Program Description:
The Healthcare-Community Partnerships (HCP) Capacity Building team applies best practices and innovative methods from PHS' experience in healthcare capacity building and quality improvement (QI) to the execution of scalable capacity building projects relevant to the latest healthcare reforms that attempt to improve health access and equity in New York. The team leads provider capacity building efforts for WholeYouNYC-including training, technical assistance, performance management, and QI-focused on strengthening providers' implementation of SCN services and ensuring they are delivered in a high-quality, equitable, and accessible way.
Position Summary:
We seek a relationship-driven, data-savvy Training and Technical Assistance (TTA) Coordinator to provide hands-on capacity building support, including training and technical assistance, to community-based organizations (CBOs) and other partners delivering services in WholeYouNYC's. Reporting to the TTA Manager, the TTA Coordinator will work closely with other TTA Coordinators, Quality Improvement Manager and Practice Improvement Specialists on the HCP Capacity Building team, as well as the TTA Manager and TTA Coordinators on the Provider Onboarding Team, to develop and strengthen provider partnerships, support service-delivery readiness and enhance network performance.
The TTA Coordinator will manage a portfolio of providers, serving as their primary point of contact and being accountable for their engagement, integration, and performance within the SCN. The effectiveness of this position will be directly tied to the performance of the providers they support.

This is a grant-funded position ending March 31, 2027.
Program Description:
The Healthcare-Community Partnerships (HCP) Capacity Building team applies best practices and innovative methods from PHS' experience in healthcare capacity building and quality improvement (QI) to the execution of scalable capacity building projects relevant to the latest healthcare reforms that attempt to improve health access and equity in New York. The team leads provider capacity building efforts for WholeYouNYC-including training, technical assistance, performance management, and QI-focused on strengthening providers' implementation of SCN services and ensuring they are delivered in a high-quality, equitable, and accessible way.
Position Summary:
We seek a relationship-driven, data-savvy Training and Technical Assistance (TTA) Coordinator to provide hands-on capacity building support, including training and technical assistance, to community-based organizations (CBOs) and other partners delivering services in WholeYouNYC's. Reporting to the TTA Manager, the TTA Coordinator will work closely with other TTA Coordinators, Quality Improvement Manager and Practice Improvement Specialists on the HCP Capacity Building team, as well as the TTA Manager and TTA Coordinators on the Provider Onboarding Team, to develop and strengthen provider partnerships, support service-delivery readiness and enhance network performance.
The TTA Coordinator will manage a portfolio of providers, serving as their primary point of contact and being accountable for their engagement, integration, and performance within the SCN. They will develop deep knowledge of the SCN-its services, workflows, requirements and best practices-to effectively support provider implementation and improvement. The effectiveness of this position will be directly tied to the performance of the providers they support.
Specifically, the TTA Coordinator will:
Build and maintain strong relationships with network partners (e.g., CBOs) through effective collaboration, communication and follow-through that fosters engagement, network quality and efficiency.
Serve as the primary liaison for their provider portfolio, providing responsive. support, troubleshooting issues as needed, and performance management.
Support partners in integrating SCN services and workflows into their existing programs, business models, and funding streams to enhance sustainability and service delivery.
Provide and/or triage targeted TTA to support partners' implementation of SCN services, including but not limited to live and self-paced trainings, individual and group technical assistance, office hours, roundtables and other engagement events and forums.

Support the design and implementation of TTA evaluation methods, processes and tools to measure effectiveness.
Monitor network performance metrics for their provider portfolio, proactively identifying areas for improvement and applying capacity-building and improvement best practices to drive high-quality, high-impact SCN service delivery.
Develop and implement improvement plans, as well as other quality improvement tools, to address partner performance issues.
Perform other related duties as assigned.
Qualifications and Experience:
Bachelor's degree or equivalent; Master's degree preferred.
2-3 years of experience in capacity building, training, technical assistance, quality improvement or related field.
Foundational knowledge of the human services sector in NYS / NYC and NYC-based community resource networks. Prior experience in issue related to community health, social services, health equity, access and quality, population health, or some subset/combination of these preferred.

The working conditions for this position will be primarily in an office space. The physical requirement for this position will require constant sitting, occasional standing, occasional bending, occasional walking, and occasionally lifting 10 lbs. or less.
