The APP uses evidence-based literature and works toward evidence-based practice to effect a positive change in the health and healthcare delivered to the patient with a past, current, or potential diagnosis of cancer. ESSENTIAL FUNCTIONS: I) Clinical * Assesses the physical and psychosocial status of patients. * Obtains and documents comprehensive health history. * Performs and documents complete, system-focused, symptom-specific physical exams. * Assesses for symptoms commonly seen in patients with cancer, impact of physical comorbidities on cancer symptoms and response to treatment, signs and symptoms of disease progression or recurrence, actual or potential late effects of cancer and its treatment in cancer survivors. * Conducts a pharmacologic assessment, including prescriptions drugs, over-the-counter medications, nutritional supplements, and other complementary or alternative therapies. * Recognizes deviations from normal in the physical assessment. * Orders and interprets findings of ancillary data, including laboratory and diagnostic tests. * Prioritizes findings identify urgent or highest priority patient problems. * Relays appropriate information regarding patient care to the collaborating physician. * Communicates plan of care to patient, caregiver, and other members of the health care team. * Writes prescriptions for medication, blood products based of laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols. * Administers therapeutic measures and obtains specimens in accordance with practice guidelines. * Provides management for symptoms of cancer and side effects related to cancer treatment. * Refers or requests consultation from physicians and other health care professionals. * Assists in the management related family dynamics and coping mechanisms during acute and chronic phases of patient care. * Serves as resource to other members of the health care team regarding provision of quality care. * Evaluates the quality of care provided and recommends changes for improvement. * Take call as required by clinic need. The APP uses evidence-based literature and works toward evidence-based practice to effect a positive change in the health and healthcare delivered to the patient with a past, current, or potential diagnosis of cancer. Shift: M-F ESSENTIAL FUNCTIONS: I) Clinical Assesses the physical and psychosocial status of patients. Obtains and documents comprehensive health history. Performs and documents complete, system-focused, symptom-specific physical exams. Assesses for symptoms commonly seen in patients with cancer, impact of physical comorbidities on cancer symptoms and response to treatment, signs and symptoms of disease progression or recurrence, actual or potential late effects of cancer and its treatment in cancer survivors. Conducts a pharmacologic assessment, including prescriptions drugs, over-the-counter medications, nutritional supplements, and other complementary or alternative therapies. Skills certification as required by the APPs specific role including but not limited to, BMBx, skin punch biopsies, paracentesis, and specialized accessing of chemotherapy delivery devices. Job Description: Maintain efficient operations of admixture area including safe preparation of medications and inventory management. Coordinate drug management and fulfillment support of multi-disciplinary clinic operations departments. ESSENTIAL FUNCTIONS: * Accurately retrieve medications required to fulfill medication orders * Demonstrate professional competency in reviewing orders for appropriateness and accuracy * Demonstrate proficiency in performing mathematical calculations required for determining drug dilution and final concentrations * Aseptically prepare drug solutions and products * Maintain admixture area in accordance with clean room guidelines * Interprets the appropriateness of a medication order before preparing the medication. * Evaluate critical patient information prior to beginning admixture according to policy and procedure. * Maintain knowledge of all policies and procedures related to cleaning procedures and schedules * Accurately perform inventory management and cleaning procedures * Maintain inventory levels and logs as per protocols * Receive inventory and oversee dispensing of all drugs * Collaborate with the extended healthcare team in clinic regarding patient care activities * Maintain all relevant clinical logs (i.e., cleaning, controlled substance, respirator, etc.) * Serve as a drug information resource to clinical staff * Effectively retrieve and utilize chemotherapy spill kits * Follows all safety standards for handling chemotherapy and other hazardous drugs. * Keep in good standing required license and certification * Complete on time assigned continuing education and competencies. Job Description: The Laboratory Phlebotomist is responsible for the collection of patient specimens for laboratory testing. This is executed by following proper guidelines for safe handling of blood, body fluids and hazardous chemicals, properly recording lab testing, quality control and assurance, maintaining appropriate inventory levels of reagents and supplies. ESSENTIAL FUNCTIONS: Demonstrates ability to perform clean venipunctures. Demonstrates ability to perform capillary collections. Follows proper guidelines for safe handling of blood, body fluids and hazardous chemicals. Demonstrates knowledge of procedures and practices Demonstrates knowledge of ongoing quality assurance activities in the office and documents breaches of quality as they occur. Maintains appropriate inventory levels of reagents and supplies. Be capable of identifying problems that may adversely affect test performance or reporting of test results and either must correct the problems or immediately notify Clinic Operations Manager and Clinical Laboratory Operations Manager. Must be able to document all corrective actions taken when test systems deviate from laboratory's established performance specifications. Be able to document in charts for “lab only” visits. Have complete understanding of LIS; which includes ordering test, rejecting test, locating and printing result reports. Other duties as assigned.
