Essential Duties:
Communicate clearly, kindly, and effectively as a primary representative of Tendercare Home Health.
Acts as the main point of contact for patients and employees regarding schedules which can include hospitalizations, call-offs, etc.
Build patient schedules that align with the patient's health insurance benefits (will be provided).
Clear alerts in Tendercare's electronic medical records system, CellTrak.
Collaborate with other departments to provide top quality, kind, and compassionate support to Tendercare patients, families, and employees.
Must strictly adhere to the Health Insurance Portability and Accountability Act (HIPAA) requirements regarding privacy and security of health information of clients of Tendercare.
Participate in a rotating Sunday on call schedule (8 a.m. Sunday to 8 a.m. Monday). Schedulers will also take turns covering on-call shifts on holidays. One scheduler should not do more than 2 holidays per year.
Performs other duties as assigned.
Required Qualifications:
Excellent verbal and written communication skills.
Must be a strong multitasker with exceptional follow-up skills.
Exceptional interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
Associate degree or equivalent experience preferred.
Strong attention to detail within multiple platforms.

Experience with medical records systems or similar software is preferred.
Ability to sit at a desk and work on a computer for extended periods (up to 8 hours per day).

Attendant Caregiver Job Statement:
Tendercare manager or the manager's designee will evaluate the competency of an employee who will perform attendant care services. Each attendant care services task that the agency chooses to have the employee perform must be evaluated and competency checked. The evaluation of the attendant and determination that the employee is competent to perform the attendant care task must occur before the employee performs that task for a client without direct agency supervision. All attendant care providers will be required to complete the Personal Care Attendant competency evaluation test or the Home Health Aide test. The test must be successfully completed by the attendant as indicated with a passing score of 90% or higher.
Duties:
- Assist the patient with activities of daily living (ADL's) such as personal hygiene, ambulating, eating, dressing, shaving, bathing, or showering as assigned by the nurse in the attendant care plan.
- Assist with ostomy bag changes, reinforcing dressings, g-tube and catheter care (if competency checked).
- Assist with prescribed range of motion exercises which the attendant and the patient have been taught by a health care professional.
- Assist with prescribed ice cap or collar, measuring and preparing special diets, assist with blood glucose testing as needed/ordered (if competency checked).
- Measuring and recording input and output of fluids.
- Observing for the appearance of gross behavioral changes in the patient and reporting to the RN case manager or other member of the clinical team.
- Maintain patient care areas in a clean, safe environment, adequate for the provision of care.
- Provides medication reminders and assist the patient to take medications as prescribed.
- Follow and document assignments in accordance with the attendant care plan. Only document care actually given. Falsification of visit documents is grounds for immediate termination. It is also against the law and prosecution may ensue.
- Transfer patients in a safe manner, in accordance with standards of care.
- Attendants are members of the interdisciplinary team. The attendants must report changes in the patient's condition to a RN or other appropriate skilled professional.
- Electronically document care provided on each visit/shift. Call any shift time changes to the office. Call for authorization for extending a shift or adding a shift to assure that there is a payer source for the care. All hours/shifts must be assigned and approved by the Tendercare office prior to working. Failure to do so can lead to disciplinary action.
- Employees must be able to pass a fit-to-work physical exam including the essential functions including the ability to bend, lift 50 pounds, push, pull, walk, stand, climb stairs, full range of motion of all extremities, and able to get up and down from the floor. Additionally, free from communicable disease.
- Any Tendercare employee that raises reasonable belief based on objective evidence that their ability to perform basic job functions is impaired, and that they pose a direct threat (to themselves or to their patients) will be referred to Occupational Health for a Fit-to-Work physical exam.

Attendant Caregiver Job Statement:
Tendercare manager or the manager's designee will evaluate the competency of an employee who will perform attendant care services. Each attendant care services task that the agency chooses to have the employee perform must be evaluated and competency checked. The evaluation of the attendant and determination that the employee is competent to perform the attendant care task must occur before the employee performs that task for a client without direct agency supervision. All attendant care providers will be required to complete the Personal Care Attendant competency evaluation test or the Home Health Aide test. The test must be successfully completed by the attendant as indicated with a passing score of 90% or higher.
Duties:
- Assist the patient with activities of daily living (ADL's) such as personal hygiene, ambulating, eating, dressing, shaving, bathing, or showering as assigned by the nurse in the attendant care plan.
- Assist with ostomy bag changes, reinforcing dressings, g-tube and catheter care (if competency checked).
- Assist with prescribed range of motion exercises which the attendant and the patient have been taught by a health care professional.
- Assist with prescribed ice cap or collar, measuring and preparing special diets, assist with blood glucose testing as needed/ordered (if competency checked).
- Measuring and recording input and output of fluids.
- Observing for the appearance of gross behavioral changes in the patient and reporting to the RN case manager or other member of the clinical team.
- Maintain patient care areas in a clean, safe environment, adequate for the provision of care.

- Attendants are members of the interdisciplinary team. The attendants must report changes in the patient's condition to a RN or other appropriate skilled professional.
- Electronically document care provided on each visit/shift. Call any shift time changes to the office. Call for authorization for extending a shift or adding a shift to assure that there is a payer source for the care. All hours/shifts must be assigned and approved by the Tendercare office prior to working. Failure to do so can lead to disciplinary action.
- Employees must be able to pass a fit-to-work physical exam including the essential functions including the ability to bend, lift 50 pounds, push, pull, walk, stand, climb stairs, full range of motion of all extremities, and able to get up and down from the floor. Additionally, free from communicable disease.

Position Summary
Performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and agency. Qualifications
Graduate of an accredited school of professional nursing.

At least 1 year of experience with infants and NG-tube care is required.
Active CPR and First Aid Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
Effective written and verbal communication skills.
Good interpersonal communication skills.
Current driver's license or state ID, and reliable transportation.

Essential Functions/Areas of Accountability:
Provides professional nursing care under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines.
Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the Registered Nurse.
Recognizes and reports changes in client condition to the Case Manager and physician, as directed.
Communicates information on a timely basis, consistent with agency policy and urgency of the situation. Communication is verbal, as needed, and through complete and accurate documentation in the clinical record.
Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager.
Individuals with the license designation of "LPN M-IV" may perform venipuncture for the purpose of obtaining blood samples for laboratory analysis.
Promotes personal safety and a safe environment for clients and coworkers.
Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures.
