Responsibilities The Clinical Nurse Care Coordinator collaborates with clinic team members to provide evidence-based patient-centered care to patients, helping them to stay up to date on their care plans and related goals towards improving health. The Clinical Nurse Care Coordinator has a special focus on high-risk medically complex patients, patients on specific disease registries, patients with disorders or chronic physical or intellectual disabilities and patients in care transitions (i.e. pediatric to adult care, recent inpatient admissions and discharges, ED follow-up, care facility discharges). This RN acts as a liaison with patient's family members and other agencies, facilitating improved communication with the aim of providing comprehensive whole person care. The Clinical Nurse Care Coordinator stays up to date on standards, assisting with programs and processes. Qualifications Associate Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program. Current Ohio Registered Nurse license. CPR/BLS certified. Knowledge of triage procedures along with clinic, physician office and/or urgent care experience. Ability to maintain quality, safety, and/or infection control standards. Knowledge of appropriate procedures and standards for the administration of medications and patient care aids. Essential Functions: Engage with the member in a variety of community-based settings to establish an effective, care coordination relationship, while considering the cultural and linguistic needs of each member. Conduct comprehensive assessments to identify the physical, mental, and socials needs of dual-eligible individuals. Key Responsibilities: * Client Interaction: * Contact clients to set up visit appointments. * Introduce yourself and provide essential materials about AnswerCare. * Conduct initial and ongoing site assessments, including personal information, home condition, and safety features. * Gather detailed information on the client's medical history, mental and behavior conditions, routines, habits, and preferences to create a comprehensive care plan. * Make necessary revisions to the care plan as patient status and needs change. * Maintain open communication with nursing shifts, schedulers, supervisors, families, and case managers. * Assessments and Monitoring: * Conduct assessments every 60 days, combined with Satisfaction Surveys. * Record vital signs during visits and note any significant changes. * Conduct unplanned assessments in case of emergencies or as requested. * Monitor client condition and supervise caregivers to ensure adherence to the care plan. * Advocate for clients to ensure their rights are met and maintain client confidentiality. * Communication: * Effectively communicate relevant information to enhance the care plan. * Prepare clinical notes and update the RN regularly, at least every 60 days. * Coordinate with Case Managers to adjust the care plan as needed.
