The Lead Telemetry Monitor Technician, under the supervision of a Registered Nurse, Director, and Clinical Manager, is responsible for conducting and documenting on-going observations of patient cardiac rhythms. S/he is the supervisory liaison between the Telemetry Monitor Techs and the Clinical Manager and/or Director.
Responsibilities
Telemetry Monitoring
* Measures telemetry rhythms every 6 hours and records data.
* Monitors heart rhythm patterns to detect, record, and report normal and abnormal heart rhythm patterns.
* Assesses interval measurements and proper and improper pacemaker/implantable defibrillator function utilizing the telemetry monitoring equipment.

EKG Operations
* Responsible for preparing medical record information, printing labels and face-sheets, making copies of rhythm sheets, and stocking supplies.
* Assesses for telemetry lead failures and replaces leads while collaborating with nursing staff to correct these failures.
* Reviews and obtains orders for 12 lead EKGs, ensuring the order is correctly entered and loaded into EMR.
* Ensures all telemetry packs have new/working batteries when utilized by patients.
* Responsible for lead placement and monitor set up for telemetry admissions.

Communication
* Obtains a list of nurses assigned to monitored patients at the beginning of each shift.
* Communicates via telephone with the assigned Registered Nurse regarding admission and discharge of patients.
* Communicates an admission rhythm and any changes to monitored rhythm; any dysrhythmias; and/or vital signs displayed on the monitor that outside of normal range.
* Communicates with Registered Nurse to discuss any alarm setting changes.
* Documents communication with Registered Nurse.

Administrative
* Communicates information from leadership to the telemetry staff members.
* Communicates staff needs to Clinical Manager and/or Director.
* Provides feedback to Clinical Manager regarding staff evaluations and assists in providing those evaluations.
* Develops schedule for Telemetry Technician staff; assists staff with schedule requests.
* Matches preceptors with newly hired Telemetry Technicians with experienced staff during the training period.
* Develops training schedule for new staff members.
* Addresses work flow issues, ensures incidents are appropriately documented and corrective steps are taken.
* Demonstrated knowledge of NAH and department policies and procedures. Works to ensure compliance of best practice amongst each staff member.

Education

Reporting to the Clinical Director of Cardiac Surveillance Unit and under the general supervision of the team facilitator and CSU leadership team, the Telemetry Technician assists the professional staff by monitoring, reporting, and documenting patient’s cardiac rate and rhythm. Properly identifies patients receiving telemetry monitoring and provides patient instruction and safe patient interactions.

1. Continually monitors and analyzes patients’ cardiac rate and rhythm, with accurate interpretation of ECG (electrocardiographic) tracing, posting tracings as changes occur and at designated times.

2. Documents ECG measurements, (PR, QRS intervals), rates and rhythms on computerized documentation and creates the Medical Record document.

3. Communicates to healthcare team the patient’s basic ECG rhythm, immediately notifies the healthcare team of life-threatening dysrhythmias.

4. Performs electrocardiograms as requested, using correct lead placement and inputing data into computer. Assists with ECG completion on off shifts and with pediatric and infant patients under the supervision of those departments.

5. Updates each patient’s telemetry history, to enhance the capabilities of our monitoring equipment which then can accurately identify rhythms. Reviews the 72 hour history of patient information at intervals, analyzes ST segment changes and alerts caregivers to significant events.

6. Initiates and discontinues patients for telemetry monitoring. Assesses appropriateness of monitoring by evaluating stability of rhythm, duration of monitoring and knowledge of patient condition per healthcare team. Logs data and requests for continuation/ discontinuation of telemetry in the patient’s medical record.

7. Enters daily telemetry charges. Maintains accurate electronic log of all patients on telemetry.
8. Serves as preceptor/ resource to orientees’ students learning interpretation of dysrhythmias and function of monitoring equipment.

9. Assumes responsibility for identifying areas in need of improvement including work processes and purchased monitoring equipment. Performs audits regarding emergency notification procedure for telemetry. Inventories telemetry equipment daily, assists with restocking, and relays the need for additional supplies.

10. Adheres to all Northwest Community Hospital standards, policies and procedures.
