Navigators will respond to and schedule patients who contact Tarzana Treatment Centers (TTC) referrals from government and non-profit organizations to include county agencies, hospitals, medical groups, health centers, mental health and substance use treatment providers and others, and navigate patients to receive services at Tarzana Treatment Centers. Seeking a full-time employee as Patient Navigator to work as part of multidisciplinary team for the Enhanced Case Management, to respond to referral sources from government and non-profit organizations to include county agencies, hospitals, medical groups, health centers, mental health and substance use treatment providers and others. Duties include but are not limited to: provide enhanced patient navigation services to identify and link eligible participants to project services; Participate in outreach efforts at community sites and in-reach at TTC service site; provide follow-up as needed to engage potential participants or those who have fallen out of care. Outreach efforts will consist of in-person outings to participants homes, Primary Care Office, and or Hospitals.
Candidate will have at minimum two years working experience in Customer Service, Healthcare services, outreach, health promotion, Intake coordination, and/or similar or related fields. Candidate should have a strong working knowledge of substance use disorder and mental health treatment services, and Primary Care services. Candidates must have a willingness to participate in ongoing training in this field. Individual must be able to work efficiently independently and as part of a collaborative team. Excellent customer service, and organizational skills are a must. Valid California drivers license and insurance required. Bilingual Spanish is preferred.

SUMMARY OF RESPONSIBILITIES:
Position is responsible for completing preliminary screening to help determine level of care and services to support recovery efforts, as well to help individual access needed health and social support services. SPECIFIC DUTIES:

Job Overview

Prototypes, a program of HealthRIGHT 360's residential substance use disorder (SUD) program is a new program contracted with Department of Public Health's (DPH) Substance Abuse Prevention and Control (SAPC) program for residential drug Medi-Cal (DMC) services for up to 99 adult men and women: 33 that are for men who are judicially involved, 33 for men, and 33 for women. The BHC's residential DMC program will employ 100 people and works closely with the other BHC programs to provide a continuum of services for low income/Medi-Cal population. The program is located on the campus of Martin Luther King Jr. Hospital in the Willowbrook area of South Los Angeles and will serve all LA County residents.
The Community Outreach & Education Specialist conducts outreach directly to potential clients (street outreach, jail in-reach, shelter visits, community events) to increase awareness and understanding of SUD treatment generally, and of the services at MRT BHC specifically. Uses motivational interviewing and harm reduction skills, as well as trauma-informed approach, to encourage eligible clients to choose treatment. Also conducts outreach to other service providers and stakeholders to market the MRT BHC program, to increase awareness of its services and target population among providers who might refer eligible clients to the program. Educates community members, service providers, MRT BHC partners, and other County department personnel about SUD, treatment, and care coordination. Knowledgeable about all of the above and responsive to client, family, and stakeholder questions.
Key Responsibilities
Direct Service:
Engage potential clients at various locations (e.g., streets, community services organizations, shelters, etc.), at frequent and regular intervals to educate and motivate them to engage in SUD treatment services.
Establishes and maintains cooperative linkages with other providers (e.g., hospital emergency departments, law enforcement, public, private, and other social, economic, health, legal, vocational, and mental health partners) to make appropriate referrals that address unmet client needs.
Conduct presentations for other County departments and partners in the MRT BHC and on the MLKCH campus, as well as potential referral partners on SUD treatment including, but not limited to: the SUD treatment system, the referral process, and how to improve care coordination.
Promote culturally and linguistically relevant public awareness about SUDs and inform the community about available SUD treatment options.
May need to also provide client care hours and submit progress notes within 72 hours of service delivery.
Documentation must maintain in compliance with agency policy and procedures, HIPAA, 42-CFR, DMH, and SAPC standards.
Assists the client with intake by completing case management assessment and entering financial and benefit information.
Supports the client in apply for Medi-cal or transfer Medi-cal county when appropriate.
Assess the client's case management needs and completes all releases of information.
Connects the client to benefits, mental health, physical health, employment, probation, DCFS, employment, housing, community resources, outpatient substance use disorder services, and aftercare.

Through effective communication, Overnight Monitor relays relevant information to other members of the interdisciplinary team and performs crisis intervention as needed.
KEY RESPONSIBILITIES
Safety and Security Responsibilities: Remains alert and awake during the entire overnight shift. Performs house runs to ensure and maintain accountability of all clients and the safety and security of the facility. Coordinates fire watch with residents. This program is a fast-paced environment where communication is paramount to provide cutting edge services to men in a residential treatment setting as well as their families.
The Client Safety Navigator (CSN) fosters a safe, respectful, and welcoming environment. CSN works with program leadership, Sherriff's or local law enforcement, and other teams to respond to emergency and crisis situations, including client drug use, overdose, programmatic interruptions, and other situations.
KEY RESPONSIBILITIES
* Ensure and maintain a safe and orderly environment for clients, staff, and visitors in compliance with the agency and industry standards and safety regulations by monitoring security cameras, attending building security meetings, working collaboratively with service providers and administrative staff, by clearing areas of crowds, debris, and out-of-place items, and by performing other tasks necessary to remain compliance.

