Division Overview: The Corrections Transition Programs (CTP) is a statewide network of clinical reentry management services with the overall purpose of increasing opportunities for successful reentry outcomes. CTP provides specialized case management services to help people prepare to return to their families and communities after incarceration. CTP works both inside prisons and in communities to provide reentry support, including, but not limited to, behavioral health referrals, public benefits enrollment, finding employment and housing resources and obtaining state IDs and vital records. We are currently looking for full-time Case Manager Starting at $44,676 - $47,650; contingent upon experience, education, etc. Position Summary: This position will provide addiction intervention, AODA counseling and recovery support services to clients and their families. This position is responsible for bringing services, agencies, resources, and other people together to assist clients in achieving established goals. This position may also work with high-risk caseloads, co-occurring clients, families, and children. This position will be located in Central Illinois, and primary areas of coverage include: Kankakee, Iriquiois, Ford, & Vermillion counties. ESSENTIAL DUTIES AND RESPONSIBILITIES: * Assist individuals in achieving service plan goals through direct services and coordination of behavior health, public agencies, and community resources. * Assess, evaluate, facilitate, and monitor individuals progress in relationship to established client center service plan and conditions of parole * Complete intake packets for program enrollment and participation * Provides referrals and crisis intervention for the individuals identified needs * Interview individuals, review records, and confer with other professionals to review individual progress, and to determine their suitability for program participation * Collaborate with internal and external partners, participate in staff meetings and monthly supervisions, and participate in integrated staffing with individuals receiving TASC services. * Identify and refer individuals with behavioral health needs * Complete and submit internal and external program reports * Complete and maintain all required individual and program related documentation which includes, documenting the results of screenings, enrollments, service plans, reports, progress notes, discharge summaries and other individual-related activities including accurate reporting of services * Monitor all individuals determined by individual progress towards service goals * Assist clients with the enrollment in public benefit systems and obtaining vital records * Monitor TASC database system and contractual/external database systems to ensure program compliance * Develops effective working relationships with the internal and external partners * Participate in meetings to adjust, update, or revise client centered service plans * Obtain and clinically analyze progress reports from internal and external partners to ensure individuals progress is on target with the benchmarks outlined in their service plan * Ensure that confidential information relating to the organization, its staff, and individuals is kept confidential * Be an excellent steward of TASC, modeling the core values of the organization Our Ideal candidate: * High school diploma or GED equivalent * Valid driver's license * CADC certification within two years of employment with TASC may be required based on contractual obligations * Must be able to pass an IDOC Background check * Demonstrated proficiency in the Microsoft Office suite * Demonstrated experience in group facilitation * Knowledge of the current ASAM placement criteria, DSM-V as it relates to substance-related disorders, and Code of Ethics for substance abuse professionals, or the ability to acquire the knowledge * Comfort working inside a correctional setting and with persons convicted of serious violent offenses * Flexibility in thinking and approaches to leading group discussions * Good verbal, written, and interpersonal communication skills * Good time management skills with a proven ability to meet deadlines Our benefits package includes: * Medical/Dental/Vision/Life Insurance and Flexible Spending * Paid Leave - Short-term Disability (STD) * Paid Time Off/Sick Time/ Floating Holiday * Tuition Reimbursement * 403 B (retirement plan) The agency currently offers hybrid work schedules that combines in-office and remote work. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES: * Collaborate with system partners starting in Cook and Will Counties and continuing in jurisdictions throughout Illinois to integrate court clerk and other tracking systems and/or their data with TASCs CMR; create bridges between siloed data sources and systems. * Develop and implement systems of interoperability between TASCs CMR, Twilio, and other platforms as needed. * Work with the implementation team on the customization of new CRM software to ensure adequate mapping and data collection for pre-trial programming. * Provide ongoing feedback on potential improvements in CRM architecture and management to ensure appropriate data collection and analysis. * Manage/oversee all aspects of record-keeping for clients, including collection, digitization, and filing client records. * Supervise C: VCSP Data Coordinators. * Assist with initial and ongoing training of staff in data entry and continual quality improvements. * Work with project/program leadership to ensure data gaps and inconsistencies are reconciled on a regular basis * Assist in the optimization of operational processes and systems to ensure efficiency and high-quality delivery of the C: VCSP model data infrastructure in collaboration with funders, PQI, and Operations. * Support PQI, directors, and finance in the management of funding and account expenditures as they relate to program administration * Support TASC leadership and program partners in developing and presenting regular updates on program outcomes and utilizing data to inform best practices, mitigation strategies, and client support/advocacy. * Serve as the lead for Data Coordinators in managing administrative tasks, including tracking inventory and ordering & receipt of new supplies. * Generate alternative solutions to address client needs and work challenges. * Crosstrain and, when needed, fill in for other project roles. * Attend all trainings as required for agency and professional development. * Monitor the quality and timeliness of client data entered into CRM and review regularly to meet performance goals and improve program quality for clients. * Review C: VCSP files, creating reports and corrective action requirements for each file. * Create and present a summary report at monthly Audit Subcommittee meetings. * Work with program staff on corrective actions. * Conduct regular audits of data and reports to ensure quality and accuracy * Develop C: VCSP Key Performance Indicators (KPIs) in collaboration with the Director of PQI; calculate and summarize KPIs monthly; identify and suggest data-informed areas for growth and project development. * Ensure that metrics on client outcomes and staff efforts are collected and reported to TASC leadership and other program stakeholders on a regular basis or as requested. * Work with the Director of PQI and other agency staff to design performance tracking and reporting tools. * Reviews discrepancies in data entered, requests clarification or advises supervisor of issues related to challenge in accordance with agency procedures. EDUCATION AND EXPERIENCE: * Bachelors degree in public policy, public health, sociology, social work, psychology, criminal justice, or a related field, etc. from an accredited college/university, Masters degree preferred * 3-5 years of experience using data to evaluate and demonstrate a need for new programs. * 1-3 years of experience supervising staff. * Familiarity with Cook County bond court, pretrial legal procedures, Cook Countys CCC Portal, and criminal justice reform landscape in Illinois, Cook County, Lake County, and Chicago. * Experience in large scale data management, project organization, data quality assurance, and task management. * Experience working with Twilio or Textedly in a business capacity preferred. * Deep understanding of how to structure data collection, storage, and analysis. Experienced in G suite and Microsoft Excel. Background in quantitative analytics a plus. * Commitment to racial justice and building an infrastructure to support lasting policy change. * Demonstrates cultural sensitivity and ability to work with diverse staff, clients, and community. SUPERVISORY RESPONSIBILITIES This position has supervisory responsibilities. Position Type and Expected Hours of Work * is a full-time position. Days and hours of work are Monday through Friday, 8:30 a.m. to 5 p.m. with occasional evening and/or weekend hours.
