A Brief Overview
This is a Full Time 40 hour a week position - all onsite. It requires 5 years of previous Nursing work experience. Candidates with previous hospital Casemanagement experience or home care experience is preferred. This position offers a $2500 sign on bonus.
Under the general supervision of the Director, the CaseManager provides clinically-based support to the delivery of effective and efficient patient care. Paces each case from physiological and economic perspectives. Has overall accountability for the utilization management and transition management for patients within the assigned caseload. Partners with Social Workers and collaborates with other health care team members to identify appropriate utilization of resources and to ensure reimbursement. Uses criteria to confirm medical necessity for admission and continued stay. With the patient, family and health care team, creates a discharge plan appropriate to the patient's needs and resources.
What you will do
* Determines medical necessity, appropriateness of admission, continuing stay and level of care using a combination of clinical information, clinical criteria, and third party information. Intervenes when determinations are not in alignment with clinical information, clinical criteria or third party information to resolve the situation. Documents all casemanagement interventions in the current electronic system (EPIC).
* Validates admission and continuing stay criteria with third party payers as well as the Attending Physicians. Recommends alternative care sites where appropriate.
* Collaborates with third party payers to prevent denial of payment and proactively addresses issues contributing to a potential denial. Intervenes to prevent the denial when possible.
* Supports the effective prevention and management of denials, including providing requested information as part of the appeal process.
* Assesses the patient and family for continuing care needs to develop, implement and evaluate an effective discharge plan in collaboration with the multidisciplinary team. Uses knowledge of usual length of stay to initiate a plan for discharge.
* Collaborates and communicates with patients/families related to reimbursement issues and to create a discharge plan. Supports the process of patient choice in establishing a discharge plan.
* Uses clinical knowledge and knowledge of anticipate response to treatment to assess patient progression towards anticipated outcomes. Communicates and coordinates with the patient/family and health care team to intervene when progression is stalled or diverted. Addresses actual/potential barriers to discharge
* Completes the interventions necessary for discharges to home with self-care, home with services or short term skilled nursing facility placement. Assembles necessary referrals, discharge summaries and pertinent information for discharge to home, placement, or transfer prior to the day of discharge.
* Actively contributes to, participates in, and follows through on interventions identified in care coordination and complex patient rounds.
* Identifies high risk patients and creates a collaborative plan to address their unique needs.

The Healthy MOMs CaseManager is responsible for providing comprehensive casemanagement services to address the opioid epidemic as it effects pregnant women, newborns and their families. This position will collaborate with community programs to assist with pregnancy related issues, work closely with all Healthy MOMs partners, and report to The Wright Center clinical staff. Responsibilities include a range of client-centered services that link clients with medically assisted therapy (MAT), primary care, psychosocial, and other services. DUTIES & ESSENTIAL JOB FUNCTIONS
Demonstrates professional interdisciplinary skills and provides comprehensive Behavioral Health CaseManagement services to clients (including treatment adherence counseling)
Develops and implements comprehensive, individualized service plans
Follows up with assigned clients to provide support, monitor progress on service plan, and provide access to resources
Provide ongoing support and resources to clients in unstable living situations
Work with interdisciplinary team and clients to develop individualized casemanagement plans
Maintains client confidentiality per The Wright Center policy and HIPAA standards
Serves as client advocate. Defends and preserves the rights of patients pertaining to all aspects of their care, knowledge, and privacy
Client home-visits may from time to time be necessary to accomplish service plan.
Actively participate in client and program-related trainings
Provides for continuity of effective client and family care by acting as a liaison/communication link between the interdisciplinary team and staff of substance abuse agencies, correctional institutions, specialty offices, visiting nurse associations, and local service organizations
Works with the interdisciplinary team to identify unmet needs that may impact adherence (homelessness, substance abuse, psychiatric illness, acute medical problems, lack of or inadequate health care benefits, etc.) and assists team in prioritizing those needs
Participates in interdisciplinary team meetings as scheduled
Assists program staff in data collection and fulfilling grant requirements. Performs data entry as required by grants
Maintains an information/referral list of community services

Requirements
REQUIRED QUALIFICATIONS
Skills and knowledge working with individuals, families, and communities to psychosocial issues that emerge during the period from pre-pregnancy through an infant's first year of life
Five years' experience in substance abuse care and services
One year of experience working with clients/patients with substance abuse disorders and mental health disorders
Experience with linking clients/patients up with supportive services in the community
Bachelor's degree (minimum) in social work, psychology, sociology, counseling, or other related field. LSW a plus.
5 years' experience in substance abuse care and service.
Understanding of medical and psychosocial issues related to opioid use disorders.
Have knowledge of, and contact with, health care entities, social service agencies and public entitlement programs in immediate and surrounding communities.
Have knowledge of service costs and budgetary parameters and be fiscally responsible in carrying out all casemanagement functions and activities.
Ability to work with diverse populations and with critical and emergency situations.
Sensitivity to cultural and economic differences.
Valid Driver's License
