Description
Community Support Team
The Team Lead Case Therapist II reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
This role is responsible for providing intensive case management services to individuals with chronic and/or persistent mental illness or co-occurring disorders.
Specific responsibilities may include but not be limited to:
* Assisting clients in engaging in needed services, assisting in finding and utilizing community resources, applying for and re-determining benefits assistance, assisting clients in having basic necessities such as food, housing and safety resources.
Caseload: This is a team-based approach. The team shares the caseload, but each team member is individually assigned specific clients for consistent follow-up and support.
Population: Clients have higher needs and are at higher risk of crisis and instability than those clients engaged in traditional case management services.
Schedule:
40 hours/week, Monday-Friday, 8:30-5:00
* No overnight or weekend hours.
Qualifications
Required:
* Experience and training in assessment, individual, family, and group counseling
* Master's Degree with LSW, LCSW, LPC, or LCPC required, or eligible for licensure
* Master's degree in social work, psychology, counseling, or related field
Preferred:
* 3-5 years in intensive treatment setting and /or mental health setting.
