The Diabetes Education Specialist (DES) delivers comprehensive and seamless diabetes self-management education and support services that integrate clinical and self-management aspects of diabetes and cardiometabolic care. The DES is an integral part of the interprofessional team and provides collaborative, comprehensive and person-centered care and education conducive to behavior change and improved quality of life across the lifespan.The DES supports and advocates for people affected by diabetes to optimize quality care. The DES promotes self-management to achieve individualized behavioral and management goals that reduce risks and optimize health outcomes. The CDES also serves as a resource to other staff members as needed. + Provides direct patient education in an individual or group setting.
