This role is responsible for providing intensive case management services to individuals with chronic and/or persistent mental illness or co-occurring disorders. Specific responsibilities may include but not be limited to: * Assisting clients in engaging in needed services, assisting in finding and utilizing community resources, applying for and re-determining benefits assistance, assisting clients in having basic necessities such as food, housing and safety resources. Caseload: This is a team-based approach. The team shares the caseload, but each team member is individually assigned specific clients for consistent follow-up and support.
