipment in a safe manner.

20.Utilizes teaching/learning theory and appropriate teaching strategies in the provision of education to nursing staff, hospital staff, students, patients, families and the community.

22.Evaluates the effectiveness of patient/family teaching and collaborates with the nursing and clinical staff in the completion of education plan.

30.Documents the plan of care, interventions and evaluations in a clear and concise manner for every patient visit in the patient's medical record following approved hospital/UHA policies and compliance standards for documentation.

38.Documents hours and submits billing reports, as appropriate.
39.Participates in the WVUH/UHA Performance Improvement Plan.
40.Applies information from Performance Improvement activities, changing practice as necessary, to improve care provided to patients.
41.Reviews and interprets obstetrics/gynecology data from sources such as patient registries, clinical and financial reports, length of stay, access to care, and cost per case reports and recommends/implements changes as necessary.
42.Participates in clinical trials and research studies with the Principal Investigator.
43.Assists Faculty, Administrators and Directors in the collection of outcome measures.