The Martin Luther King Jr. Behavioral Health Center (MLK BHC) is a program of HealthRIGHT 360 and is contracted with the Department of Public Health's Substance Abuse Prevention and Control (SAPC) program. MLK BHC offers residential Drug Medi-Cal (DMC) services for a maximum of 99 adult individuals, comprising 33 beds each for judicially involved men, men, and women. MLK BHC collaborates closely with other Behavioral Health Center programs to ensure a comprehensive range of services for low-income and Medi-Cal-eligible individuals. Situated on the Martin Luther King Jr. Hospital campus in the Willowbrook area of South Los Angeles, the program serves residents of all Los Angeles County.
The Program Director oversees all aspects of program operations within MLK BHC. Program Director provides training and leadership development for the Supervisors and Direct Service Staff. Program Director is responsible for the critical balance between providing high quality, safe services, maximizing contract and other revenues and overall fiscal responsibility. Program Director also demonstrates ownership, leadership, and strategic thinking, and motivates and inspires high performing teams.
KEY RESPONSIBILITIES
People Management:
Interview, select, hire, and terminate employees in collaboration with Human Resources.
Supervise direct reports, direct service staff, volunteers, interns, apprentices, etc.
Ensure that all direct reports follow HealthRIGHT 360's policies, procedures, position expectations, performance goals, and contractual requirements.
Provide ongoing feedback, coaching, support, and conduct formal performance evaluations in a timely manner.
Identify direct reports' strengths and weaknesses and strive to develop each team member to their fullest potential.
Deliver and arrange training and resources to ensure that direct reports are successful in their roles.

Program Management:
Provide overall leadership, coordination, and management of the program.
Create, develop, maintain, and implement systems, policies, and procedures for all aspects of service planning, development, and linkages to other resources, in accordance with county contracts, performance requirement, treatment matrix, scope of work or other program requirements as identified by the county.
Create and develop systems to track performance measures and client outcomes across programs.
Develop and implement a method of monitoring to ensure contract requirements are being met.
Keep record of all inspections conducted by the contractor, any corrective action taken, the time problem was first identified, a clear description of the problem, and the time elapsed between identification and completed corrective action, shall be provided to the County upon request.
Monitor program budget and facilitate response to ensure utilization. Ensure that all projects/programs stay within agreed budgetary limitations.
Manage risks and issues and take corrective measurements.
Implement quality improvements in the integration of behavioral health services to improve the quality and continuum of client care.
Oversee the electronic health records system for the program, including but not limited to running reports, checking for discrepancy, retention, productivity, utilization, and ensuring all discrepancies are corrected in a timely manner.
Treatment Oversight:
Ensure that the program staff provide strength-based, trauma-informed, client-centered approach, and culturally competent services in an ethical and legal manner and in accordance with program requirements, HealthRIGHT 360's policies and procedures, and all other regulatory requirements.
Utilize a client-centered approach when making program decisions.
Ensure that charts, treatment plans, and other documents are accurate and in accordance with program requirements and HealthRIGHT 360's policies and procedures to satisfy internal and external evaluating requirements by conducting periodic quality reviews of all charts and of staff's performance and productivity. Frequency of the review is determined by the Managing Director.
Ensure that documents and records are properly handled and transferred properly.

The Male Community Reentry Program (MCRP) is a residential facility with a capacity of 200 beds, designed for adult males who have less than two years remaining on their prison sentences. Instead of being incarcerated in a state prison, eligible individuals can complete the remainder of their sentences in the community through MCRP. The program offers a variety of rehabilitative services focused on substance use disorder (SUD), mental health care, primary care, employment, education, housing, family reunification, and social support. Treatment for substance use disorders and mental health issues provided by MCRP typically lasts from 6 to 12 months, ensuring comprehensive support for participants.
The Health and Wellness Coordinator plays a vital role in managing client medication, coordinating various healthcare appointments and services, ensuring compliance with regulatory standards, and providing exceptional care to our clients.
KEY RESPONSIBILITIES
Medication Management
* Record all new client medications into electronic systems.
* Coordinate pharmacy services for new medications and refills.
* Facilitate the destruction of medication when necessary and maintain accurate logs.
* Responsible for all aspects of medication including inventory, safe storage, safe handling, disposal, and accurate logging and tracking.
* Manage medication errors and coordinate the delivery of medication.

Direct Service:

The Intern Clinical Supervisor is a LCSW or Licensed Psychologist who provides supervision to interns within the HealthRIGHT 360 behavioral health programs (in Northern and Southern California). The Intern Clinical Supervisor ensures the quality of clinical services by providing individual and group supervision and offering didactic trainings and consultation to interns as needed. This position is also responsible for ensuring that trauma informed and culturally competent services are being provided and appropriate professional standards and licensing/certification requirements are being met.
This is a hybrid, part time position.
Key Responsibilities:
Supervision responsibilities: Supervision responsibilities include monitoring the clinical work of interns and providing individual and group supervision (in-person and remotely); maintaining appropriate supervision agreements and proof of hours; signing off on required paperwork for the California Board of Behavioral Sciences as needed; completing intern evaluations and written recommendations; and assisting with crisis intervention, mandated reporting, and consultation on other urgent clinical matters; communicating with program directors/managers around interns' progress or any areas of concern; traveling to different local program sites as necessary.
