About the Opportunity: Setting: Residential Shift: Day Schedule: M-F 7am to 3pm OR M/W/F with Friday being 7am to 7pm Patient Population: 6 to 21 year old individuals with ID/DD Responsibilities: The Nurse will: Develop a person-centered plan of care that prescribes strategies and interventions Support discharge planning process with members of relevant teams Provide or coordinate the resident / family education Collect comprehensive data pertinent to the resident's health or the situation Perform all other duties as assigned Qualifications: 6+ months of related Nursing experience Graduate of an accredited School of Nursing NYS Nurse (RN or LPN) license BLS certification Strong understanding of the needs of vulnerable populations including youth and young adults with Autism, Developmental Disabilities, Communication Impairments, and Medical Complexity Solid assessment, clinical and documentation skills Patient oriented Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized Desired Skills: Bachelor's Degree in Nursing National Certification in area of specialty
